Summary: This study evaluated the incidence and predictors of non-transplantable recurrence in patients with single hepatocellular carcinoma (HCC) <= 5 cm treated with liver resection (LR). The results showed that HCC size >= 4 cm and high alpha-fetoprotein (AFP) level at the time of LR were independent predictors of recurrence beyond established criteria. Microvascular invasion and microsatellite lesions were identified as additional independent risk factors for non-transplantable recurrence.

Summary: This study compared the clinical outcomes of laparoscopic anatomical resection (LAR) and laparoscopic non-anatomical resection (LNAR) for hepatocellular carcinoma (HCC) located in the posterosuperior (PS) segments. LNAR was found to have shorter operation time, lower blood loss, lower transfusion rate, and shorter hospital stay compared to LAR. Overall survival rates and recurrence-free survival rates were similar between the two groups, but LNAR had a higher intrahepatic recurrence rate. LNAR is considered safe and feasible for HCC located in PS segments when LAR is not feasible.

Summary: This multicenter study compared the short- and long-term outcomes of laparoscopic repeat liver resection (LRLR) versus open repeat liver resection (ORLR) for recurrent hepatocellular carcinoma (HCC) using propensity score matching (PSM). The results showed that LRLR had less intraoperative blood loss, lower postoperative complication rate, and shorter hospital stay compared to ORLR. The survival rates after repeat liver resection were comparable between the two groups. Subgroup analysis also showed a lower postoperative complication rate in LRLR for patients who underwent wedge resection. Therefore, LRLR is a viable option for recurrent HCC.

Summary: In patients with recurrent hepatocellular carcinoma (HCC), both repeat hepatectomy (RH) and radiofrequency ablation (RFA) are viable treatment options. This systematic review and meta-analysis aimed to compare the outcomes of patients undergoing RH versus RFA for recurrent HCC. The results showed that both treatments had comparable overall and disease-free survival rates, but patients undergoing RH were less likely to develop a second recurrence. However, RH was associated with higher overall and major morbidity rates. Despite this, repeat resection remains the preferred option due to its better disease control.

Summary: This study examined the surgical and oncological outcomes of recurrent hepatocellular carcinoma (HCC) after locoregional therapy, particularly in cases of locally recurrent HCC (LR-HCC). Among 273 patients who underwent hepatectomy for HCC, 102 with recurrent HCC were included and retrospectively reviewed. The results showed that LR-HCC patients had worse liver function and higher serum levels of AFP and AFP-L3. Additionally, perioperative morbidities were more frequently observed in patients with recurrent HCC after locoregional therapies. Long-term outcomes for these patients were worse compared to those who underwent hepatectomy. However, salvage hepatectomy for LR-HCC showed a favorable prognosis despite worse surgical outcomes.

The results showed that patients who underwent open surgery and received pretransplant mycophenolate mofetil had a lower incidence of intrahepatic cholangiopathy in ABO-incompatible liver transplantation. This suggests that pretransplant mycophenolate mofetil can reduce the risk of intrahepatic cholangiopathy.

Summary: The study shows that LDLT is comparable to DDLT in terms of graft survival for high-MELD liver transplantation. Biliary complications are more common in LDLT group while viral infection, postoperative uncontrolled ascites, and postoperative hemodialysis are more common in DDLT group. Acute chronic liver failure, intraoperative RBC transfusion, and early postoperative complications are risk factors for graft failure in LDLT group.

Summary: Liver transplantation in patients with hepatocellular carcinoma (HCC) with bile duct tumor thrombus (BDTT) remains controversial. This study analyzed the long-term recurrence and overall survival rates in HCC patients with BDTT after liver transplantation and compared them with the results in HCC patients with portal vein tumor thrombus (PVTT). The study found that HCC patients with BDTT had similar rates of recurrence and survival compared to HCC patients with PVTT after long-term follow-up after liver transplantation.

Summary: This study aimed to provide evidence of antibody responses after COVID-19 vaccination in liver transplant (LT) recipients. Among the 46 patients enrolled in the study, 87% demonstrated an antibody response after the second dose of the vaccine. The study also found that patients with lower levels of immunosuppressive drugs before and after vaccination had a significantly higher antibody response.

Summary: This study evaluated the prognostic value of non-invasive biomarkers after curative liver resection for hepatocellular carcinoma (HCC). The results showed that FIB-4 was a significant predictor of HCC prognosis after curative liver resection, while APRI had less value in prognosis. The hazard ratios for 2-year recurrence-free survival and overall survival were 1.81 and 2.36, respectively, for FIB-4. This study suggests that FIB-4 is an important predictor of HCC prognosis after curative liver resection.
