Recommended first line treatments for giardiasis include the nitroimidazoles and, recently, nitazoxanide. For refractory cases, a combination of two or more drugs may be a viable approach. A review of 10 patients with giardiasis refractory to nitroimidazoles with response to a short course (< 2 weeks), combined treatment is presented.

Norman, Fernando Salvador, Beatriz Gullon, Marta Diaz-Menendez, Ana Perez-Ayala, Azucena Rodriguez-Guardado, Magdalena Garcia-Rodriguez, Cesar Henriquez-Camacho, Josune Goikoetxea, Pau Bosch-Nicolau, Fernando Calle, Jose-Manuel Ramos-Rincon, Maria-Luisa Aznar, Maria Penaranda, Eva Calabuig, Ines Suarez-Garcia, Jose A. Perez-Molina, Rogelio Lopez-Velez

Summary: This study describes the main characteristics of migrants diagnosed with HTLV infection in the +Redivi Spanish network. The majority of the patients were asymptomatic immigrants from South America, with a high association with strongyloidiasis. There was a missed opportunity for screening potential contacts, leading to underdiagnosis of this infection.

Summary: Post-kala-azar dermal leishmaniasis caused by Leishmania infantum in an HIV-negative patient is rare, and this case highlights its association with other types of immunosuppression. In addition, this case report presents an effective therapeutic option with miltefosine, which can be very useful due to limited reported experience.

Summary: This study investigates the impact of two different dietary patterns on gut microbiota and inflammatory biomarkers in HIV-infected individuals. The results demonstrate that a Mediterranean-like diet pattern is associated with lower inflammation levels and favorable abundance of gut bacteria.

Use of benznidazole to treat chronic Chagas disease: An updated systematic review with a meta-analysis

Clara Crespillo-Andujar, Belen Comeche, Davidson H. Hamer, Ingrid Arevalo-Rodriguez, Noelia Alvarez-Diaz, Javier Zamora, Jose A. Perez-Molina

Summary: This study conducted a meta-analysis to evaluate the effectiveness of benznidazole in the treatment of Chagas disease. The results showed that benznidazole is recommended for children with Chagas disease based on serological response, but its efficacy in adults remains uncertain. Further clinical data is needed to assess the efficacy of benznidazole in both children and adults with Chagas disease.

Interactions among the mycobiome, bacteriome, inflammation, and diet in people living with HIV

Summary: This study reveals strong inter-kingdom interactions between the mycobiome and bacteriome in individuals with HIV. The mycobiome in HIV patients is distinct and enriched with specific fungal species, which are correlated with inflammatory cytokines and immune cell counts. Additionally, diet, specifically vegetable and fiber intake, affects the composition of the mycobiome.

Summary: This study describes the microbiological, clinical, and epidemiological characteristics of imported rickettsioses in travellers and migrants in Spain. The results show that there were 54 cases of imported rickettsioses from 2009 to 2020, with most patients being male and a median age of 37 years. The most common symptoms were febrile syndrome and cutaneous manifestations. Most diagnoses were confirmed by serology, and only a few cases required hospitalization. This study highlights the importance of considering rickettsial diseases in travellers from high-risk areas who have engaged in risky activities.

Summary: Human papillomavirus can lead to HSILs in the anogenital area, and the microbiome may play a role. HIV-MSM individuals have a high risk of anal cancer, but current screening methods for HSILs lack specificity. This study found that elevated levels of microbiome-encoded proteins were associated with HSILs, and measuring these biomarkers improved the current strategy of anal cancer screening.

Is It Time for Treatment as Prevention of Chronic Hepatitis B?

Jose A. Perez-Molina, Marta Rosas Cancio-Suarez, Santiago Moreno

Summary: Hepatitis B is a major global health problem, with a large number of people carrying the virus and continuing transmission. Current treatment guidelines do not include many chronically infected patients or pregnant women, making it difficult to control the spread. Expanding and facilitating treatment as a public health intervention could help achieve the United Nations' goal for 2030.

Mortality due to non-AIDS-defining cancers among people living with HIV in Spain over 18 years of follow-up

I. Suarez-Garcia, Felix Gutierrez, Jose A. Perez-Molina, Santiago Moreno, Teresa Aldamiz, Eulalia Valencia Ortega, Adrian Curran, Sara Gutierrez Gonzalez, Victor Asensi, Concha Amador Prous, Inma Jarrin, Marta Rava

Summary: The aim of this study was to describe the mortality rate of non-AIDS-defining cancer (NADC) among people living with HIV (PLWH), compare it with the general population, and assess potential risk factors. The study found that NADC is an important cause of death among PLWH, with an increased risk associated with immunodeficiency, smoking, and coinfections. This highlights the need for early detection and treatment of cancer in this population.
