Pathogen Detection Using Metagenomic Next-Generation Sequencing of Plasma Samples from Patients with Sepsis in Uganda

Summary: Metagenomic sequencing is used to detect pathogens in archived plasma samples from predominantly HIV-infected patients with sepsis in Uganda. The sequencing identified a broad range of potential pathogens, particularly viruses. Unexpectedly high rates of hepatitis B and Epstein-Barr virus (EBV) viremia were found. Further study is needed to determine the clinical importance of these viral infections in HIV patients with sepsis.

Summary: The Sudan virus disease outbreak in Uganda in 2022 highlighted the lack of effective clinical management guidelines for viral haemorrhagic fevers (VHFs). A systematic review identified 32 guidelines, with the majority being of low quality and lacking supporting evidence. The inconsistent and incomplete guidance on supportive care and therapeutics poses risks to patients and challenges for clinicians. It is recommended to establish a standard living guideline framework and invest in trials to improve the quality and applicability of guidelines, as well as identify optimal treatment strategies and affordable interventions for VHFs.

This systematic review aims to explore the rates and risk factors for pediatric post-discharge mortality in resource-poor settings.

Novel engineered chimeric engulfment receptors trigger T cell effector functions against SIV-infected CD4+T cells

Summary: Adoptive therapy using genetically engineered T cells shows promise in treating infectious diseases in immunocompromised individuals. In this study, chimeric engulfment receptor (CER) T cells designed to recognize phosphatidylserine (PS)-expressing cells successfully eliminated SIV-infected cells. The introduction of innate immune functions into T cells enhances the elimination of SIV-infected cells, and further in vivo studies are needed to evaluate their effectiveness.

Summary: This study used counterfactual placebo comparisons using other data sources to evaluate the efficacy of antiretroviral agents for HIV pre-exposure prophylaxis. The findings showed that injectable cabotegravir (CAB-LA) and daily oral emtricitabine/tenofovir disoproxil fumarate (FTC/TDF) can significantly reduce the risk of HIV infection compared to no intervention, and the efficacy of FTC/TDF against counterfactual placebo was consistent with previous placebo-controlled trials.

Summary: AZD1222 vaccine demonstrated safety, efficacy, and immunogenicity in the ongoing phase 3 trial. The vaccine provided durable protection with a significant reduction in the incidence of symptomatic COVID-19 and showed high efficacy against severe/critical disease. Humoral immune responses induced by AZD1222 waned over time.

Recognising Sepsis as a Health Priority in Sub-Saharan African Country: Learning Lessons from Engagement with Gabon's Health Policy Stakeholders

Summary: This study investigated the prioritization of sepsis in Gabon and found that it has not been prioritized due to a lack of evidence regarding its burden. The researchers suggest linking sepsis with the country's existing health sector priorities to accelerate its prioritization in policy. Involving the media in raising awareness and prioritization of sepsis could also contribute to stakeholder engagement.
