Summary: Abnormal cardiac repolarization is the underlying cause of life-threatening arrhythmias in various congenital and acquired cardiac diseases. Dysfunction of ion channels involved in repolarization at the cellular level often leads to the repolarization abnormality. Understanding the genetic and electrophysiological basis of repolarization provides insights into cardiac repolarization and may contribute to improved patient-specific management in the future.

Summary: Sudden cardiac death (SCD) is a major cause of death worldwide, often caused by acute ventricular arrhythmias (VAs) triggered by early afterdepolarizations (EADs). This study developed a tissue model to investigate the spatial-temporal dynamics of EADs in T-wave abnormalities. The results suggest possible mechanistic links between the complex spatial dynamics of EADs and T-wave morphologies.

Summary: The study found that torsades de pointes arrhythmia can be induced within a critical range of repolarization time and repolarization time heterogeneities, maintained by reentry wandering along the repolarization gradient. Local restitution curves from the long and short repolarization time regions crossed, associated with T wave inversion at coupling intervals at either side of the crossing point.

Summary: Congenital short QT syndrome (SQTS) is a rare condition characterized by shortened QTc intervals on the electrocardiogram and increased susceptibility to arrhythmias and sudden death. Mutations in K+ channel subunit genes (SQT1-3) have a strong causal relationship with SQTS and impact the electrophysiological properties of cardiac potassium currents. This article reviews the evidence for these mutations and their link to arrhythmia susceptibility, and discusses future research directions.

Summary: Catecholaminergic polymorphic ventricular tachycardia (CPVT) and short QT syndrome (SQTS) are inherited arrhythmogenic disorders. Seven CPVT and four SQTS genes have valid evidence for disease causation and should be included in genetic testing panels. Additional genes associated with conditions that may mimic clinical features of CPVT/SQTS have potential utility for differential diagnosis.
