Summary: This study established a nomogram model to predict postoperative cognitive dysfunction (POCD) in elderly patients undergoing gastrointestinal tumor resection. The model showed good efficacy in predicting POCD risk based on factors such as age, BMI, history of cerebrovascular disease, and other variables. This nomogram model could provide an important reference for the prevention, management, and treatment of POCD.

Summary: The effects of different anesthetic methods on postoperative immune function were investigated in patients undergoing gastrointestinal tumor resection. The use of general anesthesia combined with epidural anesthesia resulted in improved postoperative immune indexes, reduced perioperative stress response, and decreased opioid use. Overall, the findings demonstrate the significance of anesthesia selection in maintaining immune function and improving patient outcomes.

Summary: This study investigated the effect of remote ischemic preconditioning (RIPC) on postoperative gastrointestinal function in patients undergoing laparoscopic colorectal cancer resection. The results showed that RIPC reduced postoperative gastrointestinal dysfunction, decreased concentrations of inflammatory factors, and improved patient outcomes.

Summary: This study aimed to evaluate the effect of preoperative administration of methylprednisolone on the incidence of delirium in older patients undergoing gastrointestinal surgery and its association with the shedding of endothelial glycocalyx markers. The results showed that methylprednisolone significantly reduced the incidence of delirium and decreased the release of endothelial degradation markers.

Summary: Spine surgery often leads to gastrointestinal motility impairment, and postoperative ileus rates range from 5% to 12%. Implementing a standardized postoperative bowel medication protocol can improve bowel function, reduce complications, and lower healthcare costs.

Summary: The study showed that postoperative BDNF concentrations significantly decreased in patients with colorectal cancer and pancreatic cancer, especially in those who required tumor resections. Additionally, pre- and postoperative BDNF levels were associated with tumor size, neuroinvasion, angioinvasion, and lymph node involvement.

Summary: This study evaluated the efficacy of alvimopan in adult patients undergoing posterior spinal fusion. The results showed that alvimopan did not accelerate the recovery of bowel function compared to placebo, and favored placebo in all outcome measures. The study suggests that alvimopan may not have a clinically relevant effect in these patients.

Summary: Upper gastrointestinal (UGI) tract cancer is one of the most common cancers worldwide, but the UGI microbiota has received limited attention. This study revealed that each gastrointestinal site has its own microbial characteristics that overlap those of adjacent sites. The microbial compositions between the UGI sites and feces showed significant differences. Helicobacter pylori played a more significant role in the microbial composition of the noncardia stomach than in that of the cardia. Gastric pH and Helicobacter pylori had similar additive effects on the microbial diversity of gastric juice.

Summary: The study shows that perioperative chemotherapy in patients with localized cholangiocarcinoma undergoing resection improves disease-free survival and overall survival. The nomogram constructed from the study provides an estimate of 3-year disease-free survival after surgical resection. Randomized trials are needed to define the optimal chemotherapy regimen and sequence.

Summary: This study investigated the effect of systemic lidocaine on postoperative quality of recovery and brain injury alleviation in patients undergoing supratentorial tumor resection. The results showed that lidocaine improved early recovery quality and had certain brain-injury alleviation effects, possibly attributed to its inflammation-alleviating and analgesic properties. This study is of great importance for neurosurgical anesthesia.

Summary: This study aimed to identify predictors of long procedure time, occurrence of complications, and long hospital stay in patients with upper gastrointestinal submucosal tumors undergoing submucosal tunnel endoscopic resection (STER) and stratify risk based on these predictors. The results showed that tumor size was an independent predictor for these outcomes, with a tumor size of 25 mm being the best cutoff point.

Summary: The authors analyzed a large surgical cohort of patients with brain metastases to examine risk factors associated with preoperative and postoperative seizures and to better understand the seizure risk factors of metastatic brain tumors. The study found that frontal lobe metastases and hemorrhagic tumors were associated with a higher risk of preoperative seizures, while checkpoint inhibitor use and parietal lobe tumors appeared to be associated with seizures at 6 months postoperatively.

Summary: The objective of this study was to identify predictive factors for persistent hydrocephalus after resection of periventricular tumors in children and evaluate the need and efficacy of cerebrospinal fluid (CSF) intervention during the perioperative period. The study found that tumor pathology, location, surgical approach, extension of resection, tumor volume, preoperative Evan's index, and preoperative CSF diversion were associated with postoperative persistent hydrocephalus. Posterior median approach, partial resection, tumor volume > 90cm(3), and no preoperative CSF diversion were identified as independent risk factors for persistent hydrocephalus. Early identification of children at risk of developing persistent hydrocephalus after surgery would help in planning CSF diversion and reducing the incidence of complications.
