Pulmonary drug delivery systems for tuberculosis treatment

Tuberculosis (TB) remains a major global health problem as it is the second leading cause of death from an infectious disease worldwide, after the human immunodeficiency virus (HIV). Conventional treatments fail either because of poor patient compliance to the drug regimen or due to the emergence of multidrug-resistant tuberculosis. The aim of this review is to give an update on the information available on tuberculosis, its pathogenesis and current antitubercular chemotherapies. Direct lung delivery of anti-TB drugs using pulmonary delivery systems is then reviewed since it appears as an interesting strategy to improve first and second line drugs. A particular focus is place on research performed on inhalable dry powder formulations of antitubercular drugs to target alveolar macrophages where the bacteria develop. Numerous studies show that anti-TB drugs can be incorporated into liposomes, microparticles or nanoparticles which can be delivered as dry powders to the deep lungs for instantaneous, targeted and/or controlled release. Treatments of infected animals show a significant reduction of the number of viable bacteria as well as a decrease in tissue damage. These new formulations appear as interesting alternatives to deliver directly drugs to the lungs and favor efficient TB treatment. (C) 2014 Elsevier B.V. All rights reserved.

Resolution of MoS2 Nanosheets-Induced Pulmonary Inflammation Driven by Nanoscale Intracellular Transformation and Extracellular-Vesicle Shuttles

Amphiphilic Phosphorus Dendrons Associated with Anti-inflammatory siRNA Reduce Symptoms in Murine Collagen-Induced Arthritis

Summary: Small interfering RNA (siRNA) shows potential in treating rheumatoid arthritis by inhibiting major cytokines like tumor necrosis factor-alpha (TNF-alpha). Researchers have developed original cationic amphiphilic phosphorus dendrons that can form dendriplexes with TNF-alpha siRNA. These dendriplexes protect siRNA from degradation and enhance macrophage uptake, leading to significant inhibition of TNF-alpha expression and anti-inflammatory effects in in vitro and in vivo models. This opens up important perspectives for using these original amphiphilic dendrons in chronic inflammatory diseases.

Pickering emulsions stabilized with biodegradable nanoparticles for the co-encapsulation of two active pharmaceutical ingredients

Hyaluronic acid-conjugated liposomes loaded with dexamethasone: A promising approach for the treatment of inflammatory diseases

Summary: In this study, lipid nanoparticulate platforms loaded with dexamethasone were developed and evaluated. The dexamethasone-loaded liposomes, with sizes below 150 nm, were coated with polyethylene glycol, hyaluronic acid, or a mixture of both to cross the pulmonary mucus layer and target alveolar proinflammatory macrophages expressing CD44. In vitro experiments showed that encapsulated dexamethasone in liposomes, especially in HA-bearing ones, improved anti-inflammatory efficacy compared to the free drug. In vivo data demonstrated that dexamethasone-loaded liposomes exhibited improved consistency in efficacy compared to free dexamethasone.

Emerging nanoparticle platforms to improve the administration of glucocorticoids

Summary: Glucocorticoids (GC) are commonly used drugs for inflammatory diseases, but their prolonged use can lead to severe side effects. Recent advancements in GC prodrugs and conjugates, administered as free forms or nanoparticles, have improved drug delivery, controlled release, and targeting in inflammatory sites. This review highlights the potential of these novel strategies and discusses their design strategies and recent advances.

Diclofenac prodrugs nanoparticles: An alternative and efficient treatment for rheumatoid arthritis?

Summary: New lipidic prodrugs of diclofenac were synthesized by grafting aliphatic chains to diclofenac through an ester bond. The nanoparticles of the prodrugs were successfully formulated and exhibited good stability. The prodrug nanoparticles showed dose-dependent inhibition of TNF-α comparable to standard diclofenac sodium.

PLA-PEG forming worm-like nanoparticles despite unfavorable packing parameter: Formation mechanism, thermal stability and potential for cell internalization

Summary: Most nanoparticles used for drug delivery are spherical, but elongated particles have advantages in cellular uptake. In this study, we synthesized biocompatible polylactide-b-poly(ethylene glycol) (PLA-PEG) polymers with carboxylate moieties and used them to form worm-like nanoparticles. By adjusting the molar mass of the PLA block, we obtained worm-like nanoparticles with varying aspect ratios. The proportion of worm-like particles increased as the molar mass of the PLA block decreased, contrary to the usual behavior of block polymers. These anisotropic nanoparticles showed increased cellular uptake compared to their spherical counterparts, highlighting their potential for drug delivery applications.

Modulation of release and pharmacokinetics from nanoscale lipid prodrugs of dexamethasone with variable linkage chemistry

Summary: In this study, lipid-drug conjugates (LDCs) of dexamethasone (DXM) with different lipid-drug linkages were synthesized and turned into nanoscale particles. The encapsulation efficacy of LDCs was above 95%, with good stability and no recrystallization observed. Ester and carbonate LDC nanoparticles exerted anti-inflammatory activity, while the carbamate LDC nanoparticles were toxic to macrophages and discarded. The release rate of DXM from ester particles was faster than that from carbonate particles, leading to a lower exposure to DXM from carbonate LDC nanoparticles in pharmacokinetics and biodistribution studies.

3D human foreskin model for testing topical formulations of sildenafil citrate

Summary: This study aimed to investigate the loading of sildenafil citrate in three commercial transdermal vehicles using 3D full-thickness skin equivalent and compare the results with permeability experiments using porcine skin. The results showed that the results obtained using the 3D skin equivalent were comparable to those obtained using porcine skin, suggesting that the 3D skin model can be a valid alternative for ex-vivo skin absorption experiments.

Large volume subcutaneous delivery using multi-orifice jet injection
