Locally Recurrent Rectal Cancer: Prognostic Factors and Long-Term Outcomes of Multimodal Therapy

Pelvic recurrent rectal cancer is still a challenging clinical problem, and patients generally have a dismal prognosis and a poor quality of life. Surgical resection represents the only potentially curative treatment; neoadjuvant treatments are presently being taken into consideration to increase the resectability rate and to improve long-term survival. Among 157 patients observed with recurrent rectal cancer, a series of 58 patients who underwent surgical exploration with curative intent for isolated local recurrence at a single referral institution was retrospectively analyzed. Demographic, pathologic, and therapeutic factors were evaluated to assess long-term prognosis and local control. Forty-four (75.9%) of 58 patients underwent surgical resection. The overall 5-year survival rate for patients who underwent surgical resection was 54.2%, whereas none of the unresected patients lived 5 years (P < 0.001). Patients with R0 resection showed a statistically higher 5-year overall survival and local control rate (72.4 and 70.2%, respectively) compared to R1 patients (37.5 and 31.2%, respectively). At multivariate survival analysis, feasibility of a surgical resection and radicality of excision proved to be independent positive prognostic factors. In contrast, increased presalvage carcinoembryonic antigen serum levels, back pain at diagnosis, and an increasing degree of fixation of recurrent disease to the pelvic wall at preoperative computed tomographic scan were statistically significantly linked to decreased overall survival. Preoperative chemoradiation and radicality of the surgical excision independently influenced the local control among surgically resected patients. Surgical resection still remains the most important therapeutic and prognostic factor for patients with locally recurrent rectal cancer. Multimodal treatments can be safely performed by an experienced team in referral tertiary centers and can result in a safer outcome, better local disease control, and even long-term survival in carefully selected patients.

Summary: In a prospective observational cohort study, the frailty status of patients at the emergency department admission was evaluated for predicting in-hospital death risk stratification for those needing urgent cholecystectomy. Frailty and open surgery were found to be independent predictors of major complications. The evaluation of functional status in the emergency department could predict the risk of major complications and the need for open surgical approach in elderly patients undergoing urgent cholecystectomy.

Summary: This study found that myocardial injury during hospitalization for COVID-19 is associated with mortality and cardiovascular sequelae in survivors. Patients with myocardial injury had higher rates of all-cause mortality, major adverse cardiovascular events, arrhythmias, and inflammatory heart disease during follow-up. Myocardial injury, age, and chronic kidney disease were independent predictors of death, while myocardial injury, age, and diabetes were independent predictors of major adverse cardiovascular events.

However, patients with PPAP may have worse disease-free survival compared to those without PPAP, although the difference is not statistically significant.

Summary: Primary mucinous cystadenocarcinoma (MCA) is a rare subtype of breast carcinoma that shares similarities with mucinous cystadenocarcinoma of the ovary and pancreas. Despite the lack of expression of estrogen receptor, progesterone receptor, and HER-2, as well as high Ki67, primary breast MCAs generally have a favorable prognosis. Only 36 cases have been reported in the literature so far, making the histological diagnosis challenging due to its ambiguous morpho-phenotypic profile. It is important to differentiate it from typical mucin-producing breast carcinomas and metastases from the same histotype in other sites.

Local Treatment of Triple-Negative Breast Cancer: Is Mastectomy Superior to Breast-Conserving Surgery?

Summary: Triple-negative breast cancer (TNBC) refers to a subtype of breast cancer that lacks the expression of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2). A population-based study was conducted to compare the outcomes of breast-conserving surgery (BCS) and mastectomy (M) in 289 TNBC patients. The study found no significant differences in locoregional disease-free survival, distant disease-free survival, and overall survival between the two surgical treatments. These findings suggest that TNBC should not be considered a contraindication for breast-conserving surgery.
