Rare Cancers Europe (RCE) methodological recommendations for clinical studies in rare cancers: a European consensus position paper

While they account for one-fifth of new cancer cases, rare cancers are difficult to study. A higher than average degree of uncertainty should be accommodated for clinical as well as for population-based decision making. Rules of rational decision making in conditions of uncertainty should be rigorously followed and would need widely informative clinical trials. In principle, any piece of new evidence would need to be exploited in rare cancers. Methodologies to explicitly weigh and combine all the available evidence should be refined, and the Bayesian logic can be instrumental to this end. Likewise, Bayesian-design trials may help optimize the low number of patients liable to be enrolled in clinical studies on rare cancers, as well as adaptive trials in general, with their inherent potential of flexibility when properly applied. While clinical studies are the mainstay to test hypotheses, the potential of electronic patient records should be exploited to generate new hypotheses, to create external controls for future studies (when internal controls are unpractical), to study effectiveness of new treatments in real conditions. Framework study protocols in specific rare cancers to sequentially test sets of new agents, as from the early post-phase I development stage, should be encouraged. Also the compassionate and the off-label settings should be exploited to generate new evidence, and flexible regulatory innovations such as adaptive licensing could convey new agents early to rare cancer patients, while generating evidence. Though validation of surrogate end points is problematic in rare cancers, the use of an updated notion of tumor response may be of great value in the single patient to optimize the use of therapies, all the more the new ones. Disease-based communities, involving clinicians and patients, should be regularly consulted by regulatory bodies when setting their policies on drug approval and reimbursement in specific rare cancers.

Summary: Tenosynovial giant cell tumour (TGCT) is a rare, locally aggressive tumor that arises from the joints, bursa, and tendon sheaths. There are two types of TGCT: nodular and diffuse, with the former exhibiting a slow disease course and the latter showing locally aggressive behavior. Although not life-threatening, TGCT can cause chronic pain and negatively impact function and quality of life. CSFR1 inhibitors are effective in treating TGCT but are not widely available. The lack of guidelines and uncertainty in selecting appropriate therapy lead to inconsistent adoption of new treatments and suboptimal outcomes for patients. A global consensus meeting was held in June 2022 to develop evidence-based recommendations for the optimal management of TGCT.

Summary: Infectious diseases still pose a threat to the global community, especially in countries with limited resources. Artificial intelligence (AI) has been used to identify microorganisms, with promising results in the detection of malaria, bacteria, nematodes, and other protozoa. Utilizing digital images and various deep-learning strategies, AI has demonstrated reliable potential in assisting pathologists for accurate diagnosis.

Summary: This study aimed to describe the natural history of AR-positive recurrent/metastatic (R/M) salivary gland carcinoma (SGC) patients based on HER2 amplification status. The results showed that HER2 amplification was associated with a higher risk of disease recurrence and death from R/M disease compared to HER2-negative patients, as well as a non-statistically significant higher prevalence of CNS metastases.

The study will assess the impact of the intervention on global QoL deterioration over a 24-month period.

Summary: Treating older adults with cancer is important, but there is a lack of high-quality evidence for their optimal management. Historic under-recruitment of older adults in clinical trials limits our understanding of how existing evidence can be applied to this group. Challenges in conducting clinical trials in older adults impact the clinical relevance of results and limit their representativeness.

Summary: This study identified differential immunohistochemical patterns in the tumors of patients who responded and did not respond to nivolumab, an immunotherapy. Tumors in responders showed poor presence of CD4+ immune cells, high expression of CD56 on tumor cells, and high expression of phosphorylated mTOR protein. Further investigation with genomic analysis is planned to examine the prognostic role of the tumor microenvironment.

Summary: ASCO has taken steps to raise awareness about biosimilar products and published a statement in 2018 to provide guidance on their use in oncology. The FDA has approved a significant number of biosimilar products for use in the United States, including those for cancer treatment. The recent approval of interchangeable biosimilar products for various diseases has further expanded the market. This policy statement by ASCO proposes recommendations on value, interchangeability, clinician barriers, and patient education and access to guide their future activities.

Summary: This study aimed to investigate the changes in self-efficacy to cope with cancer over time in breast cancer patients and whether these changes were similar across patients. It also aimed to explore the relationship between these trajectories and patient psychological well-being and overall quality of life. A total of 404 participants from four countries were assessed a few weeks after breast surgery or biopsy. The majority of patients reported high levels of self-efficacy to cope, which increased over time. However, about 15% of patients experienced a decline in self-efficacy over time, which was associated with worse levels of well-being. The patterns of self-efficacy changes and their relationships with well-being were consistent across the countries.

Summary: This study aimed to examine whether machine learning can improve the prediction accuracy of clinicians in assessing breast cancer patients' quality of life. The results showed that incorporating machine learning into the clinical decision support system significantly enhanced clinicians' ability to predict patients' QoL.
