Summary: Baseline MRI characteristics, including mrT3 substaging, extramural venous invasion (EMVI) grading, mucinous adenocarcinoma, mesorectal fascia involvement, elevated pretreatment carcinoembryonic antigen level, and neoadjuvant chemoradiotherapy (NCRT), were identified as independent predictors of disease-free survival (DFS) in mrT3 LARC patients. Additionally, T3 substaging, EMVI grading, and NCRT were independent predictors of overall survival (OS). Nomograms constructed based on these characteristics allowed for individualized prediction of 3-year and 5-year DFS and 5-year OS with high discrimination and good calibration.

Summary: Spinal cord sarcoidosis is a rare manifestation of sarcoidosis and can lead to neurological sequelae. In this study, we investigated prognostic factors and efficacy of immunosuppressive treatments in patients with spinal cord sarcoidosis. We found that gadolinium enhancement and meningeal involvement on spinal cord MRI, as well as cell count on CSF analysis, were significant prognostic factors associated with relapse or progression rate. TNF-alpha antagonists were more effective than corticosteroids alone, and methotrexate was superior to azathioprine.

Summary: In a prospective observational cohort study, the frailty status of patients at the emergency department admission was evaluated for predicting in-hospital death risk stratification for those needing urgent cholecystectomy. Frailty and open surgery were found to be independent predictors of major complications. The evaluation of functional status in the emergency department could predict the risk of major complications and the need for open surgical approach in elderly patients undergoing urgent cholecystectomy.

Patients with myocardial injury had higher rates of all-cause mortality, major adverse cardiovascular events, arrhythmias, and inflammatory heart disease during follow-up. Myocardial injury, age, and chronic kidney disease were independent predictors of death, while myocardial injury, age, and diabetes were independent predictors of major adverse cardiovascular events.

Summary: Primary mucinous cystadenocarcinoma (MCA) is a rare subtype of breast carcinoma that shares similarities with mucinous cystadenocarcinoma of the ovary and pancreas. Despite the lack of expression of estrogen receptor, progesterone receptor, and HER-2, as well as high Ki67, primary breast MCAs generally have a favorable prognosis. Only 36 cases have been reported in the literature so far, making the histological diagnosis challenging due to its ambiguous morpho-phenotypic profile. It is important to differentiate it from typical mucin-producing breast carcinomas and metastases from the same histotype in other sites.

Summary: Triple-negative breast cancer (TNBC) refers to a subtype of breast cancer that lacks the expression of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2). A population-based study was conducted to compare the outcomes of breast-conserving surgery (BCS) and mastectomy (M) in 289 TNBC patients. The study found no significant differences in locoregional disease-free survival, distant disease-free survival, and overall survival between the two surgical treatments. These findings suggest that TNBC should not be considered a contraindication for breast-conserving surgery.
