Limb Apraxia in Multiple Sclerosis: Prevalence and Impact on Manual Dexterity and Activities of Daily Living

Objective: To evaluate the prevalence and impact of limb apraxia on manual dexterity and activities of daily living (ADLs) in patients with multiple sclerosis (MS). Design: Survey. Setting: University hospital. Participants: Consecutive patients (N=76) with clinically isolated syndrome, relapsing-remitting multiple sclerosis (RRMS), secondary progressive multiple sclerosis (SPMS) or primary progressive multiple sclerosis (PPMS), Expanded Disability Status Scale (EDSS) score from 0 to 6.5, and aged from 18 to 70 years were included. Interventions: Not applicable. Main Outcome Measures: Apraxia was assessed by the apraxia screen of TULIA (AST). The relationship of apraxia with ADLs and manual dexterity was evaluated using a dexterity questionnaire and the coin rotation task, respectively. Results: Overall, limb apraxia was found in 26.3% of patients (mean AST score +/- SD, 7.3 +/- 1.3; cutoff <9). Apraxia was significantly correlated with higher EDSS scores, longer disease duration, and higher age with the EDSS being predictive. Furthermore, patients with SPMS and PPMS were more apraxic than patients with RRMS. Finally, limb apraxia was significantly associated with impaired ADLs and manual dexterity. Conclusions: Limb apraxia is a frequent and clinically significant symptom contributing to disability in MS. It should therefore be evaluated and possibly treated, particularly in patients with MS reporting manual difficulties in everyday life. (c) 2012 by the American Congress of Rehabilitation Medicine

Cerebral Venous Thrombosis in Patients With Heparin-Induced Thrombocytopenia a Systematic Review

Summary: Cerebral venous thrombosis (CVT) is a common thrombotic manifestation associated with vaccine-induced thrombotic thrombocytopenia (HIT). This study aimed to review the incidence, clinical features, and prognosis of CVT in patients with HIT. The results showed that CVT is rare in patients with HIT, but HIT-related CVT has a higher risk of intracerebral hemorrhage and mortality.

Summary: This study evaluated the diagnostic accuracy of high-resolution 3D T2 sampling perfection with application-optimized contrasts (T2-SPACE) in patients with cerebral venous sinus thrombosis. Summary: This study found that bridging therapy (intravenous thrombolysis followed by mechanical thrombectomy) was associated with lower rates of good outcome compared to intravenous thrombolysis alone in minor stroke patients with large vessel occlusion. However, mismatch volume was a strong modifier of the effect of bridging therapy, with worse outcome observed in patients with mismatch volume <= 40 mL. Randomized trials should consider adding perfusion imaging for patient selection.

Summary: Based on data from a large registry, this study found that the safety of intravenous thrombolysis in very elderly patients with acute ischemic stroke is similar to that in younger patients. However, the probability of death and poor functional outcome was higher in the very elderly group.

Connecting Women in Neurosciences: A networking project to sustain mutual empowerment in young, female clinical neuroscientists

Summary: We established a peer-led networking group to address clinical, research, and gender-related challenges among young female clinical neuroscientists. Participants felt empowered and benefited from the meetings and new network. We identified challenges in combining clinical and research activities, some of which were perceived to be gender-related. Peer-to-peer networking is a low-budget intervention that encourages engagement in research activities, promotes interdisciplinary teamwork, and provides a protected environment to discuss and overcome gender-related challenges.

Deep Learning Versus Neurologists: Functional Outcome Prediction in LVO Stroke Patients Undergoing Mechanical Thrombectomy

Summary: This study collected clinical and magnetic resonance imaging data of 222 patients with large vessel occlusion stroke and used interpretable deep learning models to predict functional recovery. The model based on clinical variables and diffusion weighted imaging achieved the highest prediction performance, while adding perfusion weighted imaging did not improve the outcome prediction. The deep learning models significantly outperformed experienced neurologists when imaging data were provided.

Tablet-Based Puzzle Game Intervention for Cognitive Function and Well-Being in Healthy Adults: Pilot Feasibility Randomized Controlled Trial

Summary: This study investigates the benefits of dynamic adaptive casual puzzle games on cognitive function and well-being in healthy adults and older people. The results show that participants showed improvements in their visual attention and visuospatial measures after the puzzle game intervention. Digital games are a feasible way to train cognition, with the algorithm-based dynamic adaption allowing accommodations for persons with different cognitive levels of skill.

Hyperzographia in Neglect Exposing a Spatial Dissociation between Painting and Writing-A Case Study

Summary: This paper presents the observation of a patient who had a right hemispheric stroke and subsequently produced remarkable paintings. The patient's post-stroke productivity and the distribution of text and drawings were analyzed. The patient showed enhanced verbal and figural creativity compared to the control group. Interestingly, the paintings were predominantly located on the right side, while the text was aligned to the left side. This dissociation between writing and painting behavior was also reflected in the patient's neuropsychological performance.
