A 6-year-old boy from India developed an atypical form of rabies following a stray dog bite and as a consequence of not receiving the standard World Health Organization recommended post-exposure prophylaxis for category III wounds. Serial rising rabies virus neutralizing antibody titres in serum and cerebrospinal fluid by rapid fluorescent focus inhibition test helped confirm the diagnosis of rabies. The child has survived for 4 months since the onset of illness, albeit with neurological sequelae. (C) 2015 The Authors. Do school students with specific learning disabilities have lower emotional intelligence abilities? A cross-sectional questionnaire-based study in Mumbai, Maharashtra, India

S. Karande, S. Bhavani, N. J. Gogtay, M. P. Shiledar, S. Kelkar, A. S. Oke

Summary: This study evaluated the emotional intelligence abilities of students with specific learning disabilities and analyzed the impact of socio-demographic variables on their abilities. The results showed that these students had similar emotional intelligence abilities as their regular peers, but scored lower in the school setting and higher in the social setting. The presence of co-occurring attention-deficit/hyperactivity disorder and higher socioeconomic status were associated with lower and higher scores, respectively.

Summary: This study investigates the role of mitochondrial changes in rabies by analyzing human and canine brain tissues infected with the Rabies lyssavirus (RABV). The findings suggest that altered mitochondrial bioenergetics and cristae architecture may induce mitophagy, leading to autophagy and subsequent neuronal dysfunction in rabies.

Major Histocompatibility Complex (MHC) Class II Deficiency- A Case of Primary Immunodeficiency Disorder

Ramesh Radhika, Milind S. Tullu, Sunil Karande, Umair Ahmed Bargir

Summary: Major Histocompatibility Complex (MHC) Class II deficiency is a rare autosomal recessive primary immunodeficiency caused by mutations in regulatory genes of MHC Class II proteins. This case report describes a 9-month-old girl presenting with repeated pneumonia, lymphocytopenia, and absence of HLA-DR expression on B cells, which led to the diagnosis of MHC Class II deficiency. Genetic analysis identified a homozygous mutation in the RFX-5 gene. Despite the patient's unfortunate outcome, the importance of prenatal diagnosis was emphasized to the parents.

Summary: Rabies, a lethal zoonotic encephalitis, is a significant global health concern. This study evaluated the efficacy of the Truenat rabies assay, a rapid and portable PCR-based system, for diagnosing rabies in humans and animals. The Truenat assay showed high sensitivity and specificity in both canine and human samples. This method offers a robust and affordable point-of-care solution to enhance rabies diagnostic capacity in endemic areas.

Porphyrias: Uncommon disorders masquerading as common childhood diseases

A. Chakraborty, M. Muranjan, S. Karande, V Kharkar

Summary: Porphyrias are rare inborn errors of metabolism caused by defects in the heme biosynthetic pathway, leading to overproduction of porphyrin precursors and porphyrin species. Diagnosing porphyrias is challenging due to overlapping symptoms with common diseases. This case series highlights the importance of logical analysis of symptoms, judicious selection of investigations, and genotyping in successfully diagnosing porphyrias.
