Possible mechanisms of retinal function recovery with the use of cell therapy with bone marrow-derived stem cells

Bone marrow has been proposed as a potential source of stem cells for regenerative medicine. In the eye, degeneration of neural cells in the retina is a hallmark of such widespread ocular diseases as age-related macular degeneration (AMD) and retinitis pigmentosa. Bone marrow is an ideal tissue for studying stem cells mainly because of its accessibility. Furthermore, there are a number of well-defined mouse models and cell surface markers that allow effective study of hematopoiesis in healthy and injured mice. Because of these characteristics and the experience of bone marrow transplantation in the treatment of hematological disease such as leukemia, bone marrow-derived stem cells have also become a major tool in regenerative medicine. Those cells may be able to restore the retina function through different mechanisms: A) cellular differentiation, B) paracrine effect, and C) retinal pigment epithelium repair. In this review, we described these possible mechanisms of recovery of retinal function with the use of cell therapy with bone marrow-derived stem cells.

Summary: The study compared functional and anatomic outcomes of combined pars plana vitrectomy (PPV) and phacoemulsification (phaco) versus PPV and deferred phaco in patients with full-thickness macular hole (FTMH) and no significant cataract. Results showed similar outcomes in terms of visual acuity, macular sensitivity, and FTMH closure rate at 1-year follow-up between the two treatment approaches.

Retinal function after intravitreal injection of autologous bone marrow-derived mesenchymal stromal cells in advanced glaucoma

Summary: The study reported on the use of bone marrow-derived mesenchymal stem cells (MSCs) in treating advanced glaucoma, with no improvement seen in visual acuity or visual field post-treatment. Although one patient experienced retinal detachment and proliferative vitreoretinopathy after 15 days, the other patient showed stable ERG responses throughout the study period. Further research involving modified MSCs is needed to address the lack of visual function improvement observed in the study.

Summary: This study provides a comprehensive overview of the CNGA3 variant spectrum in 1060 genetically confirmed ACHM patients, with 385 of them carrying likely disease-causing variants in CNGA3. By combining their own genetic data with literature and public database records, the researchers extended the CNGA3 variant spectrum to a total of 316 variants, with 244 of them interpreted as likely disease-causing. They also reported 48 novel likely disease-causing variants, with 24 of them being missense substitutions. Extensive in silico analyses and functional data were provided to advance the pathogenicity assessment of the identified variants.

Intravitreal bevacizumab plus propranolol for neovascular age-related macular degeneration (the BEVALOL study): a phase I clinical trial

Summary: This study aims to investigate the safety of a combination of intravitreal bevacizumab and propranolol for the treatment of neovascular age-related macular degeneration (nARMD). The results showed that there were no adverse events or signals of ocular toxicity observed during the 12-week trial. Further studies using this combination therapy are warranted.

Combined intravitreal dexamethasone and bevacizumab injection for the treatment of persistent diabetic macular edema (DexaBe study): a phase I clinical study

Summary: This study aimed to investigate the safety and effects of combined intravitreal injection of dexamethasone and bevacizumab in patients with refractory diabetic macular edema (DME). The results indicated that the combined therapy led to increased intraocular pressure and the need for anti-glaucomatous eye drops, as well as reduced central sub-foveal thickness. However, there was no significant improvement in best-corrected visual acuity.
