Summary: Rheumatoid Arthritis (RA) is a chronic inflammatory disease that can cause joint damage, deformity, and functional impairment. Shoulder involvement is less common but can be underdiagnosed if not carefully analyzed, especially in uncontrolled disease. This case report presents a 70-year-old male with difficult-to-manage RA showing severe shoulder arthritis and a giant geode in the left humeral head on MRI. The striking MRI image of a giant geode in poorly controlled RA should raise suspicion of shoulder involvement for early investigation and treatment.

Summary: The study investigated the efficacy and safety of golimumab (GLM) in elderly patients with rheumatoid arthritis (RA) and found that GLM was effective in elderly patients regardless of methotrexate (MTX) treatment status and did not significantly affect the Health Assessment Questionnaire Disability Index (HAQ-DI). GLM survival rates were comparable between elderly and younger patients, indicating that GLM may be a suitable option for elderly RA patients who cannot use MTX.

Summary: This study compared MRI-detected structural abnormalities in patients with symptomatic midfoot osteoarthritis, patients with persistent midfoot pain, and asymptomatic controls. The findings suggest a prevalence of osteoarthritis in individuals with persistent midfoot pain but no signs of OA on radiography, particularly in specific joints. However, the MRI abnormalities were not strongly associated with pain or foot-related disability.

Summary: This study used two different machine learning algorithms to identify distinct patient clusters in osteoarthritis population, in order to enable the development of effective therapies. Both deep embedded clustering and multiple factor analysis with clustering approaches identified comorbid patients with higher BMI and low physical activity, as well as young/physically active and elderly/physically inactive clusters. These findings could be helpful for early trial design to enrich for patients who may specifically benefit from disease-modifying treatments.

Summary: This study evaluated the treatment response of enthesitis in PsA patients treated with secukinumab or adalimumab for 52 weeks. The results showed that both drugs had similar efficacy in resolving enthesitis, including time to resolution. The authors concluded that inhibition of IL-17 with secukinumab reduced clinical enthesitis similarly to TNF-alpha inhibition with adalimumab.

Summary: The OMERACT Foot and Ankle Working Group is developing a core outcome set for foot and ankle disorders in rheumatic and musculoskeletal diseases, however, there is a lack of high-quality evidence on the effectiveness of treatments for these disorders. A scoping review was conducted to identify outcome domains in the existing literature and feedback was obtained on the next steps of the project. The findings from the review and the feedback will contribute to the development of the core outcome set.
