Summary: ERAP1, ERAP2, and LNPEP, which are aminopeptidases, play a role in autoimmune diseases. The expression of ERAP2 is increased while ERAP1 is decreased in psoriasis patients with autoimmune-linked variants of ERAP. Different layers of skin cells show different expression patterns of aminopeptidases, and in psoriasis, basal keratinocytes also express CCL2 and IL-15. In addition, ERAP2-high skin fibroblasts also express more CXCL14.

Summary: This study investigated the longitudinal association between treatment with tumor necrosis factor inhibitors (TNFi) and the progression of radiographic sacroiliitis in patients with early axial spondyloarthritis (SpA). The results showed that receiving TNFi treatment for at least 12 months in the previous 2-year period was associated with a significant decrease in sacroiliitis scores. This effect became evident 2 to 4 years after treatment initiation.

Assessment of Circulating lncRNA H19 in Ankylosing Spondylitis Patients and Its Correlation with Disease Activity

Summary: Ankylosing spondylitis (AS) is a chronic inflammatory disease that causes severe pain and stiffness in the joints. The role of lncRNA H19 in AS was investigated and it was found to play a key role in the pathogenesis of the disease. The expression of lncRNA H19 was significantly upregulated in AS patients compared to healthy controls and was positively correlated with disease activity and inflammatory markers.

Which factors are associated with bone marrow oedema suspicious of axial spondyloarthritis as detected by MRI in the sacroiliac joints and the spine in the general population?

Summary: This population-based cohort study identified factors associated with MRI lesions suggestive of axial spondyloarthritis (axSpA) in individuals aged <45 years. The presence and extension of bone marrow edema (BME) in spinal and sacroiliac joints (SIJ) were strongly associated with factors such as delivery during the last year, HLA-B27 positivity, body mass index, and back pain. These findings support the hypothesis of mechanical strain contributing to BME in the general population aged <45 years and suggest that HLA-B27 positivity may play a role in severity rather than susceptibility to SIJ-BME.

Assessment of hip involvement in patients with ankylosing spondylitis: reliability and validity of the Hip Inflammation MRI Scoring System

Emerging therapies for the treatment of spondyloarthritides with focus on axial spondyloarthritis

Summary: This review focuses on two emerging drugs, bimekizumab and filgotinib, for the treatment of axial spondyloarthritis and psoriatic arthritis. Unlike other IL-17 inhibitors, JAK inhibitors are also effective in rheumatoid arthritis. Further research is needed to determine the efficacy of these drugs in the treatment of axial spondyloarthritis and psoriatic arthritis.

Good construct validity of the CT Syndesmophyte Score (CTSS) in patients with radiographic axial spondyloarthritis

Summary: The objective of this study was to assess the construct validity of the CT Syndesmophyte Score (CTSS) for measuring structural spinal damage in patients with radiographic axial spondyloarthritis. The study compared the scores of CTSS and modified Stoke Ankylosing Spondylitis Spinal Score (mSASSS) in detecting syndesmophytes and their correlation with spinal mobility. The results demonstrated good agreement between CTSS and mSASSS in detecting syndesmophytes, and higher correlation of CTSS with spinal mobility measures.

Summary: A 68-year-old woman presented with bilateral swelling of the salivary glands, sicca symptoms of eyes and mouth, itching, fatigue and weight gain. After ruling out rheumatic diseases such as Sjoegren's syndrome, IgG4-related disease and sarcoidosis, Kimura's disease was diagnosed due to a high level of IgE. This case and a brief literature review are presented.

Facilitators and barriers for vaccination in patients with inflammatory rheumatic musculoskeletal diseases: a prospective cohort study

Summary: This study investigated the attitudes and perceived factors towards vaccination in patients with rheumatic musculoskeletal diseases, including pneumococci, influenza, and SARS-CoV-2 vaccines. The study found that most patients had good knowledge about vaccination and the facilitators were more important than barriers. Patients generally indicated that healthcare professionals' recommendations were a major motivation for vaccination. There were more barriers towards SARS-CoV-2 vaccination, with personal intrapersonal issues being the most common.

The study found that inflammation was associated with lower TBD, but lower TBD itself did not increase the risk of new bone formation at the same vertebra.

Summary: This study demonstrates the impact of disease-specific patient-reported outcomes on global functioning in patients with axial spondyloarthritis (axSpA), while objectively assessed spinal mobility scores have limited impact on patient-reported global functioning. The results also suggest that global functioning in this cohort is not highly dependent on the degree of structural damage in the axial skeleton.

Fifty years after the discovery of the association of HLA B27 with ankylosing spondylitis

Summary: The human lymphocyte antigen B27 (HLA B27) is a member of the HLA class I family of genes in the major histocompatibility complex. It is associated with ankylosing spondylitis (AS) and plays a role in the immune system's defense against microbes. HLA B27 accounts for less than 30% of the genetic load and can be found in 60-90% of axSpA patients worldwide. Its prevalence in the population is linked to the prevalence of the disease.

Dysfunctional high-density lipoprotein in chronic inflammatory rheumatic diseases

Summary: The mechanism behind the low cholesterol concentrations in chronic inflammatory rheumatic disease (CIRD) is not fully understood. This study found that CIRD patients have higher concentrations of oxidized high-density lipoprotein (HDLox), indicating impaired anti-inflammatory properties of HDL. This effect may contribute to the increased cardiovascular risk in chronic inflammatory diseases.
