Summary: Liver transplantation is an effective treatment option for nonresectable patients with early-stage HCC, especially when they meet the Milan criteria. However, the use of CNIs for immunosuppression increases the risk of tumor regrowth. mTOR inhibitors have emerged as an alternative immunosuppressive approach that can address both immunosuppression and cancer control. This review summarizes the roles of mTOR inhibitors in managing HCC patients undergoing LT and proposes strategies to overcome common adverse effects.

Summary: Clinical trials studying immune checkpoint inhibitors (ICIs) have excluded immunocompromised patients. This study reviewed the existing literature and analyzed treatment outcomes of 144 patients. The findings suggest that effective immunotherapy with retained transplant was achieved in approximately 30.8% of patients. The overall response rates of immunotherapy were similar to non-immunocompromised cancer patients, but publication bias may overestimate positive outcomes.

Summary: In the last two decades, advancements in organ preservation, surgical techniques, and personalised immunosuppression have reduced the occurrence of acute rejections and early post-transplant complications. However, long-term graft survival rates have remained stagnant, possibly due to chronic calcineurin inhibitor toxicity. Skin cancer, particularly non-melanoma skin cancer (squamous cell carcinoma and basal cell carcinoma), is the most common malignancy among Caucasian solid organ transplant recipients. Various factors, including immunosuppression, contribute to the susceptibility to skin cancers, which have higher mortality rates compared to the general population. Early identification and treatment, along with patient education on sun protection and self-diagnosis, are crucial preventive measures. Collaboration between transplant clinicians, dermatologists, and surgeons is essential for effective management of these complications.

Summary: Post-transplant lymphoproliferative disorder (PTLD) is a spectrum of lymphoid conditions associated with Epstein Barr Virus (EBV) and immunosuppressive drugs. It remains a major cause of morbidity and mortality after heart transplantation. EBV is a key driver in many PTLD cases, with B-cell proliferation and impaired T-cell immune surveillance playing a crucial role. This article reviews the pathogenesis, recent advances, and potential therapeutic targets to reduce the burden of PTLD after heart transplantation.

Dysregulation of this pathway is associated with cancer development and resistance to therapy. This review focuses on the major dysregulations in the PAM signaling pathway in cancer and discusses strategies for overcoming treatment resistance. The role of PAM signaling in immunology and immunotherapies is also discussed.

Summary: The DZIF Transplant Cohort is a multicentre prospective cohort study focusing on infectious complications after solid organ and stem cell transplantation. It aims to understand the risk factors associated with infections, collect detailed data and biosamples to facilitate studies in infectious diseases and transplant medicine. The study has collected baseline data and biosamples from approximately 1400 patients as of January 2020, with plans to recruit 3500 patients by 2023 and continue follow-up visits and documentation of infectious events until at least 2025.

Summary: This study investigated the underlying diseases, treatments, and outcomes of patients with eosinophilic annular erythema (EAE). The results showed that topical or systemic corticosteroids, hydroxychloroquine, and dapsone were the most common and efficient treatments. In refractory patients, a combination of systemic corticosteroids with hydroxychloroquine achieved a high complete clinical response rate.

Summary: The objective of this study was to describe the clinicopathological features of Merkel cell carcinoma (MCC) in solid-organ transplant recipients (SOTR) and identify factors associated with tumor outcomes. The results showed that the proportion of MCC patients with Merkel cell polyomavirus (MCPyV) was low in SOTR, and solid-organ transplantation was associated with an increased incidence of progression, MCC-specific mortality, and overall mortality. In SOTR, MCPyV positivity and switching to an mTOR inhibitor (mTORi) were associated with an increased incidence of progression.
