Summary: Pancreatic cysts are increasingly being used as a diagnostic tool due to the availability of high-quality imaging. These cystic lesions can be either neoplastic or non-neoplastic, with different management approaches required for each. Magnetic resonance imaging and endoscopic ultrasound are effective non-invasive diagnostic methods for evaluating pancreatic cysts. The future focus should be on developing methods to quickly diagnose high-grade dysplasia or early cancer in order to provide timely treatment and avoid unnecessary surgery or surveillance.

Summary: Accurate and reliable segmentation of the pancreas and its lesions on computed tomography (CT) images is crucial for medical imaging. In this study, we propose a Strongly Representative Semantic-guided Segmentation Network (SRSNet) that effectively utilizes original image features and explores semantic information. Our method achieves high performance in pancreas and pancreatic tumor segmentation, outperforming state-of-the-art algorithms.

Summary: The use of antibiotics for prophylaxis in pancreatic cystic lesions is still a debated topic. Current research lacks in quantity and has a small sample size, leading to biased results. Pooled data analysis shows that antibiotic prophylaxis does not have a significant impact on the rate of cyst infection. Larger international studies are needed to resolve this controversy.

Summary: Early detection and diagnosis of precursor lesions for pancreatic malignancy is crucial. Current diagnostic tools for cystic precursor lesions, such as CEA levels and cytology, have limitations. Molecular analysis of cystic fluid can improve the diagnosis and risk stratification of pancreatic malignancy precursor lesions.

Summary: This article reports a case of a 20-year-old male patient with a 14-year history of recurrent abdominal pain, who was found to have a persistent focal heterogeneous lesion affecting the tail of the pancreas. Surgical resection was performed, and the final pathology report revealed a gastric-type pancreatic duplication cyst. Similar misdiagnosed cases were also found in the literature. Therefore, a high index of suspicion is needed for pancreatic duplication cysts to avoid unnecessary hospital admissions and procedures.

Summary: Pancreatic cystic lesions (PCLs) are commonly found incidental findings on cross-sectional imaging. Magnetic resonance imaging (MRI) has become the preferred non-invasive method for predicting cyst type, stratifying risk of neoplasia, and monitoring changes in PCLs. In some cases, additional diagnostic approaches such as endoscopic ultrasound (EUS) and molecular analysis may be necessary for treatment decisions. The use of radiomics and artificial intelligence in MRI could enhance the stratification of PCLs and improve treatment guidance.

Summary: Pancreatic hydatid cysts are rare and often misdiagnosed. A history of living in an endemic area and a positive anti-echinococcus IgG antibody status can help with the diagnosis. Radical resection combined with oral albendazole is the standard treatment for pancreatic hydatid cysts.

Summary: This study investigated the application of MRI quantitative parameters in discriminating between mucinous and non-mucinous pancreatic cystic lesions. The results showed that signal intensity measurements were not reliable in differentiating PCLs, while radiomics features were able to successfully discriminate between the two entities.

Summary: In recent years, there has been a significant increase in age-related diseases worldwide due to longer life expectancy. The aging pancreas undergoes various morphological and pathological changes, which can predispose individuals to diseases such as diabetes, dyspepsia, pancreatic ductal adenocarcinoma, and pancreatitis. Pancreatic senescence is associated with underlying factors including genetic damage, DNA methylation, endoplasmic reticulum stress, mitochondrial dysfunction, and inflammation. This review article discusses the alterations in morphologies and functions of the aging pancreas, particularly beta-cells, and summarizes the mechanisms of pancreatic senescence to identify potential targets for treating aging-related diseases.

Summary: A 54-year-old female patient with abdominal pain was diagnosed with cholelithiasis, liver hydatid cyst, and hypodense multicystic mass lesion in the pancreatic body. Through endoscopic ultrasound examination, the pre-diagnosis of pancreatic hydatid cyst was made. Cystotomy and external drainage procedures can reduce surgical morbidity and mortality.

Summary: Embryological development of the pancreas can have multiple aberrations, but most ductal anomalies are asymptomatic. Accurate pre-operative diagnosis is important to prevent inadvertent duct injury during surgery. MRCP and EUS are effective tools for diagnosing ductal anomalies.

Summary: This case report describes a rare occurrence of localized lactoferrin amyloid deposition in the pancreas presenting as a mural nodule in a cyst, leading to pancreatoduodenectomy in a 69-year-old woman. It highlights the importance of considering amyloid deposition in the differential diagnosis of mural nodules in pancreatic cystic lesions seen on imaging, and the possibility of benignity in some cases.

Summary: This review examines the role of pancreatic steatosis in the development of pancreatic cancer through epidemiological studies and experimental animal models. It highlights the prevalence of pancreatic steatosis and its association with pancreatic cancer. The review also explores the cellular mechanisms underlying pancreatic steatosis and its impact on the tumor microenvironment, as well as the effects of bariatric surgery. Additionally, it identifies gaps in current knowledge and proposes future research directions for understanding the mechanisms of other obesity-associated cancers.

Summary: Diabetes mellitus, affecting nearly 536.6 million people worldwide, is primarily caused by the death or dysfunction of beta cells in the pancreas. Studies suggest that interactions between neighboring cells in the pancreas play a crucial role in supporting beta cell function.

Summary: This study reports the initial experience of using the Connect-EA, an esophageal magnetic compression anastomosis device, for salvage of primary repair in two high-risk complex esophageal atresia (EA) patients. Two approaches, totally endoscopic and hybrid operative, were successfully employed. The Connect-EA offers a novel intervention for delayed primary esophageal repair in complex EA patients, who have high-risk tissue characteristics and multi-system comorbidities that limit operative repair.

Summary: This study aimed to determine the resolution time of I-123-MIBG avidity after radiation therapy (RT) in neuroblastoma (NB) and the rate of local failure (LF). The median time to resolution of I-123-MIBG activity after RT was found to be 78 days. Primary lesions without residual disease showed excellent local control, while the LF rate was higher after RT for residual disease and there was no benefit observed for boost RT.

Summary: Gastroschisis is a common congenital anomaly where the midgut fails to return to the abdominal cavity, resulting in exposed intestines. The incidence is increasing globally, with complicated cases associated with gastrointestinal conditions leading to higher risks for patients. Treatment requires a multidisciplinary approach with a focus on bowel protection and closure of the abdominal wall defect.
