Implementation of supported conversation for communication between nursing staff and in-hospital patients with aphasia

Background: Patients admitted with aphasia due to stroke may find it difficult to access information and participate in decision-making concerning their own treatment, care, and rehabilitation. An increased understanding of the importance of communicative access has prompted speech-language therapists to direct intervention at contextual factors, including communication partner training.Aims: An implementation project is described in which supported conversation for adults with aphasia (SCA) was adapted for use at a large hospital stroke unit. The project aims were (1) to develop a procedural guideline for interdisciplinary staff to communicate with in-patients with aphasia, (2) to develop an interdisciplinary training course and educate all staff members, and (3) to make available a set of shared communication tools. The present study reports the outcome of the training programme for nursing staff.Methods & Procedures: A stepwise adaptation and implementation procedure is described which led to the development of the guideline, tools, and training programme. A mixed-methods design was used to measure changes pre- and post-training for nursing staff, including assessment of quantitative and qualitative outcomes. All nurses and nursing assistants received a questionnaire before and after their participation in an SCA workshop, and seven members from the nursing staff also participated in individual semi-structured interviews about their experiences with the SCA method.Outcomes & Results: Questionnaires from 31 nursing staff members showed that they rated their understanding of aphasia higher after the workshop and they perceived communication to be less frustrating for the patient. Changes were also noted in the types of strategies they used. In the interviews, the nurses described feeling more confident about their ability to communicate with patients, more certain about establishing understanding with patients, and more willing to initiate conversations about complex topics. Difficulties with using tools and techniques were attributed to shortage of time, picture tools being too complex, and patient symptoms.Conclusions: Implementation was considered successful based on the nursing staff's evaluations. Contributing factors may have been staff's involvement in adaptation, leadership support, and a working culture on the stroke unit characterised by readiness to adapt to guidelines. To ensure that the majority of staff members will actually apply tools and techniques, continued monitoring of the implementation process will be necessary as well as education of new staff and re-evaluation of procedures.

Summary: The COVID-19 pandemic had a significant impact on stroke admissions, IV thrombolysis (IVT), and mechanical thrombectomy volumes, with a decline observed during the first year. However, mechanical thrombectomy volumes remained stable in high-volume centers. These findings suggest a preservation of stroke care for higher severity of disease during the first year of the pandemic.

Autonomic dysfunction after mild acute ischemic stroke and six months after: a prospective observational cohort study

Summary: This is a study on autonomic dysfunction in ischemic stroke patients, which found that autonomic dysfunction is prevalent in stroke patients and associated with a worse clinical outcome. The study also found that the head-up tilt table test is tolerable in an acute stroke setting and can be used to optimize patient care.

Capillary transit time heterogeneity inhibits cerebral oxygen metabolism in patients with reduced cerebrovascular reserve capacity from steno-occlusive disease

Summary: Healthy cerebral perfusion is characterized by a homogeneous distribution of capillary transit times, but disruption of this homogeneity may hinder oxygen extraction. Capillary transit time heterogeneity (CTH) refers to some capillaries having excessively low blood flows, while others have excessively high blood flows, resulting in short transit times. CTH is often seen in cerebral vessel disease. This study aimed to investigate the relationship between CTH and cerebral steno-occlusive vessel disease. Results showed that patients with impaired cerebrovascular reserve capacity had elevated CTH and a significant reduction in cerebral metabolic rate of oxygen (CMRO2) after administration of acetazolamide (ACZ), indicating the potential negative impact of CTH on oxygen extraction. This highlights the importance of addressing CTH in patients with cerebral vessel disease.

High-intensity training in patients with lacunar stroke: A one-year follow-up

Summary: The study aimed to investigate the challenges of initiating and adhering to regular exercise post-stroke, as well as the positive effects of early initiation of home-based high-intensity interval training (HIIT) on physical activity. The results showed that, although HIIT did not increase long-term cardiorespiratory fitness, it increased the time spent on vigorous-intensity activities.

Processes of Care and Associated Factors in Patients With Stroke by Immigration Status

Summary: This study investigated the differences in acute stroke care between immigrants and Danish-born residents, and found that immigrants were less likely to receive early stroke care and be admitted to stroke unit, as well as received fewer individualized stroke care processes. These disparities were primarily influenced by the socioeconomic status and cohabitation of immigrants.

Cortical hemodynamic response during cognitive Stroop test in acute stroke patients assessed by fNIRS

Summary: This study explored the feasibility of fNIRS as a biomarker to assess executive function in AIS patients. The results showed that AIS patients performed worse than the healthy controls in the SCWT. However, both groups exhibited an inverse hemodynamic response in the prefrontal cortex during the test.

Pleocytosis in cerebrospinal fluid attributed to ischemic stroke: A review of the literature

Summary: This review aimed to assess the prevalence of pleocytosis in the cerebrospinal fluid following ischemic stroke without inflammatory or infectious etiology. The results showed that the prevalence of pleocytosis ranged from 0% to 28.6% with a mean of 11.8%. The study indicated that pleocytosis following ischemic stroke is uncommon and should prompt further investigations.

Summary: This study aims to test the feasibility and efficacy of personalized, multi-electrode anodal TDCS of the ipsilesional primary motor hand area (M1-HAND) in patients with subacute stroke and upper-extremity paresis, and assess the effects of TDCS on motor network connectivity and interhemispheric inhibition using functional MRI and transcranial magnetic stimulation. The results of this study may inform future personalized TDCS studies in patients with focal neurological deficits after stroke.

A Systematic Review of the Circadian Rhythm of Bone Markers in Blood

Summary: This review provides an overview of the circadian variation of bone markers and the factors that influence this rhythmicity. Many bone markers show a marked circadian variation with nighttime or early morning peaks. Factors such as fasting can affect the circadian variation of these markers. Understanding and considering these circadian variations and influencing factors are important for accurate measurement of bone markers.
