Prevention of infant eczema by neonatal bacille Calmette-Guerin vaccination: The MIS BAIR randomized controlled trial

Laure F. Pittet, Nicole L. Messina, Kaya Gardiner, Bridget Freyne, Veronica Abruzzo, Kate L. Francis, Clare Morrison, Christel Zufferey, Peter Vuillermin, Katrina J. Allen, Anne-Louise Ponsonby, Roy Robins-Browne, Frank Shann, Katie L. Flanagan, Rod Phillips, Susan Donath, Dan Casalaz, Nigel Curtis

Summary: There is insufficient evidence to recommend neonatal BCG vaccination in all infants for the prevention of eczema in the first year of life; however, a modest beneficial effect was observed among high-risk infants. A single dose of BCG-Denmark soon after birth could reduce the incidence of eczema in infants with two atopic parents.

Should children be vaccinated against COVID-19?

Petra Zimmermann, Laure F. Pittet, Adam Finn, Andrew J. Pollard, Nigel Curtis

Summary: The debate over whether all children under 12 should be vaccinated against COVID-19 continues, with considerations including the relatively low risk of acute COVID-19 in children and the complexity of weighing the risks and benefits of vaccination. Other factors to consider include protection from long-term consequences, population-level impacts, vaccine supply, cost, and avoiding quarantine measures. Ongoing evaluation is needed due to the emergence of new variants of concern.

The Impact of Antimicrobial Stewardship in Children in Low- and Middle-income Countries A Systematic Review

Yara-Natalie Abo, Bridget Freyne, Diana Kululanga, Penelope A. Bryant

Summary: There are multiple effective strategies to reduce antibiotic consumption in children in low- and middle-income countries (LMIC). However, the impact of different antimicrobial stewardship (AMS) interventions on clinical outcomes remains unclear.

Hepatitis B vaccine co-administration influences the heterologous effects of neonatal BCG vaccination in a sex-differential manner

Laure F. Pittet, Lianne Cox, Bridget Freyne, Susie Germano, Rhian Bonnici, Kaya Gardiner, Susan Donath, Clare L. Collins, Dan Casalaz, Roy Robins-Browne, Katie L. Flanagan, Nicole L. Messina, Nigel Curtis

Summary: The co-administration of BCG and HBV vaccines during neonatal vaccination has an impact on immune responses, with BCG vaccine reducing cytokine responses to unrelated pathogens. The co-administration of HBV vaccine with BCG vaccine influences immune responses to neonatal BCG vaccination in a sex-differential manner.

Revaccination with Bacille Calmette-Guerin (BCG) is associated with an increased risk of abscess and lymphadenopathy

Paola Villanueva, Ushma Wadia, Nigel Crawford, Nicole L. Messina, Tobias R. Kollmann, Michaela Lucas, Laurens Manning, Peter Richmond, Laure F. Pittet, Nigel Curtis

Summary: The frequency and types of adverse events following initial vaccination and revaccination with Bacille Calmette-Guerin (BCG) vary worldwide. A randomised controlled trial (the BRACE trial) using active surveillance determined the incidence and risk factors for BCG injection site abscess and regional lymphadenopathy. Revaccination with BCG increases the risk of injection site abscess and lymphadenopathy.

Non-tuberculous Mycobacterial Infection of the Retropharyngeal Space

Alastair Weng, Nigel Curtis, Seilesh Kadambari, Kaya Gardiner, David Burgner, Amanda Gwee

Summary: This case report discusses a 20-month-old girl with Mycobacterium avium complex infection of the retropharyngeal space, and summarizes the literature on the pathogenesis and management of nontuberculous mycobacterial infections in the retropharynx.

An Approach to the Treatment of Children With COVID-19

Alison Boast, Nigel Curtis, Johanna Holschier, Rachael Purcell, Samantha Bannister, Christine Plover, Maidhili Chinnapan, David Burgner, Suzanne L. Boyce, Sarah McNab, Amanda Gwee

Summary: There is limited data to guide treatment recommendations for children with acute, symptomatic COVID-19. This review proposes a management approach based on published evidence, approved medications, and safety profiles of recommended drugs in this age group.

Monkeypox-What Pediatricians Need to Know

Petra Zimmermann, Nigel Curtis

Summary: Monkeypox is a zoonotic disease that has recently seen a global increase in cases, predominantly among men who have sex with men. While previous outbreaks have disproportionately affected children, the risk to children from the clade II strain is unclear. Nonetheless, children and pregnant women are considered high-risk groups and should be considered for antiviral treatment. Smallpox vaccination offers protection, but the duration of this protection is unknown, and vaccinated individuals can still be infected. Authorities should be prepared to vaccinate children if the current outbreak spreads.

Interpreting the Results of Trials of BCG Vaccination for Protection Against COVID-19

Christie C. A. Noble, Nicole L. Messina, Laure F. Pittet, Nigel Curtis

Summary: BCG vaccination has potential off-target effects on nonmycobacterial infections. Trials investigating the off-target protection of BCG against COVID-19 have shown conflicting results, likely due to differences in participant characteristics, BCG vaccination variables, outcome measures, and follow-up durations. Controlling these factors will be crucial in designing future trials and intervention strategies to fully exploit the off-target effects of BCG.

Off-target effects of bacillus Calmette-Guerin vaccination on immune responses to SARS-CoV-2: implications for protection against severe COVID-19

Nicole L. Messina, Susie Germano, Rebecca McElroy, Rajeev Rudraraju, Rhian Bonnici, Laure F. Pittet, Melanie R. Neeland, Suellen Nicholson, Kanta Subbarao, Nigel Curtis

Summary: BCG vaccination has immunomodulatory effects on SARS-CoV-2 and reduces the secretion of cytokines associated with severe COVID-19. In addition, BCG vaccination promotes specific T cell immune responses and activates eosinophils.

Training in Ultrasound to Determine Gestational Age (TUDA): Evaluation of a Novel Education Package to Teach Ultrasound-Naive Midwives Basic Obstetric Ultrasound in Malawi

Summary: This study demonstrates that ultrasound-naive practitioners can be trained to confidently and competently perform basic obstetric ultrasound dating scans after a 10-day training program. Local teams can also be oriented to successfully deliver the program virtually. Focused training on specific skills increases the potential for widescale and sustainable implementation.
