Propofol attenuates the decrease of dynamic compliance and water content in the lung by decreasing oxidative radicals released from the reperfused liver

BACKGROUND: Remote Pulmonary injuries after hepatic reperfusion are frequently caused by reactive oxygen species (ROS)-induced damage. The choice of anesthetics may affect the balance between oxidants and antioxidants, and propofol, a commonly used anesthetic, has an antioxidant effect. In this study, we developed a model to study pulmonary function with hepatic ischemia/reperfusion (I/R) manipulation, with the aim of defining remote Pulmonary dysfunction after hepatic reperfusion and determining if propofol affects this dysfunction by altering ROS production from the liver or lungs. METHODS: Adult male rats weighing 160-250 g were randomly divided into four groups according to the type Of Surgery (sham or I/R) and the anesthetic administered (pentobarbital or propofol). To induce I/R, the portal vein and hepatic artery to the left and medial lobes of the liver were clamped. All of the measurements were done after 5 h of reperfusion, after 45 min of ischemia. Pulmonary function after hepatic I/R was determined by dynamic compliance, resistance and wet-to-dry ratio, and by histopathology. Hepato-cellular injuries were confirmed by alanine aminotransferase, whereas ROS production was measured from the inferior vena cava, jugular vein, and carotid artery. Products of lipid peroxidation, thiobarbiturate acid reactive substances and malondialdehyde, were measured in lung and hepatic tissues. RESULTS: Remote lung injury after hepatic I/R was shown by a significant decrease of Cdyn, and increases in resistance and the wet-to-dry ratio. ROS production was significantly increased and was highest in samples from the inferior vena cava. Thiobarbiturate acid reactive substances and malondialdehyde in the liver and serum alanine aminotransferase were significantly increased only in the I/R+pentobarbital group. All of the changes were significantly attenuated in the I/R+ propofol group (P = 0.05). With propofol infusion, there was decreased ROS production from the reperfused liver, with less hepato-cellular injury, followed by well-maintained pulmonary function. CONCLUSION: Remote pulmonary dysfunction and reperfusion injury in the liver were demonstrated in our rat model, as well as massive ROS production and lipid peroxidation. Propofol infusion attenuated remote pulmonary injury by lessening oxidative injury from the reperfused liver.

Interval dynamics of transplantability for hepatocellular carcinoma after primary curative resection: risk factors for nontransplantable recurrence

Summary: This study investigated the changes in transplantability between primary and recurrent Hepato-cellular carcinoma (HCC) after hepatic resection (HR) and identified risk factors for nontransplantable recurrence (NTR). The study found that tumor diameter, resection extent, pathological grade, microvascular invasion, and early recurrence were associated with NTR. The overall transplantable pool increased after HR, suggesting that liver transplantation may not be necessary for some patients.

Longitudinal analysis of transplant candidates with primary sclerosing cholangitis in an Asian liver transplant center

Summary: Primary sclerosing cholangitis (PSC) is a rare disease in Asia and little research has been conducted on this ethnic group, especially in wait-listed patients for liver transplantation (LT). This study aimed to investigate the prognosis and outcomes of wait-listed patients with PSC in an Asian transplant center. The results showed that female PSC patients had higher AST to platelet ratio index scores and bilirubin levels compared to males, and were more likely to present with decompensating events. Female gender and LT were identified as protective factors for overall survival in multivariable analysis.

Comparisons on the intraoperative desaturation and postoperative outcomes in non-intubated video-assisted thoracic surgery with supraglottic airway devices or high-flow nasal oxygen: A retrospective study

Summary: This study compared the intraoperative oxygenation and perioperative outcomes between non-intubated video-assisted thoracic surgery (NIVATS) with supra-glottic airway devices (SADs) and NIVATS with high flow nasal oxygenation (HFNO). The results showed that the HFNO group had a higher desaturation event rate, lower nadir SPO2, and longer hospitalization. However, propensity score matching analysis revealed no significant between-group difference in the desaturation rate. Smoking and HFNO were associated with intraoperative desaturation.

Viral Status and Treatment Efficacy in Recurrent Hepatocellular Carcinoma After Primary Resection

Summary: This retrospective study analyzed 726 patients who experienced intrahepatic recurrence after primary hepatectomy for hepatocellular carcinoma (HCC). The results showed that both radiofrequency ablation (RFA) and transarterial chemoembolization (TACE) were effective in improving long-term survival, particularly in patients with hepatitis B virus (HBV). Antiviral treatment complemented the survival of patients with hepatitis C virus (HCV) after RFA, especially in late first recurrence.

Efficacy of intraoperative thoracoscopic intercostal nerve blocks in nonintubated and intubated video-assisted thoracic surgery: A randomized study

Summary: This study compared the analgesic and sedative effects of thoracoscopic intercostal nerve blocks (TINBs) during nonintubated and intubated video-assisted thoracic surgery (VATS). The results showed that TINBs reduced the requirement for anesthesia and analgesia in VATS. However, nonintubated VATS had a significantly higher risk of hypotension after TINBs. Real-time data provided by density spectral arrays (DSAs) were beneficial for the preemptive management, especially in nonintubated VATS.
