Acoustic Analysis of Changes in Articulation Proficiency in Patients With Advanced Head and Neck Cancer Treated With Chemoradiotherapy

Objectives: Our aim was to characterize articulation proficiency and differences between tumor sites before and after chemoradiotherapy for advanced head and neck cancer with the help of acoustic measures. Our further goal was to improve objective speech measures and gain insight into muscle functioning before and after treatment. Methods: In 34 patients with laryngeal or hypopharyngeal, nasal or nasopharyngeal, or oral or oropharyngeal cancer, we acoustically analyzed nasality, vowel space, precision, and strength of articulation in 12 speech sounds (/alpha/, /i/, /u/, /p/, /s/, /z/, /1/, /t/, /tj/, /k/, /x/, /r/) before treatment and 10 weeks and 1 year after treatment. Outcomes were compared between assessment points and between tumor sites. Results: Nasality in nonlaryngeal sites was significantly reduced by treatment. Most affected in articulation were the oral or oropharyngeal cancer sites, followed by the nasal or nasopharyngeal sites. One year after treatment, vowel space had not recovered and consonant articulation had weakened. Laryngeal sites were less affected in articulation by tumor or treatment. Conclusions: Analyses of articulatory-acoustic features are a useful instrument for assessing articulation and speech quality objectively. Assessment of a number of sounds representing various articulation manners, places, and tongue shapes revealed patterns of speech deterioration after chemoradiotherapy. The results suggest that patients' speech could benefit from articulation exercises to address changes in muscle coordination and/or sensitivity and to counteract side effects and underexercise atrophy.

Dysphagia, trismus and speech impairment following radiation-based treatment for advanced stage oropharyngeal carcinoma: a one-year prospective evaluation

Summary: The study aimed to evaluate swallowing, mouth opening and speech function of patients with stage III-IV oropharyngeal carcinoma (OPC) during the first year after radiation-based treatment (RT(+)) with the introduction of a dedicated preventive rehabilitation program. Results show that functional outcomes deteriorated until 6 months post-treatment, improved by 12 months but did not return to baseline levels, with a significant proportion of patients still experiencing dysphagia, trismus and speech problems. Despite successful implementation, there is room for improvement in the current rehabilitation program, with pretreatment sarcopenia identified as a potential target for rehabilitation strategies.

[18F]FDG-PET accurately identifies pathological response early upon neoadjuvant immune checkpoint blockade in head and neck squamous cell carcinoma

Summary: This multicenter RCT investigated the effect of three different service-delivery modes on patients' adherence to prophylactic swallowing exercises during radiotherapy. The study found that adherence decreased over time in all groups, but the therapist-supported group achieved the highest adherence rates. Increasing face-to-face contact with a speech-language therapist can overcome the problem of low adherence in this population.

Understanding and predicting the performance of passive heat and moisture exchangers using a numerical model

External validation of an MR-based radiomic model predictive of locoregional control in oropharyngeal cancer

Summary: This study aims to externally validate a pre-treatment MR-based radiomics model for predicting locoregional control in OPSCC, and assess the impact of dataset differences on predictive performance. Radiomic features were extracted from the primary tumor volumes of 157 OPSCC patients in a different institute. The results show that the model's performance slightly decreases when applied to the validation cohort, but can be improved by using similar patient subgroups and harmonization techniques.
