Predicting favourable outcomes in the setting of radiofrequency catheter ablation of long-standing persistent atrial fibrillation: A pilot study assessing the value of left atrial appendage peak flow velocity

Background - Catheter ablation is an effective and potentially curative treatment in patients with atrial fibrillation (AF). Aim - To test the hypothesis that left atrial appendage peak flow velocity (LAV) assessed by echocardiography can accurately predict successful catheter ablation as well as favourable outcome in the setting of long-standing persistent AF. Methods - This prospective pilot study enrolled 40 patients with long-standing persistent AF (age 60 +/- 11 years; persistence of AF 4.2 +/- 2 years) who underwent a first catheter ablation procedure using a standardized sequential stepwise protocol. LAY was assessed before the catheter ablation procedure along with classical factors (age, sex, left atrial area, AF cycle length, AF duration and left ventricular ejection fraction), all of which were tested using logistic regression for ability to predict restoration of sinus rhythm during catheter ablation as well as absence of recurrence during a 1-year follow-up. Results - Eighteen patients (45%) experienced AF termination during the procedure and 18 patients (45%) did not develop any recurrence during the first 12 months. Multivariable analysis demonstrated that high LAN/ (>0.3 m/s) was the only independent predictor of AF termination (odds ratio 5.91, 95% confidence interval 1.06-32.88; P=0.04) and absence of recurrence at 1 year (odds ratio 4.33, 95% confidence interval 1.05-17.81; P=0.04). Conclusions - This pilot study demonstrated the feasibility and importance of LAY measurement in the setting of long-standing persistent AF to predict successful catheter ablation and favourable mid-term outcome. Long-term outcomes of left atrial appendage isolation using cryoballoon in persistent atrial fibrillation

Summary: The long-term safety and efficacy of left atrial appendage isolation (LAAi) in addition to pulmonary vein isolation (PVI) using cryoballoon (CB) in persistent atrial fibrillation (AF) were investigated in this study. The results showed that the LAAi + PVI strategy using CB had acceptable long-term outcomes in patients with persistent AF. However, systemic thrombo-embolic events were an important concern throughout the follow-up, especially in cases of non-adherence to anticoagulants.

Sports-related sudden cardiac arrest in young adults

Summary: Data on sports-related sudden cardiac arrest (SrSCA) among young adults are limited. This study aimed to determine the overall incidence, characteristics, and outcomes of SrSCA in young adults. Among the cases identified, most occurred in recreational male sports participants and the use of automated external defibrillators (AEDs) by bystanders was disappointingly low. However, survival rates were significantly higher among those who received bystander cardiopulmonary resuscitation (CPR) and early AED use. Coronary artery disease was found to be the most prevalent cause of SrSCA in young adults.

Summary: Body surface area (BSA) has been reported to be a stronger predictor for prognosis than body mass index in heart failure (HF) patients. However, the sex-specific association of BSA with mortality remains unclear. This study evaluated the association of BSA with two-year all-cause mortality in HF patients and found a U-shaped curve association in females and a linear regression association in males.

Major gaps in the information provided to patients before implantation of cardioverter defibrillators: a prospective patient European evaluation

Summary: This study aims to evaluate the information transmission to patients prior to ICD implantation and its impact on their quality of life. The findings show that although 71.5% of patients felt optimally informed at the time of implantation, many were not provided with information about ICD-related complications, driving restrictions, and end-of-life ICD deactivation. Women were less involved in the decision-making process and reported to be less often optimally informed before ICD implantation compared to men.

Summary: This study conducted a survey to show how clinicians currently approach the knowledge gap in dealing with recurrences after a failed ablation for atrial fibrillation (AF). The majority of respondents perform routine monitoring for AF recurrences after ablation and prefer a long-term monitoring strategy. The combination of absence of symptoms and recorded AF is considered as the definition of success by most respondents.

Summary: This study compared the procedural efficacy and safety, as well as the 12-month AF recurrence, between the PolarX and AFA-Pro cryoballoon systems. It was found that the PolarX cryoballoon reaches lower temperatures than the AFA-Pro, but it is associated with a higher rate of transient phrenic nerve palsy. No significant differences were observed in terms of 1-year freedom from AF recurrence between the two systems.

Two-year outcomes of leadless vs. transvenous single-chamber ventricular pacemaker in high-risk subgroups

Regional disparity on patient characteristics and perceptions after implantable cardioverter-defibrillator implantation: results from an EHRA patient survey

Summary: The aim of this study was to identify potential regional disparities in characteristics of ICD recipients, patient perceptions and perspectives after implantation and level of information provided to patients. The study found that patients in Western Europe reported higher satisfaction after ICD placement, while patients in Central/Eastern Europe felt more optimally informed at the time of device implantation. Therefore, novel strategies are needed to address regional differences in patients' quality of life and provision of information.

Summary: The aim of this study is to investigate the impact of direct oral anticoagulant (DOAC) management on the incidence of pocket haematoma in patients undergoing pacemaker or implantable cardioverter-defibrillator implantation. The results of a large multicenter prospective observational study showed that the incidence of clinically relevant haematoma was low in patients with DOAC interruption, indicating that DOAC management can effectively reduce the risk of pocket haematoma.
