Coronary Artery Calcium Screening: Does it Perform Better than Other Cardiovascular Risk Stratification Tools?

Coronary artery calcium (CAC) has been advocated as one of the strongest cardiovascular risk prediction markers. It performs better across a wide range of Framingham risk categories (6%-10% and 10%-20% 10-year risk categories) and also helps in reclassifying the risk of these subjects into either higher or lower risk categories based on CAC scores. It also performs better among population subgroups where Framingham risk score does not perform well, especially young subjects, women, family history of premature coronary artery disease and ethnic differences in coronary risk. The absence of CAC is also associated with excellent prognosis, with 10-year event rate of 1%. Studies have also compared with other commonly used markers of cardiovascular disease risk such as Carotid intima-media thickness and highly sensitive C-reactive protein. CAC also performs better compared with carotid intima-media thickness and highly sensitive C-reactive protein in prediction of coronary heart disease and cardiovascular disease events. CAC scans are associated with relatively low radiation exposure (0.9-1.1 mSv) and provide information that can be used not only for risk stratification but also can be used to track the progression of atherosclerosis and the effects of statins.

Summary: This study explores the factors in the serum of patients with rheumatoid arthritis (RA) and their mechanisms of interaction with and influence on the cholesterol loading capacity (CLC) of macrophages. The results show that oxidized LDL can directly influence CLC in dual-seropositive RA patients. Two additional and independent pathways, through anti-oxLDL IgG and PCSK9, may mediate the effects of oxPL-apoB100 on CLC, depending on CRP and seropositivity status.

Summary: Coronary computed tomography angiography (CCTA) has significantly improved the imaging of adverse coronary plaque features. Recent findings have shown that quantitative and qualitative assessment of coronary plaque by CCTA can enhance the prediction of future major adverse cardiovascular events. It can also aid in the identification of high-risk non-obstructive coronary plaque, differentiate myocardial infarction types, and track disease progression and response to medical therapy.

Summary: This study aimed to investigate the associations of plasma TMAO levels and its changes over time with all-cause and cause-specific mortality. The study included 6,785 adults and found that higher plasma TMAO levels were associated with increased risks of all-cause mortality, cardiovascular disease mortality, and death due to kidney failure.

Summary: People with HIV have a higher risk of myocardial infarction, with a greater proportion of type 2 MI due to oxygen demand-supply mismatch compared with type 1 resulting from atherothrombotic plaque disruption. HIV-positive individuals report a higher prevalence of cigarette and alcohol use. However, the impact of smoking and alcohol consumption on MI type among people living with HIV is not well studied.
