In this work, time-of-flight secondary ion mass spectrometry (TOF-SIMS) was used for detecting systematic variations in the spatial and compositional distributions of lipids in human tissue samples. Freeze-dried sections of subcutaneous adipose tissue from six chronic kidney disease (CKD) patients and six control subjects were analysed by TOF-SIMS using 25 keV Bi(3)(+) primary ions. Principal component analysis of signal intensities from different fatty acids, diacylglycerol and triacylglycerol ions showed evidence for systematic variations in the lipid distributions between different samples. The main observed difference in the spectra was a concerted variation in the signal intensities from the saturated lipids relative to the unsaturated lipids, while variations in the fatty acid chain lengths were considerably weaker. Furthermore, the three samples showing the lowest degree of saturation came from CKD patients, while three of the four samples with the highest degree of saturation were from control subjects, indicating that low saturation levels in the glycerol lipid distribution may be more frequent in patients with CKD. Systematic differences in the spatial distributions between saturated and unsaturated glycerol lipids were observed in several analysed areas. (c) 2008 Elsevier B.V. All rights reserved.

Low-Plasma Insulin-Like Growth Factor-1 Associates With Increased Mortality in Chronic Kidney Disease Patients With Reduced Muscle Strength

Zhimin Chen, Erik Nilsson, Bengt Lindholm, Olof Heimburger, Peter Barany, Peter Stenvinkel, Abdul Rashid Qureshi, Jianghua Chen

Summary: Chronic kidney disease (CKD) is associated with metabolic and nutritional abnormalities, including resistance to insulin-like growth factor-1 (IGF-1) and reduced muscle mass and strength. Low levels of IGF-1 and hand-grip muscle strength (HGS) are independent predictors of increased mortality in CKD patients. The association of IGF-1 with survival in CKD patients depends on nutritional status.

Accelerated Vascular Aging in Chronic Kidney Disease: The Potential for Novel Therapies

S. Hobson, S. Arefin, A. Witasp, L. Hernandez, K. Kublickiene, P. G. Shiels, P. Stenvinkel

Summary: The pathophysiology of vascular disease is associated with accelerated biological aging and a combination of various risk factors. Chronic kidney disease (CKD) serves as a valuable model to study the structural, functional, and molecular aspects of arterial aging. The cardiorenal syndrome results from the detrimental interaction between the kidney and the cardiovascular system. Understanding the molecular mechanisms of uremic early vascular aging provides insights into inflammatory pathways and emerging risk factors, offering new prospects for pharmacological and nutritional interventions.

Indoxyl Sulphate Retention Is Associated with Microvascular Endothelial Dysfunction after Kidney Transplantation

Sam Hobson, Samsul Arefin, Awahan Rahman, Leah Hernandez, Thomas Ebert, Henriette de Loor, Pieter Evenepoel, Peter Stenvinkel, Karolina Kublickiene

Summary: Kidney transplantation is the preferred treatment for chronic kidney disease patients, offering improved quality of life and reduced mortality compared to dialysis. However, cardiovascular disease remains a leading cause of death in this population. A study investigated the functional properties of blood vessels two years after kidney transplantation and found that vessel stiffness improved while endothelial function worsened. Additionally, higher levels of a particular compound, indoxyl sulphate, were associated with decreased endothelial function and increased P-selectin levels after transplantation. Further experiments revealed that indoxyl sulphate impaired endothelium-dependent relaxation, potentially contributing to cardiovascular risk.

Summary: Kidney transplantation (KT) can improve the neurological status of chronic kidney disease (CKD) patients by affecting circulating BBB-specific biomarkers. This study compared the levels of specific biomarkers in kidney-failure patients before and after KT, showing decreased levels of NSE and increased levels of BDNF and NfL at the two-year follow-up. Male patients had higher BDNF levels compared to females. KT can improve the neurological status of CKD patients, as indicated by changes in BBB-specific biomarkers.

Summary: Elevated phosphate in urine may indicate high intake of inorganic P salts from food additives. Elevated phosphate in plasma is linked to vascular dysfunction and calcification. This study explored the associations between phosphate in urine and plasma, estimated phosphate intake, and incidence of cardiovascular disease (CVD). It found that higher levels of urinary phosphate were associated with CVD and myocardial infarctions. Plasma phosphate showed a weaker association with CVD.

Summary: This study investigated the bacterial DNA signatures in the circulation of CKD patients and those receiving KTx treatment. The results suggest that dysbiosis increases as CKD progresses, accompanied by an increase in trimethylamine (TMA) producing pathobionts Pseudomonas and Bacillus. Surprisingly, KTx patients displayed a significantly different microbiota compared to CKD5 patients, with further increase in TMA producing Bacillus and loss of salutogenic Lactobacilli. Two genera (Veillonella and Saccharimonidales) showed significant differences in abundance following KTx, reflecting a reciprocal relationship between TMA producers and utilisers.

Inflammation and gut dysbiosis as drivers of CKD-MBD

Pieter Evenepoel, Peter Stenvinkel, Catherine Shanahan, Roberto Pacifici

Summary: Two decades ago, the term chronic kidney disease-mineral and bone disorder (CKD-MBD) was coined to describe the abnormalities in biochemical, bone, and extra-skeletal calcification in patients with CKD. CKD-MBD is a prevalent complication that contributes to fractures, cardiovascular disease, loss of quality of life, and premature mortality. Current therapies targeting phosphate retention, vitamin D metabolism, and parathyroid hormone disturbances have been largely unsuccessful, calling for new approaches. The role of the gut microbiome, immune system, inflammation, and oxidative stress in CKD-MBD is explored in this review, along with potential therapeutic options.
