Radiographic progression is associated with resolution of systemic inflammation in patients with axial spondylarthritis treated with tumor necrosis factor α inhibitors: A study of radiographic progression, inflammation on magnetic resonance imaging, and circulating biomarkers of inflammation, angiogenesis, and cartilage and bone turnover

Objective To investigate the relationship of circulating biomarkers of inflammation (C-reactive protein [CRP], interleukin-6 [IL-6], and YKL-40), angiogenesis (vascular endothelial growth factor), cartilage turnover (C-terminal crosslinking telopeptide of type II collagen [CTX-II], total aggrecan, matrix metalloproteinase 3 [MMP-3], and cartilage oligomeric matrix protein [COMP]), and bone turnover (CTX-I and osteocalcin) to inflammation on magnetic resonance imaging (MRI) and radiographic progression in patients with axial spondylarthritis (SpA) beginning tumor necrosis factor a (TNFa) inhibitor therapy. Methods. MRIs were evaluated according to the Berlin sacroiliac ( SI) joint and spine inflammation scoring method at baseline, week 22, and week 46. Radiographs were evaluated using the modified Stoke Ankylosing Spondylitis Spine Score at baseline and week 46. Patients with new syndesmophytes were identified. Biomarker levels in patients were compared to levels in healthy subjects. Results. Higher pretreatment MRI inflammation scores for SI joints and/or lumbar spine were associated with higher baseline CTX-II levels, but not with higher levels of biomarkers of inflammation and bone turnover. During treatment with TNF alpha inhibitors, a decrease in MRI inflammation scores from baseline to week 22 was associated with larger percentage decreases in and a normalization of CRP and IL-6 levels as compared to an increase or no change in MRI scores. Development of new syndesmophytes was associated with larger percentage decreases in CRP and IL-6 levels and an increase in osteocalcin level, and with normalization of CRP and IL-6 levels from baseline to week 22. Persistent systemic inflammation was associated with radiographic nonprogression. Conclusion. Our findings indicate that inflammation on baseline MRI is associated with higher CTX-II levels. Radiographic progression is associated with decreased systemic inflammation, as assessed by IL-6 and CRP levels and MRI, supporting the notion of a link between the resolution of inflammation and new bone formation in SpA patients during anti-TNF alpha therapy.

Evidence was derived from new RCTs and recommendations were made for different patient groups based on their treatment history and response. Different classes of therapeutic options were strongly recommended for different patient groups.

Is it time to move on from pelvic radiography as the first-line imaging modality for suspected sacroiliitis?

Summary: Pelvic radiography is recommended as the initial imaging modality for suspected axSpA, but CT and MRI are superior for diagnosing sacroiliitis. Recent findings suggest that low-dose CT of the sacroiliac joints and MRI with sequences sensitive to cortical bone are more accurate and reliable than radiographs.

Evolution of Magnetic Resonance Imaging Lesions at the Sacroiliac Joints During and After Pregnancy by Serial Magnetic Resonance Imaging From Gestational Week Twenty to Twelve Months Postpartum

Summary: This study investigated the prevalence, evolution, and topography of sacroiliac joint MRI lesions in pregnant and postpartum women. The results showed that 41% of women met the current sacroiliitis definition at 12 months postpartum, which may result in false-positive diagnoses of axial spondyloarthritis in postpartum women. The lesions were mainly clustered in the anterior middle joint portions and there were virtually no erosions at 12 months postpartum.

New bone formation at the sacroiliac joint in axial spondyloarthritis: characterization of backfill in MRI and CT

Summary: By comparing CT and MRI scans of patients with axial SpA, it was found that joint space lesions show increased density and may contain calcified matrix, suggesting new bone formation. The proportion of calcified matrix gradually increases in type C lesions.

Dual-Energy-CT for Osteitis and Fat Lesions in Axial Spondyloarthritis: How Feasible Is Low-Dose Scanning?

Summary: This study aimed to evaluate the ability of low-dose dual-energy computed tomography (ld-DECT) virtual non-calcium (VNCa) images to detect bone marrow pathologies in patients with axial spondyloarthritis (axSpA). Sixty-eight patients underwent both ld-DECT and MRI for assessment. The accuracy of ld-DECT in detecting osteitis and fatty bone marrow deposition was evaluated, and correlations with MRI were calculated. The results showed that ld-DECT had limited sensitivity and specificity in detecting these pathologies, and higher radiation might be needed for more accurate analysis.

Quantification of cross-vendor variation in ADC measurements in vendor-specific prostate MRI-protocols

Summary: The purpose of this study was to quantify the variability of ADC and investigate if there were significant differences in ADC between MRI systems and sequences. A phantom model with fixed ADC values was tested in six MRI systems using different sequences. Statistical analysis revealed significant differences in ADC measurements between vendors, while the variation of ADC between MRI systems and sequences was deemed clinically irrelevant.

Summary: This is the first report to compare EULAR and national treatment recommendations for PsA patients across Europe, and the first this decade to compare ASAS-EULAR and national treatment recommendations in axSpA patients. An electronic survey was conducted with rheumatologists in 15 European countries from October 2021 to April 2022. The study found variations in adherence to EULAR/ASAS-EULAR recommendations and the use of different medications across countries, highlighting the need for updates and alignment with international recommendations.

Summary: This study compared the performance of two new web-based MRI scoring systems (KIMRISS and MOAKS) in quantification of bone marrow lesions (BML) and synovitis-effusion (S-E). The results showed that KIMRISS had higher reliability and usability in evaluating BML and synovitis. Both methods should be further compared in clinical trials of new therapies for OA.

Summary: This study investigates the differences and commonalities in the set-up, data collection, and patient-reported outcome measures (PROMs) in European clinical registries for axial spondyloarthritis (axSpA) and psoriatic arthritis (PsA). The study finds significant heterogeneity in registry design and data collection, but an increase in data completeness of PROMs in recent years. These findings may impact the results of collaborative research in the future.
