Intrapartum signal quality with external fetal heart rate monitoring: a two way trial of external Doppler CTG ultrasound and the abdominal fetal electrocardiogram

The objective of this study was to assess the fetal heart rate (FHR) signal quality of non-invasive abdominal fetal electrocardiogram (fECG) in comparison to the Doppler ultrasound cardiotocogram (CTG) during the first and second stage of labour. This was a prospective observational study of non-invasive fECG using five abdominally sited electrodes against the traditional Doppler ultrasound CTG probe on 144 patients. Data were analysed for signal quality before and after outlier removal. Abdominal fECG signal quality was significantly better during the first stage of labour in comparison to Doppler CTG (median fECG reliability of 95.7 % vs. median 87.3 % for Doppler, p < 0.001), whereas during second stage of labour, equivalence was demonstrated (p > 0.05). For the first and second stage of labour, fECG showed 106/135 (78.5 %) and 46/98 (46.9 %) women having fetal signal loss below 20 %, respectively. Similarly, Doppler ultrasound demonstrated 104/135 (77.0 %) and 51/98 (52.0 %) women having fetal signal loss below 20 % during first and second stage of labour, respectively. The non-invasive abdominal fECG presents an improved FHR signal quality during the first stage of labour and an equivalent signal quality during the second stage.

Summary: This study aims to validate the ICH and ICH-GS scores in patients with cerebellar hemorrhage and compare the outcomes of surgical management with conservative treatment. The results showed that both ICH and ICH-GS scores have good prognostic accuracy for overall mortality and functional outcomes. Surgical management was beneficial for patients with severe cerebellar hemorrhages, indicated by high ICH scores, while conservative treatment was reasonable for patients with lower ICH scores. Therefore, ICH and ICH-GS scores are useful tools for predicting survival and outcome in cerebellar ICH patients.

Angled-tip vs. straight-tip guidewire in ERCP: a randomized, multicenter study

The Conventional Weil Osteotomy Does Not Require Screw Fixation

Summary: The Weil osteotomy is commonly performed for chronic metatarsalgia, and screw fixation is historically used for metatarsal head refixation. However, this study suggests that screw fixation is not always necessary. Retrospective analysis of 155 patients with 278 Weil osteotomies demonstrated a lower revision rate in patients without screw fixation. Therefore, regular screw fixation after a Weil osteotomy may not be justified based on the available data.

Preservation of the Subtalar Joint Determines Outcomes in a 10-Year Evaluation of Ankle Arthrodesis

Summary: Posttraumatic osteoarthritis may require surgical fusion of the ankle joint if non-surgical therapy is unsuccessful. The decision to perform fusion is based on the patient's pain and disability, radiographic imaging, and surgeon experience, without strict guidelines. This study compared outcomes of tibiotalocalcaneal arthrodesis (TTCA) and tibiotalar arthrodesis (TTA) to highlight the importance of the subtalar joint. The results showed significantly worse clinical scores in the TTCA group compared to the TTA group, indicating the need for careful consideration of generous subtalar joint fusion with planned ankle arthrodesis.

Septic History Limits the Outcome of Tibiotalocalcaneal Arthrodesis

Summary: Joint destruction necessitates tibiotalocalcaneal arthrodesis (TTCA) in cases of clinical deficits that cannot be controlled conservatively, possibly leading to sepsis. We aimed to compare the underlying etiology of posttraumatic joint destruction and the outcomes after TTCA in patients with a septic or aseptic history. The significantly worse results of S-TTCA compared to A-TTCA show the long and stressful ordeal that patients with a septic history suffer. Further attention must be paid to infection prophylaxis and, if necessary, early infection revision.

A 23-Year Observational Follow-Up Clinical Evaluation of Direct Posterior Composite Restorations

Summary: The purpose of this study was to observe the quality of posterior composite restorations after more than 23 years of service. A total of 22 patients with 42 restorations underwent two follow-up examinations. Results showed significantly worse scores for six out of seven criteria at the second follow-up evaluation. The study suggests the need for further research with extended follow-up time and regular assessments.

Summary: Background & Aims: This study aimed to investigate the transmission and characteristics of recently acquired HCV infections (RAHCs) over a 10-year period. The majority of RAHC cases were found in HIV-coinfected men who have sex with men (MSM), and internationally connected transmission networks were observed in most patients. Spontaneous clearance rates were low, and reinfection rates increased in a subset of MSM patients with high-risk behavior.
