Summary: This study comprehensively analyzed the immune heterogeneity in islet transplantation using single-cell RNA sequencing. T lymphocytes and myeloid cells were found to be the main immune components of grafts, especially in allografts. Furthermore, allogeneic islet cells were transformed into antigen-presenting cell-like cells, which may promote the interaction with cytotoxic CD8(+) T cells and lead to the destruction of islet allografts.

Furthermore, pregnancy and postpartum-induced changes varied within each endocrine cell type, as well as in endothelial cells and antigen-presenting cells within islets.

Summary: This study assessed the prevalence of estimated glomerular filtration rate (eGFR) abnormalities in children with type 1 diabetes (T1D) and explored the relationship between clinical characteristics and eGFR abnormalities. The findings showed that a notable proportion of children with T1D displayed eGFR abnormalities early on, which is concerning for long-term risk of chronic kidney disease (CKD), particularly in adolescence.

Summary: Pancreatic islet transplantation has therapeutic potential in type 1 diabetes and chronic pancreatitis, but the long-term outcomes are modest. This study analyzed prohormone peptide levels in patients and mouse models of islet transplantation and found that higher proinsulin-to-C-peptide and proIAPP-to-total IAPP ratios were associated with insulin dependency. Histological analysis also revealed altered prohormone processing in failing grafts.
