Clinically Relevant Deficits in Performance Tests in Patients With Axial Spondyloarthritis

Uta Kiltz, Eerik P. Ahomaa, Salima F. E. Van Weely, David Kiefer, Bjoern Buehring, Xenofon Baraliakos, Jurgen Braun

Summary: This study aimed to assess the association between self-reported and performance-based physical functioning in patients with axial spondyloarthritis (axSpA), and to identify the most frequently impaired performance tests. The results showed that many axSpA patients had impairments in physical performance tests, particularly in tasks requiring coordination and muscle power of the lower extremity.

The influence of age on the prevalence of inflammatory and structural MRI lesions in the sacroiliac joints of patients with and without axial spondyloarthritis

Xenofon Baraliakos, Anna Kuehn, Styliani Tsiami, Uta Kiltz, Martin Fruth, Juergen Braun

Summary: Age has different influences on the MRI lesions in the sacroiliac joints of patients with and without axial spondyloarthritis (axSpA). Bone marrow edema (BME) and erosions are more frequent in axSpA patients regardless of age, while fat lesions (FL) are only significantly different in patients over 50 years old.

The effect of anti-inflammatory treatment on depressive symptoms in spondyloarthritis: does the type of drug matter?

Casper Webers, Uta Kiltz, Juergen Braun, Desiree van der Heijde, Annelies Boonen

Summary: Treating active SpA improves depressive symptoms, especially with TNF inhibitors (TNFis) which have a stronger effect mainly due to their impact on disease activity.

Emerging therapies for the treatment of spondyloarthritides with focus on axial spondyloarthritis

Juergen Braun, Uta Kiltz, Xenofon Baraliakos

Summary: This review focuses on two emerging drugs, bimekizumab and filgotinib, for the treatment of axial spondyloarthritis and psoriatic arthritis. Unlike other IL-17 inhibitors, JAK inhibitors are also effective in rheumatoid arthritis. Further research is needed to determine the efficacy of these drugs in the treatment of axial spondyloarthritis and psoriatic arthritis.

Good construct validity of the CT Syndesmophyte Score (CTSS) in patients with radiographic axial spondyloarthritis

Rosalinde Stal, Sofia Ramiro, Xenofon Baraliakos, Juergen Braun, Monique Reijnierse, Rosaline van den Berg, Desiree van der Heijde, Floris A. van Gaalen

Summary: The objective of this study was to assess the construct validity of the CT Syndesmophyte Score (CTSS) for measuring structural spinal damage in patients with radiographic axial spondyloarthritis. The study compared the scores of CTSS and modified Stoke Ankylosing Spondylitis Spinal Score (mSASSS) in detecting syndesmophytes and their correlation with spinal mobility. The results demonstrated good agreement between CTSS and mSASSS in detecting syndesmophytes, and higher correlation of CTSS with spinal mobility measures.

Bilateral swelling of the salivary glands and sicca symptoms: an unusual differential diagnosis-Kimura's disease, a rare allergic condition with a high IgE serum level-a case report and review of the literature

Jueergen Braun, Thomas Mairinger, Oliver Kaschke, Kai Behrendt, Josef Ramsbacher, Kirsten Karberg

Summary: A 68-year-old woman presented with bilateral swelling of the salivary glands, sicca symptoms of eyes and mouth, itching, fatigue and weight gain. After ruling out rheumatic diseases such as Sjoegren's syndrome, IgG4-related disease and sarcoidosis, Kimura's disease was diagnosed due to a high level of IgE. This case and a brief literature review are presented.

Facilitators and barriers for vaccination in patients with inflammatory rheumatic musculoskeletal diseases: a prospective cohort study

Ioana Andreica, Iulia Roman, Imke Redeker, Xenofon Baraliakos, Juergen Braun, Uta Kiltz

Summary: This study investigated the attitudes and perceived factors towards vaccination in patients with rheumatic musculoskeletal diseases, including pneumococci, influenza, and SARS-CoV-2 vaccines. The study found that most patients had good knowledge about vaccination and the facilitators were more important than barriers. Patients generally indicated that healthcare professionals' recommendations were a major motivation for vaccination. There were more barriers towards SARS-CoV-2 vaccination, with personal intrapersonal issues being the most common.

Inflammation, bone loss and 2-year bone formation at the same vertebra in axial spondyloarthritis: a multilevel MRI and low-dose CT analysis

Mary Lucy Marques, Nuno Pereira da Silva, Desiree van der Heijde, Rosalinde Stal, Xenofon Baraliakos, Juergen Braun, Monique Reijnierse, Caroline Bastiaenen, Sofia Ramiro, Floris A. van Gaalen

Summary: The study investigated the association between inflammation and lower trabecular bone density (TBD) in radiographic axial spondyloarthritis (r-axSpA), and whether a lower TBD increases the likelihood of 2-year bone formation at the same vertebra. The study found that inflammation was associated with lower TBD, but lower TBD itself did not increase the risk of new bone formation at the same vertebra.

Global Functioning in Axial Spondyloarthritis is Stronger Associated With Disease Activity and Function Than With Mobility and Radiographic Damage

David Kiefer, Juergen Braun, Uta Kiltz, Varvara Chatzistefanidi, Daniela Adolf, Ilka Schwarze, Maria Kabelitz, Uwe Lange, Jan Brandt-Juergens, Edgar Stemmler, Sabine Sartingen, Xenofon Baraliakos

Summary: This study demonstrates the impact of disease-specific patient-reported outcomes on global functioning in patients with axial spondyloarthritis (axSpA), while objectively assessed spinal mobility scores have limited impact on patient-reported global functioning. The results also suggest that global functioning in this cohort is not highly dependent on the degree of structural damage in the axial skeleton.

Fifty years after the discovery of the association of HLA B27 with ankylosing spondylitis

Juergen Braun, Joachim Sieper

Summary: The human lymphocyte antigen B27 (HLA B27) is a member of the HLA class I family of genes in the major histocompatibility complex. It is associated with ankylosing spondylitis (AS) and plays a role in the immune system's defense against microbes. HLA B27 accounts for less than 30% of the genetic load and can be found in 60-90% of axSpA patients worldwide. Its prevalence in the population is linked to the prevalence of the disease.

Dysfunctional high-density lipoprotein in chronic inflammatory rheumatic diseases

Sabina Waldecker-Gall, Felix Seibert, Sebastian Bertram, Adrian Doevelaar, Juergen Braun, Xenofon Baraliakos, Nina Babel, Christoph Waldecker, Linda Scharow, Nikolaos Pagonas, Timm H. Westhoff

Summary: The mechanism behind the low cholesterol concentrations in chronic inflammatory rheumatic disease (CIRD) is not fully understood. This study found that CIRD patients have higher concentrations of oxidized high-density lipoprotein (HDLox), indicating impaired anti-inflammatory properties of HDL. This effect may contribute to the increased cardiovascular risk in chronic inflammatory diseases.
