Summary: The study aimed to develop a novel risk stratification scoring system specific to living donor liver transplantation (LDLT) recipients in order to improve the accuracy of predicting short-term outcomes. Based on the Sequential Organ Failure Assessment (SOFA) score and other clinical factors, the GRWR-SOFA score system was constructed, dividing patients into four classes with varying mortality rates. The model showed superior discriminatory power for predicting short-term mortality and helps clinicians identify appropriate care levels for different patient subgroups receiving LDLT.

Summary: Living donor liver transplantation (LDLT) is more common in Asia than in North America, where living donor kidney transplantation, including paired exchanges for ABO-incompatible pairs, is prevalent. The first liver paired exchange (LPE) in the United States, completed over two consecutive days, was initiated by a non-directed donor and benefited an ABO-incompatible pair. This innovative approach demonstrates the feasibility of LPE in centers with experience in LDLT, offering new possibilities for transplantation in cases of ABO incompatibility.

Summary: This study analyzed the risk factors and impact of cytomegalovirus (CMV) infection on the survival of living donor liver transplantation (LDLT) patients. The results showed that CMV infection was an independent risk factor for graft survival. Female sex, pretransplant MELD score, pretransplant in-hospital stay, ABO incompatibility, donor macrovesicular steatosis >= 10%, and re-operation before index postoperative day were identified as independent risk factors for CMV infection. These risk factors should be considered in the surveillance and treatment of CMV infections after LDLT.

Summary: The study compares the risk and prognostic factors of PTLD between adult kidney and liver transplant recipients. The results showed that PTLD develops earlier in liver transplant recipients, especially in patients over 45 years old, those receiving tacrolimus or not receiving cyclosporin. In kidney transplant recipients, PTLD onset is significantly affected by tacrolimus use, male sex, and age over 45 years old. Survival is longer in liver transplant recipients under 45 years old and they are more likely to achieve complete remission. The factors affecting PTLD development and outcome differ between kidney and liver transplant recipients.

Summary: Recurrence of autoimmune hepatitis (AIH) is common after liver transplantation, and it is associated with younger age at transplantation, the use of mycophenolate mofetil post-transplant, sex mismatch, and high pre-transplant IgG levels. Recurrent AIH is associated with impaired graft and overall survival. Better characterization, prevention, and treatment strategies are needed for the management of recurrent AIH.

Summary: This study analyzed a database of simultaneous liver-kidney transplant recipients and identified factors associated with poor post-transplant survival. Older age, higher liver disease severity, diabetes, dialysis prior to transplant, and ICU admission were all associated with lower survival rates. Similarly, receiving organs from older donors with poorer kidney function profiles also resulted in lower survival rates.
