Diastolic function parameters are improved by the addition of simvastatin to enalapril-based treatment in hypertensive individuals

Objective: Diastolic dysfunction (DD) is a frequent condition in hypertensive patients whose presence increases mortality and whose treatment remains unclear. The aim of this study was to investigate in a prospective, double-blinded, placebo-controlled randomized design the additive effect of simvastatin on DD in enalapril-treated hypertensive patients with average cholesterol levels. Methods: Hypertensive patients with DD and LDL-cholesterol <160 mg/dL underwent a run-in phase to achieve a systolic blood pressure (SBP) <135 mmHg and diastolic blood pressure (DBP) <85 mmHg with enalapril. Hydrochlorothiazide was added when need to achieve blood pressure control. Four weeks after reaching the optimum anti-hypertensive regimen patients were randomized to receive 80 mg simvastatin (n = 27) or placebo (n = 28) for a period of 20 weeks. Echocardiograms were performed before and after treatment with measurement of maximum left atrial volume (LAV), conventional and tissue Doppler velocities in early diastole (E, e') and late diastole (A, a'). Results: After 20 weeks, the simvastatin group presented reduction in SBP (-4 +/- 2 mmHg, p = 0.02), increase in E/A ratio (1.0 +/- 0.05 to 1.2 +/- 0.06, p = 0.03) and decrease of LAV indexed to body surface area (24.5 +/- 0.9 to 21.1 +/- 0.8 ml/m(2), p = 0.048), as compared with placebo arm. No change in systolic function and no correlation between the E/A ratio, LAV and changes in blood pressure or lipid profile were observed. Conclusions: The addition of simvastatin to enalapril in hypertensive patients with average cholesterol levels improves parameters of diastolic function independently of changes in blood pressure or cholesterol. Summary: Atherogenic dyslipidemia is associated with carotid-femoral pulse wave velocity (cf-PWV), and there is a linear association between triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), and the triglycerides to high-density lipoprotein (TG/HDL-C) ratio with arterial stiffness. HDL-C has a stronger association with arterial stiffness, while the association of TG/HDL-C with arterial stiffness is dependent on HDL-C levels.

Determinants and prognostic value of in-hospital infection in patients waiting for permanent pacemaker implantation

Summary: This study investigated the predictors and prognostic impact of in-hospital infection among patients waiting for permanent cardiac pacemaker implantation. It found that urinary catheter use, complete atrioventricular block, implantation of temporary pacemaker and diabetes mellitus were independent predictors of infection. Patients who developed infection had a higher risk of early mortality after pacemaker implantation. The use of a clinical score can help prioritize high-risk patients and reduce this adverse event.

Very low HDL levels: clinical assessment and management

Summary: Individuals with very low HDL-C levels, such as Tangier disease, LCAT deficiency, and familial hypoalphalipoproteinemia, have an increased risk of premature atherosclerosis. However, comparing populations with small variations in HDL-C due to polygenic alterations does not confirm these findings, suggesting an indirect association or heterogeneity in the pathophysiological mechanisms related to HDL-C reduction. Trials evaluating HDL functions show a stronger association between the HDL system and atherosclerotic risk, but the causal relationship is still unclear. Current randomized clinical trials aim to increase HDL-C concentration but have not shown independent reduction in cardiovascular risk. Therefore, there is a need to understand the molecular action and cellular interaction of HDL beyond plasma HDL-C concentration to mitigate atherosclerotic risk.

Summary: Advanced glycation end products (AGEs) accumulation is associated with the progression of CKD-bone disorders. This study investigated the relationship between AGEs measured in blood, skin, and bone with bone histomorphometry parameters, protein, gene expression, and serum biomarkers in CKD patients. The results showed that bone AGEs accumulation was inversely correlated with serum receptor activator of NF-KB ligand/PTH ratio, and RAGE expression was negatively correlated with serum tartrate-resistant acid phosphatase-5b/PTH.

Summary: This study found that the post-PCI FFR of the left anterior descending (LAD) artery was lower than that of non-LAD arteries, highlighting the importance of interpreting post-PCI FFR on a vessel-specific basis. Although a higher post-PCI FFR was associated with improved prognosis, its predictive capacity for events differed between the LAD and non-LAD arteries, being poor in the LAD and moderate in the non-LAD vessels.

Summary: This study compared the effectiveness and cost-effectiveness of glucose-lowering therapies (GLT) in individuals with type 2 diabetes in Brazil. The findings showed that SGLT2 inhibitors, GLP-1 agonists, and pioglitazone can reduce the relative risks of non-fatal cardiovascular events. Pioglitazone was found to be the most cost-effective treatment in both primary and secondary prevention scenarios.

Summary: This retrospective observational study compared the efficacy of intravenous hydration and outpatient oral hydration in preventing contrast-induced nephropathy in patients undergoing coronary procedures. The results showed no significant difference in the incidence of contrast-induced nephropathy between the two groups. The outpatient oral hydration protocol appears to be as effective as in-hospital intravenous hydration for renal protection.
