Serum concentrations of 25-OH vitamin D in patients with systemic lupus erythematosus (SLE) are inversely related to disease activity: is it time to routinely supplement patients with SLE with vitamin D?

Background Low serum vitamin D concentrations have been reported in several autoimmune disorders. Objective To assess whether low serum vitamin D concentrations are related to disease activity of patients with systemic lupus erythematosus (SLE). Methods 378 patients from several European and Israeli cohorts were pooled and their disease activity was measured by two different methods: 278 patients had SLE disease activity-2000 (SLEDAI-2K) scores and 100 patients had European Consensus Lupus Activity Measurement (ECLAM) scores. In order to combine the two systems the scores were converted into standardised values (z-scores), enabling univariate summary statistics for the two variables (SLEDAI-2K and ECLAM). The commercial kit, LIAISON 25-OH vitamin D assay (310900-Diasorin) was used to measure serum concentration of 25-OH vitamin D in 378 patients with SLE. Results A significant negative correlation was demonstrated between the serum concentration of vitamin D and the standardised values (z-scores) of disease activity scores as measured by the SLEDAI-2K and ECLAM scales (Pearson's correlation coefficient r = -0.12, p = 0.018). Conclusions In a cohort of patients with SLE originating from Israel and Europe vitamin D serum concentrations were found to be inversely related to disease activity.

Evaluation of the Efficacy of BBIBP-CorV Inactivated Vaccine Combined with BNT62b2 mRNA Booster Vaccine

Summary: In this prospective study, the efficacy of heterologous vaccination with BNT162b2 on individuals previously administered with two doses of BBIBP-CorV vaccine was analyzed. The results showed that the total Ig levels significantly increased after BNT162b2 boosters, suggesting that BNT162b2 can successfully enhance the effects of two-dose BBIBP-CorV vaccination.

The interferon in idiopathic inflammatory myopathies: Different signatures and new therapeutic perspectives. A literature review

Summary: Idiopathic inflammatory myopathies (IIM) have diverse molecular pathogenetic mechanisms, resulting in the partial inefficacy of current immunomodulatory drugs. Genomic and proteomic studies have revealed the role of interferon (IFN) in IIM, showing a predominant type I IFN response in dermatomyositis (DM), a type 2 IFN profile in anti-synthetase syndrome (ASyS) and inclusion body myositis (IBM), and a less robust IFN footprint in immune-mediated necrotizing myopathy (IMNM). The janus kinase/signal transducer and activator of transcription (JAK/STAT) pathway mediates intracellular IFN signaling, which can be targeted by janus kinase inhibitors (JAKi), currently approved for other autoimmune diseases. This review summarizes the evidence of IFN role in IIM pathogenesis and the ongoing clinical trials on IFN-targeting drugs, with a focus on JAKi.

JAK inhibitors for the treatment of VEXAS syndrome

Summary: Vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic (VEXAS) syndrome is a newly described autoinflammatory disease. Its diagnosis is based on somatic mutations of the UBA1 X-linked gene in hematopoietic progenitor cells. The clinical manifestations vary, ranging from autoinflammatory symptoms to underlying hematologic disorders such as myelodysplastic syndromes. The response to treatment for VEXAS is poor, but recent studies have shown that Janus kinase inhibitors (JAK-I) can be effective in managing the disease.

Serum Biomarkers in Connective Tissue Disease-Associated Pulmonary Arterial Hypertension

Summary: CTD-PAH is a life-threatening complication of connective tissue diseases characterized by increased pulmonary arterial pressure and resistance. Due to non-specific symptoms and lack of consensus on screening strategies, CTD-PAH is often diagnosed at an advanced stage. Non-invasive tools, particularly novel serum biomarkers, may improve early diagnosis and disease monitoring of CTD-PAH.

Summary: A retrospective cohort study investigated the effect of acute alcohol consumption on outcomes in patients with acute ischemic stroke treated with intravenous thrombolysis (IVT). Elevated blood alcohol concentration was found to be a significant protective factor against unfavorable short-term and long-term outcomes. Patients with higher blood alcohol concentration showed lower NIHSS scores at 3 and 7 days after IVT.

Summary: This study aimed to assess complement activation and its association with clinical variables in aPL-positive patients with a favorable disease course. The results showed that complement activation persists in the quiescent phase of the disease in patients with catastrophic APS, re-thrombosis, and aPL carriers. This finding suggests a possible mechanism for aPL-induced thrombosis.

The SLE-DAS provides an accurate and feasible flare tool in the clinical setting: a validation study

Treat-to-target in real-life psoriatic arthritis patients: achieving minimal disease activity with bDMARDs/tsDMARDs and potential barriers

Summary: The study aims to describe the frequency of minimal disease activity (MDA) in real-life psoriatic arthritis (PsA) patients, explore predictors of MDA longitudinally, and examine the frequency and predictors of low disease activity (LDA) in patients with axial involvement (axPsA). PsA patients receiving stable biological/targeted-synthetic Disease-Modifying Anti-Rheumatic Drugs (bDMARDs/tDMARDs) were evaluated for disease activity indices, including MDA and ankylosing spondylitis disease activity score-LDA (ASDAS-LDA), at baseline and every 6 months. The results showed that 52-61% of patients achieved MDA and 17-24% achieved ASDAS-LDA. AxPsA, fibromyalgia, osteoarthritis, and BMI ≥35 were less frequently observed in patients with sustained MDA. Age, bDMARDs/tDMARDs duration, axPsA, fibromyalgia, osteoarthritis, and BMI ≥35 were significantly and independently associated with MDA. Age, fibromyalgia, and osteoarthritis were also independently associated with ASDAS-LDA.
