Evaluation and Treatment of Pericarditis A Systematic Review

IMPORTANCE Pericarditis is the most common form of pericardial disease and a relatively common cause of chest pain. OBJECTIVE To summarize published evidence on the causes, diagnosis, therapy, prevention, and prognosis of pericarditis. EVIDENCE REVIEW A literature search of BioMedCentral, Google Scholar, MEDLINE, Scopus, and the Cochrane Database of Systematic Reviews was performed for human studies without language restriction from January 1, 1990, to August 31, 2015. After literature review and selection of meta-analyses, randomized clinical trials, and large observational studies, 30 studies (5 meta-analyses, 10 randomized clinical trials, and 16 cohort studies) with 7569 adult patients were selected for inclusion. FINDINGS The etiology of pericarditis may be infectious (eg, viral and bacterial) or noninfectious (eg, systemic inflammatory diseases, cancer, and post-cardiac injury syndromes). Tuberculosis is a major cause of pericarditis in developing countries but accounts for less than 5% of cases in developed countries, where idiopathic, presumed viral causes are responsible for 80% to 90% of cases. The diagnosis is based on clinical criteria including chest pain, a pericardial rub, electrocardiographic changes, and pericardial effusion. Certain features at presentation (temperature >38 degrees C [>100.4 degrees F], subacute course, large effusion or tamponade, and failure of nonsteroidal anti-inflammatory drug [NSAID] treatment) indicate a poorer prognosis and identify patients requiring hospital admission. The most common treatment for idiopathic and viral pericarditis in North America and Europe is NSAID therapy. Adjunctive colchicine can ameliorate the initial episode and is associated with approximately 50% lower recurrence rates. Corticosteroids are a second-line therapy for those who do not respond, are intolerant, or have contraindications to NSAIDs and colchicine. Recurrences may occur in 30% of patients without preventive therapy. CONCLUSIONS AND RELEVANCE Pericarditis is the most common form of pericardial disease worldwide and may recur in as many as one-third of patients who present with idiopathic or viral pericarditis. Appropriate triage and treatment with NSAIDs may reduce readmission rates for pericarditis. Treatment with colchicine can reduce recurrence rates.

Anti-interleukin-1 agents for pericarditis: a primer for cardiologists

Summary: Anti-interleukin (IL)-1 agents are important drugs for the treatment of autoinflammatory and rheumatic conditions. IL-1 alpha and IL-1 beta are strong proinflammatory cytokines, while IL-1Ra can mitigate their proinflammatory effects. Overproduction of IL-1 alpha and IL-1 beta is a therapeutic target in patients with recurrent idiopathic pericarditis. Currently available anti-IL-1 agents include anakinra, rilonacept, and canakinumab, which have demonstrated efficacy and safety in observational studies and clinical trials.

Natriuretic peptides to differentiate constrictive pericarditis and restrictive cardiomyopathy: A systematic review and meta-analysis

Summary: The systematic review and meta-analysis showed that BNP and NT-proBNP levels were significantly lower in patients with constrictive pericarditis (CP) compared to restrictive cardiomyopathy (RCM). The pooled area under the curve (AUC) of the BNP level demonstrated good diagnostic accuracy in differentiating both conditions.

Post-pericardiotomy syndrome: insights into neglected postoperative issues

Research on anti-inflammatory drugs for treatment shows conflicting evidence, with colchicine showing promising results for prevention.

Summary: This study aimed to develop a machine-learning scoring system based on easily accessible clinical variables from the ESC-EHRA Atrial Fibrillation Ablation Long-Term Registry to predict the probability of 1-year arrhythmic recurrence after catheter ablation for AF patients. Among four different supervised machine-learning models, the random forest model achieved the best performance with a 19-variable version, outperforming existing scores. The final calibrated model, available as a web calculator, showed significantly better predictive performance.

Right ventricular epicardial arrhythmogenic substrate in long-QT syndrome patients at risk of sudden death

The Brugada syndrome: risk stratification

Summary: Thirty years after its description, the understanding of Brugada syndrome has significantly increased. The spontaneous type 1 ECG pattern is a well-established prognostic marker in asymptomatic patients, with double the risk of arrhythmic events compared to drug-induced ECG pattern. Careful monitoring, including periodic ECGs and 12-lead 24-hour Holter monitoring, is necessary due to the variability of the ECG pattern over time. Additional electrocardiographic markers, such as AV block and increased QRS duration, have been associated with increased arrhythmic risk.

Atrial fibrillation fundamentals: from physiopathology to transcatheter ablation

Summary: Atrial fibrillation is a common and harmful arrhythmia. Coumel's Triangle can help understand its complex pathogenesis, with substrate, trigger, and catalyst factor playing a role. Anti-arrhythmic drugs have limited efficacy, while interventional therapy is more effective, especially in early stages of the disease, and can reduce mortality in certain populations. Ablation procedures may vary depending on the type of AF and presence of atrial dilation.

Summary: The study evaluated the long-term prognosis of a large cohort of asymptomatic patients with Brugada ECG pattern. The entire population of asymptomatic patients with BrECG exhibited a relatively low event rate per year, with those having spontaneous type-1 BrECG and positive EPS showing higher risk. Asymptomatic patients with drug-induced-only BrECG had minimal arrhythmic risk, but ongoing follow-up was recommended to detect the appearance of spontaneous type-1 BrECG pattern.
