Intra-subject Variability in High Resolution Anorectal Manometry Using the London Classification: Diagnostic and Therapeutic Implications

George Triadafilopoulos, John O. Clarke, Afrin Kamal, Leila Neshatian

Summary: This pilot study examines the durability and reproducibility of HR-ARM findings, showing that only a minority of patients had stable measurements, raising questions about the validity of a single measurement.

Patient Access, Utilization, and Perceptions of Neighborhood and Built Environment Resources

Keeley J. Pratt, Jamie Blalock, Lindsay Breslin, Haley Kiser, Andrew Hanks, Brian C. Focht, Michael Outrich, Sabrena Noria, Bradley Needleman

Summary: This study surveyed postoperative patients to explore their perceptions of neighborhood and built environment resources, and found that the patients had the highest access to lower-cost national food retailers and fitness facilities. Financial challenges were identified as the major barrier in finding food products to meet patients' goals, while patients expressed a need for better trained staff and facilities in fitness centers. Patients reported highest access to walking/running trails, city parks, and sidewalks for outdoor physical activities.

An analysis of readmission trends by urgency and race/ethnicity in the MBSAQIP registry, 2015-2018

Sean M. O'Neill, Bradley Needleman, Vimal Narula, Stacy Brethauer, Sabrena F. Noria

Summary: The study found that readmission rates for ideal bariatric patients improved over time, mainly driven by reductions in non-urgent causes. Racial disparities persist for both urgent and non-urgent causes of readmission.

Impact of post-discharge phone calls on non-urgent hospital returns < 90 days following primary bariatric surgery

Dahlia M. Kenawy, Lindsay M. Breslin, J. C. Chen, Muna M. Tamimi, Joann K. North, Mahmoud Abdel-Rasoul, Sabrena F. Noria

Summary: High-quality post-discharge phone calls may help mitigate early non-urgent hospital returns in patients who have undergone primary bariatric surgery. Care coaching represents a potential intervention to decrease high rates of non-urgent hospital returns in these patients.

The BARI-hoods Project: neighborhood social determinants of health and postoperative weight loss using integrated electronic health record, census, and county data

Keeley J. Pratt, Andrew S. Hanks, Harvey J. Miller, Michael Outrich, Lindsay Breslin, Jamie Blalock, Sabrena Noria, Stacy Brethauer, Bradley Needleman, Brian Focht

Summary: By integrating electronic health records, census data, and county data, this study explores disparities in social determinants of health (SDoH) and weight loss among bariatric surgery patients in Ohio. The findings suggest that proximity to food stores is associated with better weight loss over a two-year period, and lower poverty levels do not negatively affect weight loss in Black patients.

Associations between psychological evaluation outcomes, psychiatric diagnoses, and outcomes through 12 months after bariatric surgery

Haley M. Kiser, Ashleigh A. Pona, Brian C. Focht, Lorraine Wallace, Natasha Slesnick, Sabrena Noria, Bradley Needleman, Keeley J. Pratt

Summary: This study aimed to investigate the association between patients' initial preoperative psychological evaluation outcomes and current psychiatric diagnoses with postoperative outcomes. The results showed that patients who received a clinical decision of RFU had higher weights over 12 months compared to those in the NFU group, but this association was not found for percent total weight loss and other psychiatric variables.

Weight Regain After Bariatric Surgery: Scope of the Problem, Causes, Prevention, and Treatment

Sabrena F. Noria, Rita D. Shelby, Katelyn D. Atkins, Ninh T. Nguyen, Kishore M. Gadde

Summary: Although bariatric surgery is the most effective treatment for severe obesity, a proportion of patients experience weight regain after surgery. The causes of weight regain are not well-defined, but dysregulated/maladaptive eating behaviors, lifestyle factors, and physiological compensatory mechanisms are believed to contribute. Current interventions, including dietary, supportive, behavioral, and exercise approaches, have not shown significant impact on weight regain. Future studies should aim to better understand the etiologies of weight regain and develop non-surgical interventions with demonstrated efficacy in rigorous trials.

Human metabolome variation along the upper intestinal tract

Jacob Folz, Rebecca Neal Culver, Juan Montes Morales, Jessica Grembi, George Triadafilopoulos, David A. Relman, Kerwyn Casey Huang, Dari Shalon, Oliver Fiehn

Summary: This study used a non-invasive, ingestible sampling device to collect and analyze intestinal samples and stool samples from 15 healthy participants. The research found significant differences in food metabolites between stool and intestines, and the intestinal metabolome can reflect the connection between diet, host, and microbial metabolism.

Mixed Esophageal Disease (MED): A New Concept

George Triadafilopoulos, Hiroshi Mashimo, Roger Tatum, John O'Clarke, Mary Hawn

Summary: Mixed esophageal disease (MED) is a disorder that affects the structure and/or function of the esophagus, presenting symptoms and signs that resemble or partially resemble other well-defined esophageal conditions. The MED concept emphasizes the overlap syndrome, incorporating various clinical, endoscopic, imaging, and functional features that have an impact on the patient's quality of life, prognosis, and management. This article focuses on frequently encountered MED scenarios in medico-surgical practices worldwide, highlighting the need for better understanding and management to improve outcomes and prognosis. Proper recognition, definition, and a collaborative, multidisciplinary approach are crucial in optimizing therapeutic outcomes while minimizing complications.

High Prevalence of Anorectal Dysfunction in Ambulatory Patients with Chronic Constipation, Regardless of Colon Transit Time

George Triadafilopoulos, Megan Lee, Leila Neshatian

Summary: This study assessed the relative utility of the London classification in the decision-making of patients with chronic constipation (CC) by comparing wireless motility capsule (WMC) and high-resolution anorectal manometry (HR-ARM) results. It was found that anorectal dysfunction had a high prevalence in patients with slow colonic transit time (CTT) regardless of the classification criteria. Therefore, the specificity and utility of WMC and HR-ARM in assessing patients with CC need further investigation.

The integrated relaxation pressure may not be an appropriate gold standard for deglutitive relaxation due to reliance on a single intragastric reference sensor

Ofer Z. Fass, Kirsten A. Regalia, Andrew J. Sweatt, Monica C. Nandwani, Thomas A. Zikos, Nielsen Q. Fernandez-Becker, Linda A. Nguyen, Irene S. Sonu, George Triadafilopoulos, John O. Clarke

Summary: This study aimed to investigate the impact of selecting different gastric reference sensors on IRP measurement and diagnosis. The results showed that choosing different reference sensors could lead to variations in IRP values, resulting in inconsistent diagnoses in 28% of HRM studies. Selecting the correct gastric reference sensor is crucial for accurate test results and avoiding misdiagnosis.

Bariatric Patient Engagement in a Pre-surgery Virtual Patient Navigation Platform (VPNP)

Anna Chee, Mahmoud Abdel-Rasoul, Kaeli Zoretich, Kayla Diaz, Sabrena F. Noria

Summary: This study analyzed patient engagement with a virtual patient navigation platform (VPNP) designed to guide bariatric surgery candidates through the pre-operative workup. The study found that the majority of patients did not engage with the app, and non-engagement was associated with lower rates of private insurance. The SUS survey showed high perceived usability of the VPNP, but future work will focus on addressing reasons for non-engagement to improve patient participation.
