Summary: Based on a systematic review and network meta-analysis, it was found that using a M-sized bougie (33-36 Fr.) in laparoscopic sleeve gastrectomy (LSG) is the optimal choice to achieve a balance between effectiveness and perioperative safety.

Summary: This study retrospectively investigated a total of 15,721 cases of sleeve gastrectomy procedures and found an incidence rate of 0.5% for gastric leaks. Several risk factors were identified, and the healing probability of leaks was lower in patients who required surgical treatment compared to those who did not.

Summary: Severe adhesions from previous abdominal surgeries can complicate subsequent bariatric surgery, but sleeve gastrectomy (SG) is a preferred option due to its lower complication rates and shorter operative time. This paper presents a case of managing a pancreatic leak after SG in a patient with multiple prior abdominal surgeries, highlighting the challenges faced during the procedure. Despite postoperative complications, the patient showed improvement after treatment and was eventually discharged after recovery.

Summary: This study compared the treatment methods for sleeve gastrectomy leaks and found no significant difference in outcomes between intraluminal occlusion and repeated debridement. The initial radiologic appearance was predictive of length of stay and complication severity.

Summary: This study evaluated the updated algorithm for endoscopic treatment of leaks following LSG, including septotomy and balloon dilation. The results showed that the benefits of pigtails and septotomy in treating leaks are significant and should be included in the armamentarium of any bariatric endoscopic service.

Summary: The study compared two methods for staple line reinforcement (SLR) in reducing major complications after sleeve gastrectomy (SG), finding that hybrid suture was associated with a lower incidence of postoperative bleeding compared to running suture. Hybrid suture also showed shorter suture time and less need for additional sutures for hemostasis, indicating its potential benefits in reducing complications.

Summary: The three different approaches to the staple line did not significantly affect the incidence of surgical complications in laparoscopic sleeve gastrectomy. The complete oversewing group had a longer operative time and a significantly higher rate of revisional surgery compared to the partial oversewing group.

Summary: This study retrospectively analyzed all patients treated for gastrointestinal leaks between July 2013 and March 2019, finding that endoscopic therapies performed earlier and in patients with no prior bariatric surgeries have a higher chance of success.

Summary: Symptoms such as abdominal pain, tachycardia, or fever after LSG should raise suspicion of a leak. CT scan of the abdomen and UGIC study have high sensitivity for detecting leaks. Some patients are easily misdiagnosed during the medical visit process, highlighting the importance of improving accuracy in leak diagnosis.

Summary: The study compared the incidence of GERD, esophagitis, and BE postoperatively in patients undergoing VSG and RYGB, finding that VSG patients had a higher frequency of postoperative GERD, while BE was more prevalent among RYGB patients. De novo esophageal reflux symptomatology was more common in VSG patients, but RYGB patients had a higher rate of re-admission.
