Multiplex PCR for detection of acquired plasmid-borne fosfomycin resistance fos genes in Escherichia coli

Summary: A rapid and reliable multiplex PCR was developed to simultaneously detect known plasmid-mediated fos genes conferring acquired resistance to fosfomycin. Our technique was tested on a collection of Escherichia coli isolates previously identified as bearing fosA, fosC, and fosL-like genes, demonstrating a sensitivity and specificity of 100%.

Progressive in vivo development of resistance to cefiderocol in Pseudomonas aeruginosa

Summary: This study reports the development of cefiderocol resistance among sequential Pseudomonas aeruginosa clinical isolates, without exposure to the drug. The resistance was driven by multifactorial mechanisms, including mutational changes in iron transporter proteins associated with drug uptake and overexpression of certain genes.

Emergence of Carbapenemase-Producing Hypervirulent Klebsiella pneumoniae in Switzerland

Summary: There is an increasing occurrence of multidrug-resistant (MDR) and hypervirulent (hv) Klebsiella pneumoniae (MDR-hvKp) convergent clones. These strains can cause difficult-to-treat infections and have a higher mortality rate in healthy adults. This study in Switzerland identified a rate of 9.0% K. pneumoniae with a virulence genotype, producing carbapenemase and recovered from rectal swabs, urine, and blood. The presence of alarming MDR-hvKp and MDR nonhypervirulent K. pneumoniae (MDR-nhv-Kp) isolates highlights the need for surveillance networks to track and monitor their spread.

Resistance to ceftazidime-avibactam in a KPC-2-producing Klebsiella pneumoniae caused by the extended-spectrum beta-lactamase VEB-25

Summary: Carbapenem-resistant Enterobacterales, including KPC-producing Klebsiella pneumoniae, pose a major threat to public health due to their rapid spread. Ceftazidime-avibactam, a beta-lactam/beta-lactamase inhibitor combination, has shown excellent activity against multidrug-resistant KPC-producing Enterobacterales strains. However, there is an increasing occurrence of CAZ-AVI-resistant K. pneumoniae isolates, mainly due to the emergence of KPC variants that confer resistance to CAZ-AVI but retain carbapenem resistance.

A Selective Culture Medium for Screening Cefiderocol Resistance in Enterobacterales, Pseudomonas aeruginosa, and Acinetobacter baumannii

Summary: Cefiderocol (FDC) is an effective siderophore cephalosporin against multidrug-resistant Gram-negative bacteria. The SuperFDC medium was developed to rapidly and accurately identify FDC-resistant pathogens, which is crucial for controlling their spread. The medium showed high sensitivity and specificity in detecting FDC-resistant isolates with various resistance mechanisms.

In vitro-obtained meropenem-vaborbactam resistance mechanisms among clinical Klebsiella pneumoniae carbapenemase-producing K. pneumoniae isolates

Summary: A study was conducted to select MEM-VAB resistant mutants and decipher the resistance mechanisms. The mutants showed reduced susceptibility to various antibiotics and had mutations in the ompK36 gene. Complementation with the wildtype ompK36 gene reduced the minimal inhibitory concentrations. Additionally, the bla KPC gene copy numbers were significantly increased in the resistant mutants.

Rapid detection of cefiderocol susceptibility/resistance in Acinetobacter baumannii

Impact of veterinary antibiotics on plasmid-encoded antibiotic resistance transfer

Summary: The study aimed to evaluate whether subdosages of antibiotics administered in veterinary medicine could enhance plasmid transfer and resistance gene exchange in gut microbiota. The results showed that antibiotics in veterinary medicine may induce the transfer of plasmid-encoded resistance and contribute to the global spread of antibiotic resistance genes.

Summary: Rational combination therapy can increase efficacy and prevent the emergence of resistance. This study found that the combination of ceftazidime/avibactam and fosfomycin showed strong synergy in suppressing bacterial count and preventing resistance. Clinical evaluations are needed to explore dose reductions and the use of this combination for highly resistant strains.

Plasmid-mediated fosfomycin resistance in Escherichia coli isolates of worldwide origin
