Summary: In patients with dilated cardiomyopathy (DCM), stratifying the risk for major ventricular arrhythmic events or sudden cardiac death based on left ventricular ejection fraction (LVEF) <= 35% is effective after 2 years of guideline-directed medical therapy (GDMT), but not after 6 months. Waiting 24 months before primary prevention ICD implantation may be appropriate for selected DCM patients.

The results showed a reduced all-cause mortality associated with CRT-D in patients with nonischemic cardiomyopathy, while no significant reduction was observed in patients with ischemic cardiomyopathy.

Summary: The study found apparent non-linear associations between night-time heart rate and ventricular tachyarrhythmias (VTAs) and ICD shocks. An increasing incidence of VTAs and ICD shocks was observed at both low and high levels of night-time heart rate, with a night-time heart rate of 50-70 bpm potentially being the optimal therapeutic target for the management of ICD/CRT-D recipients.

Summary: This study evaluated the long-term outcome of patients with arrhyth-mogenic cardiomyopathy (ACM) who underwent third-generation subcutaneous implantable cardioverter defibrillator (S-ICD) implantation with the intermuscular (IM) two-incision technique. The findings showed that although the third-generation S-ICD with the IM two-incision technique appeared to have a low risk of complications and inappropriate shocks (IS) due to cardiac oversensing, the risk of IS due to myopotential mainly during effort should be considered.

Summary: This study showed that myocardial scar characterization based on cardiac magnetic resonance imaging can improve risk stratification for ventricular arrhythmia occurrence in patients with ischemic cardiomyopathy who received an Implantable Cardioverter-Defibrillator (ICD). The analysis revealed that total scar mass and border zone mass independently predicted appropriate ICD therapy, suggesting the importance of myocardial scar assessment in determining the risk of ventricular arrhythmias in this patient population.

Summary: This study aimed to compare the differences in appropriate and inappropriate ICD interventions, complications, disease-related adverse events, and mortality between hypertrophic cardiomyopathy patients implanted with a subcutaneous ICD (S-ICD) and those implanted with a transvenous ICD (TV-ICD). The results showed that patients with S-ICD had a lower risk of appropriate and inappropriate ICD interventions and major lead-related complications compared to those with TV-ICD.

Summary: This study retrospectively analyzed data from patients with nonischemic dilated cardiomyopathy (DCM) treated at our hospital from January 2020 to November 2021. The study found that in addition to standard medical treatments, the insertion of an implantable cardioverter defibrillator (ICD) can significantly reduce the sudden cardiac death (SCD) rate in patients with nonischemic DCM.

Summary: HFrecEF is recognized as a distinct clinical entity with increasing prevalence, and continued guideline-directed medical therapy is crucial to promote myocardial recovery. Despite the lower risk of sudden cardiac death in HFrecEF compared to HFrEF, the benefits of continued ICD therapy need to be weighed against potential risks and limitations.
