Chemotherapy Before Liver Resection of Colorectal Metastases Friend or Foe?

Objective: We conducted a systematic review of the published literature to critically assess benefits and risks of the use of preoperative chemotherapy in patients presenting with colorectal liver metastases. Background: In many centers, chemotherapy is used before hepatic resection of colorectal metastases, even in the presence of a single lesion. Application of chemotherapy requires clear conceptual distinction between patients presenting with resectable lesions (neoadjuvant) versus patients presenting with unresectable lesions, for which chemotherapy is used to reach a resectable situation (downsizing). Methods: The literature (PubMed) was systematically reviewed for publications related to liver surgery and chemotherapy according to the methodology recommended by the Cochrane Collaboration. Results: For unresectable liver metastases, combination regimens result in enhanced tumor response and resectability rates up to 30%, although the additional benefit from targeted agents such as bevacizumab or cetuximab is marginal. For resectable lesions, studies on neoadjuvant chemotherapy failed to convincingly demonstrate a survival benefit. Most reports described increased postoperative complications in a subset of patients due to parenchymal alterations such as chemotherapy-associated steatohepatitis or sinusoidal obstruction syndrome. Conclusion: Preoperative standard chemotherapy can be recommended for downsizing unresectable liver metastases, but not for resectable lesions, for which adjuvant chemotherapy is preferred.

Summary: Pancreatic cancer often presents as locally advanced or borderline resectable. It is currently unclear which chemotherapy is preferred for patients in these categories. This study found that initial treatment with FOLFIRINOX appears to provide a survival benefit for patients that are ultimately unresectable, compared to gemcitabine-based chemotherapy.

Summary: This study aimed to analyze the outcomes of GEP-NETs treatment and compare them with the latest treatment recommendations. The results showed that treatment steps and stages of GEP-NETs were related to survival rates. Surgery was commonly used in both early and advanced stages, while the use of Somatostatine-Agonist as a drug treatment was also common.

Summary: This study investigated the relationship between SSTR2 immunoreactivity and SDHB immunoreactivity, mutational status, and clinical behavior of PPGLs. It also evaluated SSTR-based therapies in metastatic PPGLs. The results showed that SSTR2 expression was associated with SDHB/SDHx mutations and metastatic disease. Somatostatin receptor-based therapies had a high disease control rate in metastatic PPGLs.

Transplantation for metastatic liver disease

Summary: The liver is a common site for metastases, especially from gastrointestinal tumors. Liver transplantation is a promising and sometimes controversial treatment option for neuroendocrine and colorectal liver metastases. Studies have shown excellent long-term outcomes for neuroendocrine liver metastases, but questions remain regarding patient selection, the role of neoadjuvant/adjuvant treatments, and the optimal timing of transplantation. Recent studies have reinvigorated interest in liver transplantation for unresectable colorectal liver metastases, and ongoing trials aim to determine the potential benefits compared to palliative chemotherapy. This review provides a critical summary of current knowledge and highlights areas for further research.

Current Research and Development in Hyperthermic Intraperitoneal Chemotherapy (HIPEC)-A Cross-Sectional Analysis of Clinical Trials Registered on ClinicalTrials.gov

Summary: This study analyzed HIPEC trials registered on ClinicalTrials.gov to identify their focus and assess publication of trial results. Among 234 identified trials, 11% were published and 6% reported results but not as full papers. Most trials were recruiting patients (36%) or completed but unpublished (20%). The majority of trials focused on treatment strategies (68%) and oncological outcomes (53%). The most studied neoplasms were peritoneally metastasized colorectal cancer, gastric cancer, and ovarian cancer.

PD-L1 and HIF-2α Upregulation in Head and Neck Paragangliomas after Embolization

Summary: In pheochromocytomas and paragangliomas, hypoxia upregulates the expression of PD-L1 and HIF-2 alpha, promoting tumor progression and immune microenvironment alterations. Head and neck paragangliomas undergoing embolization show significantly higher expression levels of PD-L1 and HIF-2 alpha, suggesting that targeted treatment using HIF-2 alpha and checkpoint inhibitors may be beneficial.

Management of abdominal pseudotumours in haemophilia: a systematic review

Summary: This study evaluated therapy strategies for symptomatic abdominal haemophilic pseudotumours and found that primary surgical resection can be considered as the first-line treatment, with preoperative embolisation as a bridging therapy before surgery. Diagnostic biopsy and percutaneous drainage should be avoided to prevent complications.
