Summary: Patients with angina pectoris of coronary artery disease often experience moderate pain episodes and exhibit moderate to high levels of kinesiophobia. Factors such as personal monthly income, New York Heart Function Assessment classification, pain intensity, and pain resilience are associated with kinesiophobia. This study emphasizes the importance of addressing kinesiophobia in patients with coronary artery disease.

Summary: In patients with elevated cardiac troponin I (cTnI), 60% were diagnosed with coronary artery disease (CAD), 17% were suspected to have microvascular angina (MVA), and the remaining 23% had non-angina (NA). Patients with CAD had elevated blood pressure and slightly decreased heart rate, while those with NA had higher levels of high-density lipoprotein (HDL). Reduced HDL level was independently associated with the presence of CAD.

Summary: Nearly half of patients with angina have non-obstructive coronary artery disease (ANOCA), with two-thirds of these patients having coronary microvascular disease (CMD). Impaired coronary flow reserve (CFR) is associated with myocardial ischemia and adverse cardiovascular outcomes. The CMD workstream aims to improve accuracy of diagnosis and treatment through standardized procedures and various assessment methods.

Summary: Coronary artery disease is a prevalent condition with detrimental effects on affected individuals. Stable angina pectoris, its most frequent manifestation, can be challenging to manage despite available treatments. Various approaches have been developed for refractory angina pectoris, with enhanced external counterpulsation and coronary sinus reduction being recognized techniques. Clinical trial results for these techniques are encouraging but limited in sample size. Other intervention methods, such as transmyocardial laser revascularization and spinal cord stimulation, have controversial or negative results. Angiogenic therapy with targeted intramyocardial vascular endothelial growth factor injection or CD34+ cell therapy may be beneficial but further investigation is needed.

Summary: This study developed machine learning models using clinically relevant biomarkers to predict the presence of stable obstructive coronary artery disease (CAD). The best-performing model showed an accuracy of 74.6% and outperformed established pre-test probability models. This approach could improve discrimination of patients with suspected obstructive CAD and guide non-invasive testing for ischemia.

Summary: Chronic inflammation is associated with atherosclerosis development and cardiovascular events. This study investigated the influence of C-reactive protein (CRP), an important marker of inflammation, on the clinical outcomes of patients with chronic kidney disease (CKD) and stable coronary artery disease (CAD) undergoing percutaneous coronary intervention (PCI). The results showed that high CRP levels adversely affect long-term cardiac events in these patients.

Summary: The study found that angina affects almost one-quarter of stable coronary artery disease patients, but resolves without events or coronary revascularization in most cases. Patients whose angina resolved within one year with conservative management had outcomes similar to those without angina, while persistence or occurrence of angina was associated with worse outcomes.

Summary: The study investigates the use of global myocardial work (MW) combined with treadmill exercise stress to detect significant coronary artery disease (CAD) in patients with angina pectoris. The results show that peak global work efficiency (GWE) and recovery global wasted work (GWW) can predict significant CAD. A model combining peak GWE and recovery GWW performs better in identifying significant CAD compared to peak GWE alone. Overall, global MW parameters combined with exercise stress serve as an accurate noninvasive screening method before invasive diagnostic techniques.

Summary: Data on myocardial infarction treatment in patients with previous coronary artery bypass grafting is limited. Among patients hospitalized with myocardial infarction, 10.3% had previous CABG. Patients with previous CABG were less likely to receive revascularization than those without CABG, and the association between CABG and risk of death varied in STEMI and NSTEMI patients. Revascularization was associated with a lower risk of in-hospital death in patients with previous CABG presenting with both STEMI and NSTEMI.

Summary: This study systematically evaluated the clinical effectiveness and safety of electroacupuncture combined with conventional drugs in the treatment of stable angina pectoris. The results showed that compared with drug therapy alone, the combination of electroacupuncture and drug therapy was more effective in improving clinical symptoms, electrocardiography, visual analog score, and Seattle Angina Scale, with a lower incidence of adverse events. However, the limited number of high-quality literature with rigorous study design protocols may introduce bias to the study results, and further clinical trials are needed to verify the conclusions.

Summary: SCAD is a rare but well-known cause of acute coronary syndrome in women, with the role of sexual hormones in its pathophysiology. This study analyzed 245 women with SCAD, categorizing them into premenopausal and post-menopausal groups. Post-menopausal women were older, more likely to have hypertension and dyslipidemia, and were managed conservatively more often than premenopausal women, despite similar in-hospital outcomes.

Summary: The DapaTAVI trial aims to evaluate the efficacy and safety of dapagliflozin in patients with severe AS undergoing TAVI. Candidate patients should have prior history of HF admission and meet criteria such as diabetes mellitus, reduced left ventricular ejection fraction, or certain estimated glomerular filtration rate. The trial will involve 1020 patients who will be randomized to receive dapagliflozin or no dapagliflozin, with key secondary outcomes including HF hospitalization, cardiovascular mortality, and total number of HF rehospitalizations.

Summary: Heart transplantation is the gold standard treatment for advanced heart failure. Identification of the etiology, such as familial hypercholesterolemia (FH), is crucial for personalized medicine. Genetic testing revealed carriers of rare pathogenic variants in LDLR and APOB genes, leading to the diagnosis of FH in patients who may have otherwise been missed.

Summary: Familial hypercholesterolemia (FH) is a common genetic disorder associated with premature atherosclerotic cardiovascular (CV) disease (ASCVD). However, it is often not diagnosed correctly. Initiating lipid-lowering therapy (LLT) early in life can significantly reduce ASCVD risk in FH patients. Genetic testing can be useful in identifying FH.

Summary: Mitochondrial DNA (mtDNA) is different from nuclear genome in terms of inheritance patterns, susceptibility to mutations, and heteroplasmy. This study aimed to evaluate mtDNA variants associated with premature cardiovascular disease. Patients with premature myocardial infarction were compared with a control group, and the presence of mtDNA polymorphisms and clinical data were examined. The results showed that certain mtDNA mutations and heteroplasmy were significantly more frequent in premature myocardial infarction patients, particularly in males. These findings suggest that mtDNA mutations and heteroplasmy may be associated with unstable premature atherosclerosis disease in men, emphasizing the importance of protective mitochondrial pathways and preventing exposure to cardiovascular risk factors.

Summary: This study aimed to evaluate the prognostic value of myocardial strain (LVGLS) in patients with severely depressed left ventricular ejection fraction (LVEF) who underwent mitral transcatheter edge-to-edge valve repair with MitraClip. The results showed that regardless of LVEF, MitraClip was safe and improved the mid-term functional class of patients, and LVGLS can be used to select optimal candidates and predict worse prognosis.

Summary: The aim of this study is to investigate the effects of exposure to Saharan dust on airway inflammation and oxidative stress in patients with stable chronic heart failure. The study will involve 40 patients with chronic heart failure and will analyze their inflammation and oxidative stress levels.
