Pattern on the Antinuclear Antibody-HEp-2 Test Is a Critical Parameter for Discriminating Antinuclear Antibody-Positive Healthy Individuals and Patients With Autoimmune Rheumatic Diseases

Objective. To identify features of antinuclear antibody (ANA)-HEp-2 test results that discriminate ANA-positive healthy individuals and patients with autoimmune rheumatic diseases (ARDs). Methods. We sequentially retrieved data on 918 healthy individuals and 153 patients with ARDs after clinical assessment. ANA-positive healthy individuals for whom data were available were reevaluated after 3.6-5.0 years. An ANA-HEp-2 test result was considered positive when a clear ANA pattern was observed at 1: 80 dilution in 2 distinct commercial HEp-2 slides by 2 blinded independent observers. Results. ANAs were present in 118 healthy individuals (12.9%) and 138 patients with ARDs (90.2%). The ANA titer was higher in patients with ARDs than in healthy individuals (P < 0.001). The ANA pattern profile was distinct in the 2 groups. Nuclear homogeneous, nuclear coarse speckled, and nuclear centromeric patterns appeared exclusively in patients with ARDs. The nuclear dense fine speckled pattern occurred only in healthy individuals. The most frequent ANA pattern in both groups was the nuclear fine speckled pattern, which occurred at lower titer in healthy individuals than in patients with ARDs (P < 0.001). Anti-extractable nuclear antigen was present in 1 healthy individual (anti-SSA/Ro) and in 52 patients with ARDs (37.7%). None of the 40 reevaluated healthy individuals developed ARDs, and 29 (72.5%) remained ANA positive. All healthy individuals who became ANA negative had an ANA titer of 1: 80 at baseline. Conclusion. Our findings suggest that the titer, and especially the pattern, on the ANA-HEp-2 test strongly enhances our ability to discriminate ANA-positive healthy individuals and patients with ARDs.

Different epidemiologic profiles of systemic vasculitis between Brazil and Peru-preliminary results in two referral centers from both countries

Summary: This study compared the prevalence of systemic vasculitides between two vasculitis referral centers in Brazil and Peru. The study found that the frequency of Behget's disease, Takayasu arteritis, and giant cell arteritis was higher in the Brazilian center, while the frequency of microscopic polyangiitis and renal-limited vasculitis was higher in the Peruvian center.

Autoimmune/autoinflammatory syndrome induced by adjuvants: a focus on silicone

Summary: This review focuses on silicone-induced autoimmune/autoinflammatory syndrome induced by adjuvants (ASIA). The use of silicone breast implants (SBI) has increased worldwide, raising concerns about their potential effects on the immune system, particularly in individuals with pre-existing immune dysregulation. Despite epidemiological studies, the association between SBI and autoimmune/rheumatic disorders remains inconclusive. While explantation of silicone breast implants improves unspecific symptoms in the majority of patients with silicone-induced ASIA, the outcome appears to be different in patients with definitive autoimmune rheumatic disease (AIRD). Prospective studies evaluating the risk of flares after SBI in patients with AIRD are lacking. It is important to discuss the risk of ASIA with patients with AIRD and consider long-term safety and implant-related outcomes on a case-by-case basis for shared decision-making between patients and physicians.

A cell-based assay for detection of anti-fibrillarin autoantibodies with performance equivalent to immunoprecipitation

Summary: Anti-fibrillarin autoantibodies are useful for the diagnosis and prognosis of systemic sclerosis. A reliable cell-based anti-fibrillarin assay was developed and found to be comparable to the gold standard immunoprecipitation. Positive anti-fibrillarin results were associated with increased occurrence of certain disease phenotypes.

Antinuclear antibodies (ANA) as a criterion for classification and diagnosis of systemic autoimmune diseases

Summary: The classification and diagnosis of systemic autoimmune diseases rely on a set of criteria that include clinical, laboratory, imaging, and pathology factors. Autoantibodies play a crucial role in the classification criteria of many autoimmune diseases. HEp-2 indirect immunofluorescence assay is capable of detecting a wide range of autoantibodies. A positive HEp-2 IFA test can indicate the most likely autoantibody specificities in the sample. This paper suggests that the classification and diagnostic criteria for certain autoimmune diseases could be improved by modifying the accepted staining patterns for each disease based on relevant autoantibody specificities.

Summary: In this study, ECV304 cells were found to have self-citrullination capacity and novel cit-target proteins were identified in the autocitrullinated products immunoprecipitated by sera from RA patients. The auto-cit-prot-ECV304 ELISA showed high sensitivity and specificity for RA diagnosis.
