Summary: The KDIGO 2022 Clinical Practice Guideline for Diabetes Management in Chronic Kidney Disease is an update of the 2020 guideline. The guideline was updated based on reviewing new evidence and using the GRADE approach to assess evidence and develop consensus practice points. The recommendations were updated in the areas of comprehensive care and glucose-lowering therapies, while recommendations in the areas of glycemic monitoring, lifestyle interventions, and management approaches remained unchanged.

It can lead to a perceived lack of treatment effectiveness, even when patients adhere to prescribed inhaled maintenance therapies. Training and ongoing assessment are often lacking in real-world practice. This review examines the evidence for deterioration of inhaler technique over time, identifies contributing factors, and explores innovative approaches to addressing the problem.

Blood eosinophil counts have been adopted as a biomarker to predict responses to ICS in COPD patients. The development of novel inhaled combination treatments has provided valuable evidence to guide the use of different classes of combination treatments.

Effect of Regular, Low-Dose, Extended-release Morphine on Chronic Breathlessness in Chronic Obstructive Pulmonary Disease: The BEAMS Randomized Clinical Trial

Summary: This randomized clinical trial compared the effect of different doses (8 mg/d or 16 mg/d) of extended-release morphine versus placebo on chronic breathlessness in people with chronic obstructive pulmonary disease (COPD). The study found that daily low-dose extended-release morphine did not significantly reduce the intensity of worst breathlessness after 1 week of treatment. These findings do not support the use of these doses of extended-release morphine to relieve breathlessness.

It is important to recognize and address other morbidities that can mimic or aggravate COPD exacerbations to improve outcomes and reduce readmissions.

Recent research has shown that various factors throughout the life course increase the risk of developing COPD. Innovations in omics and imaging techniques have provided greater understanding of the disease's pathophysiology, potentially leading to advancements in its prevention, diagnosis, and treatment. This review focuses on recent advances in the epidemiology, pathophysiology, imaging, diagnosis, and treatment of COPD.

Diagnostic and prognostic implications of 2018 guideline for the diagnosis of idiopathic pulmonary fibrosis in clinical practice

Summary: This study examined the implications of the 2018 updated guideline for IPF diagnosis compared to the 2011 guideline, finding moderate reliability and fair interobserver agreement for probable UIP. The study also revealed that patients with indeterminate UIP CT pattern had a better prognosis, and probable UIP showed lower positive predictive value compared to possible UIP. Additionally, BAL fluid analysis improved diagnostic certainty for IPF diagnosis in patients with probable UIP CT pattern.
