Trunk and Lower Limb Fat Mass Evaluated by Dual-Energy X-Ray Absorptiometry in a 20-to 80-Year-Old Healthy Italian Population

Background and Aims: Global fat mass distribution seems to correlate with different levels of cardio-metabolic risk; centrally distributed fat carries a high risk of cardiovascular disease, while lower limb adiposity may have a protective effect against insulin resistance. Reference data regarding body composition have already been published for the Italian population; the aim of this study was to add reference values for trunk and lower limb fat mass, and their ratio (TLR), developing percentile distributions for age brackets between 20 and 80 years. Methods: A retrospective analysis of a multicenter, cross-sectional study was conducted and 1,570 healthy Italian adults (1,241 females and 329 males) were selected. The regional fat mass, measured by dual-energy X-ray absorptiometry total body scan, was analyzed and the TLR was calculated. Results: In both genders we observed higher trunk fat mass values in older subjects against a smaller difference in BMI values. The leg fat mass was higher in old men, while it was similar in women at different ages. The TLR values in older subjects doubled those of younger subjects in both genders (62% in males and 71% in females). Conclusions: The identified ranges for trunk, leg fat mass, and TLR may be used as reference values to describe the global fat mass distribution in healthy individuals and to identify states of altered body fat distribution. Summary: This real-world analysis examined the clinical and economic impact of non-dialysis-dependent CKD patients with and without secondary hyperparathyroidism (sHPT) in Italy. The study found that CKD patients with sHPT had a higher incidence of progression to dialysis and cumulative mortality, as well as significantly higher healthcare costs compared to CKD patients without sHPT. The findings highlight the importance of timely intervention to improve the management of CKD patients affected by sHPT.

Ultrasonographic non-radiographic erosions could predict the efficacy of belimumab in articular systemic lupus erythematosus

Summary: The aim of this study was to characterize lupus-related arthritis and assess the efficacy of belimumab in treating articular manifestations of SLE. The results showed that belimumab treatment significantly improved disease activity in patients without erosions, but had no effect on patients with erosions. Additionally, the presence of ultrasound-detected erosions was associated with a decreased efficacy of belimumab. However, further research is needed to confirm these findings due to the small sample size.

Summary: This study assessed the efficacy of belimumab in treating joint and skin manifestations in a nationwide cohort of SLE patients. The results showed that belimumab significantly improved joint and skin symptoms and reduced the use of glucocorticoids. A significant proportion of patients who did not achieve complete remission initially achieved remission during follow-up.

Summary: Psoriatic arthritis (PsA) is effectively treated with biological disease-modifying antirheumatic drugs (bDMARDs). A real-life observational study was conducted to compare the retention rates of IL-17 inhibitors and TNF inhibitors in PsA patients. The study found that secukinumab had a trend towards better retention rate compared to TNF inhibitors, especially in patients with axial involvement.

Factors associated with radiographic progression in rheumatoid arthritis starting biological diseases modifying anti-rheumatic drugs (bDMARDs)

Summary: This study aimed to investigate the factors associated with radiological progression in rheumatoid arthritis (RA) patients treated with biological DMARDs (bDMARDs). The results showed that baseline erosive disease, higher disease activity during treatment, younger age, and monotherapy were independently associated with the development of bone erosions. Additionally, the study team developed a predictive model that accurately forecasted the occurrence of new erosions in patients receiving bDMARDs.
