Vitamin D is a major regulator of mineral homeostasis through its action in the kidney, intestine, bone and parathyroid glands. On these tissues, its active form, calcitriol, acts by binding to a specific nuclear receptor that belongs to the steroid/thyroid hormone receptor family. This receptor, however, has also been identified in several additional human tissues. So, apart from its traditional actions related to calcium, vitamin D and its synthetic analogs are being increasingly recognized for their anti-proliferative, pro-differentiative and immunomodulatory activities. Low levels of vitamin D have been linked to many chronic diseases. Decreased muscle function and increased fall risk in elderly people; prostate, breast and colorectal cancers; diabetes mellitus; and other health problems have been associated to low circulating levels of 25-hydroxyvitamin D. This paper presents an overview of the available scientific evidence for the non-calcemic actions of vitamin D in humans. Arg Bras Endocrinol Metab. 2010;54(2):110-7

Clinical aspects of SARS-CoV-2 infection and vitamin D COVID-19 and the endocrine system: special issue for reviews in endocrine and metabolic disorders (Felipe Casaneuva, Editor in Chief) A. Giustina and JP Bilezikian, Guest Editors

Leonardo Bandeira, Marise Lazaretti-Castro, Neil Binkley

Summary: The potential role of vitamin D deficiency in the severity of Covid-19 infection has been suggested based on existing evidence of its effects on the immune system. While most observational and cohort studies indicate a relationship between low vitamin D levels and greater Covid severity, results from interventional studies are inconsistent. Prospective randomized controlled studies are underway to further evaluate the effects of vitamin D on the outcomes of Covid-19.

Concentrations of total, bioavailable, and free 25OHD in individuals with and without primary hyperparathyroidism and their correlations to DXA and trabecular bone score

Livia Marcela Santos, Monique Ohe, Sthefanie Pallone, Isabela Nacaguma, Ilda Kunii, Renata Silva, Cynthia Maria Brandao, Jose Gilberto Vieira, Marise Lazaretti-Castro

Summary: This study aimed to investigate the association between 25OHD and bone mass and microarchitecture in PHPT patients and controls. The results showed that PHPT patients had lower bone density and microarchitecture values compared to controls, but there were no significant differences in 25OHD concentrations between the two groups.

Summary: This study evaluated the changes in different forms of 25(OH)D after supplementation with vitamin D3 in individuals with primary hyperparathyroidism and controls. The results showed that after supplementation, total 25(OH)D levels increased in both groups, but the increase was lower in the primary hyperparathyroidism group. Furthermore, total 25(OH)D had a better correlation with parathyroid hormone (PTH) in this situation.

Serum 25 hydroxyvitamin D concentrations in individuals over 80 years old and their correlations with musculoskeletal and health parameters

Mariana Zuccolotto Foroni, Maysa Seabra Cendoroglo, Eliane Naomi Sakane, Rosangela Villa Marin-Mio, Patricia Ferreira do Prado Moreira, Sergio Setsuo Maeda, Marise Lazaretti-Castro

Summary: The study aimed to assess the serum concentrations of 25 hydroxyvitamin D in elderly individuals in Sao Paulo, Brazil and investigate its associations with musculoskeletal system, physical performance, and health markers. The findings revealed a high prevalence of vitamin D deficiency and showed positive correlations between serum 25 hydroxyvitamin D concentrations with bone mass and dynamic balance, and negative correlations with PTH levels and osteoporosis diagnosis.

Vitamin D in the older population: a consensus statement

Andrea Giustina, Roger Bouillon, Bess Dawson-Hughes, Peter R. Ebeling, Marise Lazaretti-Castro, Paul Lips, Claudio Marcocci, John P. Bilezikian

Summary: This paper presents the findings of the 5th International Conference Controversies in Vitamin D held in Italy. The conference aimed to discuss the relationship between Vitamin D and aging. The results indicate that vitamin D deficiency can lead to various skeletal and extra-skeletal outcomes, and older adults are at a higher risk due to factors such as reduced sun exposure. Strategies to prevent negative outcomes include sun exposure, food fortification, and supplementation. The most effective and practical strategy for the aging population is vitamin D supplementation.

Use of Teriparatide in Hyperphosphatemic Familial Tumor Calcinosis: Evaluating the Interaction Between FGF23 and PTH on the Phosphaturic Effect

Sthefanie Giovanna Pallone, Ilda Sizue Kunii, Renata Elen Costa da Silva, Marise Lazaretti-Castro

Summary: Hyperphosphatemic familial tumor calcinosis (HFTC) is a rare disease characterized by high levels of phosphate in the blood and deposition of calcium and phosphorus crystals. This case report describes a 50-year-old woman diagnosed with HFTC, who had a homozygous variant in the GALNT3 gene. Despite treatment with acetazolamide, sevelamer, and a phosphorus-restricted diet, her phosphatemia remained high and calcifications continued to progress. The use of teriparatide (TPTD) showed some effectiveness in reducing phosphatemia, but also led to increased calciuria.
