Summary: This article provides practical advice for the clinical management of patients with suspected extraesophageal gastroesophageal reflux disease. The advice is based on available evidence from published studies, clinical trials, and clinical guidelines. It emphasizes the importance of considering extraesophageal manifestations of gastroesophageal reflux disease and the need for a multidisciplinary approach in diagnosis and treatment.

Summary: The Lyon Consensus 2.0 provides conclusive criteria for diagnosing gastro-oesophageal reflux disease (GERD) and offers additional metrics to support or refute the diagnosis when primary criteria are inconclusive. A global team evaluated research since the original Lyon Consensus and collaborated on developing statements to update criteria. The updated consensus includes a modern definition of actionable GERD, describes the likelihood of symptoms being related to reflux episodes, and provides diagnostic strategies and testing options based on proven or unproven GERD.

Summary: Innovations in endoscopy have significantly improved the diagnosis and treatment of GERD. Endoscopy and biopsies can directly observe and determine esophageal abnormalities. Future research should focus on personalized treatment and the primary pathophysiology of GERD.

Summary: This population-based cohort study conducted in Denmark, Finland, Norway, and Sweden from 1979 to 2018 found that negative upper endoscopy in patients with GERD is associated with a significant and long-lasting decrease in both incidence and mortality of upper gastrointestinal cancer. The risk reduction persisted for at least 5 years in incidence and 10 years in mortality after a negative upper endoscopy.

Summary: The Endoscopic Pressure Study Integrated System (EPSIS) is a new diagnostic tool for GERD that shows good accuracy in diagnosing the condition by assessing intragastric pressure waveform patterns. In this study, analysis of EPSIS parameters, specifically pressure difference and waveform gradient, demonstrated high diagnostic accuracy for abnormal acid reflux in patients with GERD symptoms.

Summary: This study aims to assess the reliability of the KidZ Health Castle formula (KHC-F) in determining the correct probe position of a multichannel intraluminal impedance pH. The results show that the positioning with KHC-F is correct in two-thirds of the participants and has a strong correlation with the target position. Compared with other formulas, KHC-F and KHC-F v2 outperform in terms of positioning accuracy, mean error, and 95% limits of agreement.

Summary: This study aimed to evaluate the feasibility, effectiveness, and safety of anti-reflux mucosal ablation (ARMA) in the treatment of gastroesophageal reflux disease (GERD). The results showed that ARMA is a feasible and safe procedure that effectively reduces GERD symptoms in the short term, with clinical success achieved in approximately two-thirds of patients. Both reflux hypersensitivity and confirmed GERD patients, regardless of their response to acid suppression medication, may be suitable candidates for ARMA treatment.

Summary: The systematic review and meta-analysis demonstrated a consistent association between abnormal GEFV indicated by Hill's classification III/IV and symptomatic GERD and erosive esophagitis. It is recommended to include Hill's classification in routine endoscopy reports and workup for GERD.

Summary: In this study, over half of patients with COPD were diagnosed with RE, but RE was not associated with age, sex, smoking history, or severity of airflow limitation. AECOPD was also not related to the presence or severity of RE. The use of theophylline may increase the risk of RE in COPD patients.

Summary: This study aimed to assess whether reduced masticatory function can be used as a predictor for gastroesophageal reflux disease (GERD) and esophageal dysphagia. The results showed that reduced masticatory function was associated with GERD and esophageal dysphagia.

Summary: This study aimed to assess the overlap rate of gastroesophageal reflux disease (GERD) and functional dyspepsia (FD) and the yield of esophagogastroduodenoscopy in patients clinically presenting with FD. The results showed that GERD-FD overlap was more common than each disorder alone, with postprandial distress syndrome being significantly prominent. Organic dyspepsia was uncommon in patients clinically fulfilling the Rome IV criteria for FD.
