Summary: This study found that perioperative administration of 60mg duloxetine daily significantly reduced pain severity upon movement, resting pain, morphine requirements, and increased satisfaction level at discharge within 3 weeks after total hip arthroplasty. However, there was no clinically meaningful difference between the duloxetine group and the placebo group in terms of pain reduction. Adverse events did not differ between groups.

Summary: This study identifies the relationship between preoperative opioid use and increased complications after primary total hip arthroplasty (THA). The results show that higher opioid doses are associated with a higher risk of emergency department visits, readmissions, dislocation, infection, and revision surgery. Continued education on the harmful effects of opioids prescribed for nonoperative treatment of osteoarthritis is still necessary.

Summary: This study aimed to investigate the association between the size of initial opioid prescription and the likelihood of refill after elective primary total knee and hip arthroplasty. National datasets of insurance claims were retrospectively analyzed, and a cohort of opioid-naive patients who underwent surgery between January 2015 and November 2019 was identified. The refill rate within 30 days after discharge was the primary outcome, and the total amount of opioid filled in the initial prescription was the predictor variable. The results showed that larger initial opioid prescription size was associated with small decreases in 30-day refill rate after total joint arthroplasty, suggesting that concerns about decreasing postsurgical opioid prescribing may not be warranted.

Summary: This retrospective cohort study aimed to investigate the correlation between pain relief after injection and arthroplasty. The study included 88 patients who received intra-articular hip injection before primary total hip arthroplasty. Pain scores were compared at different stages, and it was found that the response to injection could predict pain relief after arthroplasty. The response to local anaesthetic in the diagnostic phase might be a better predictor than corticosteroids. Majority of patients with osteoarthritis can expect better pain relief after arthroplasty compared to the therapeutic phase after injection.

Summary: The study conducted a systematic search and analysis of analgesic trials for total hip arthroplasty (THA), finding that in most patients undergoing THA under general anaesthesia, postoperative acute pain should subside to a mild level within 4 hours. However, there remains a substantial knowledge gap in the management of patients with chronic pain and opioid dependence.

Summary: Overprescription of pain medications contributes to the opioid epidemic and affects veterans significantly. The implementation of an opioid taper protocol resulted in reduced administration of opioids postdischarge. The study suggests that taper protocols should be considered for postoperative pain management.

Summary: This study found that perioperative dexamethasone is associated with reduced length of stay and decreased postoperative complications after total joint arthroplasty. It was found to lower the risk of pulmonary embolism, deep vein thrombosis, postoperative nausea and vomiting (PONV), acute kidney injury, and urinary tract infection. However, it did not significantly reduce postoperative opioid utilization.

Summary: This study aimed to assess the relationship between prednisone dosage and rates of periprosthetic joint infection (PJI) following total joint arthroplasty (TJA). The study found that patients receiving prednisone therapy were at increased risk of PJI and sepsis, but no dose-dependent relationship between prednisone and infectious complications was identified.

Summary: The study found that EVs in human biofluids mainly contain HO-1 protein, with the majority of HO-1 protein localized in the extracellular vesicles. These findings suggest that a significant proportion of EV HO-1 in peripheral human biofluids is derived from the central nervous system and may contribute to the systemic manifestations of various neurological conditions.

Summary: This study found that higher levels of salivary HO-1 expression were detected in patients experiencing an acute psychotic episode compared to control participants, and this overexpression was positively associated with psychiatric symptom severity and disability. The results suggest that overexpression of HO-1 in saliva may serve as a potential biomarker for psychosis and should be explored further in larger clinical trials.

The authors highlight the importance of identifying reliable biomarkers to improve early disease stage treatment with specific medications. Key biomarkers include CGRP, glutamate, nerve growth factor, inflammatory markers, and oxidative stress markers. New genetic and epigenetic biomarkers are emerging candidates for future research.

Summary: This study compares the likelihood of back and neck pain between acute and chronic pain-related TMD patients. The results show that compared to acute TMD, chronic TMD patients are twice as likely to report neck pain, but not back pain. Chronic disability is also associated with an increased likelihood of neck pain, but not back pain.
