Pancreatic incidentalomas: clinical and pathologic spectrum

Background: Incidental abnormalities are increasingly being detected. The pathology and clinical outcome of patients with pancreatic incidentalomas have not been well characterized. Methods: We reviewed the records of 356 patients with pancreatic abnormalities from August 2001 to June 2007. Clinical and pathologic data were collected for a cohort of patients who had incidental pancreatic lesions detected by imaging modalities. Results: Fifty-seven pancreatic incidentalomas were identified. Ninety percent of them were detected by computed axial tomography (CT). The most frequent indications for imaging were genitourinary symptoms and cancer surveillance. Sixty percent of the lesions were solid, and 40% were cystic. Surgical resection was performed in 33 patients. Locally advanced disease was found in six patients, and metastatic disease was found in 9 patients. The most frequent diagnoses were ductal adenocarcinoma, neuroendocrine tumors, and serous cystadenoma. Conclusions: Patients with pancreatic incidentalomas account for a significant patient subgroup. Incidental pancreatic lesions occur frequently and require prompt surgical evaluation. (c) 2008 Elsevier Inc. All rights reserved.

Summary: A retrospective study found that laparoscopic harvesting of pedicled omental flap for cerebral revascularization in patients with Moyamoya disease is a safe and effective method. There were some complications during the procedure, but overall the length of hospital stay and improvement of symptoms were acceptable.

Leiomyomatosis in an Infant With a SUFU Splice Site Variant: Case Report

Summary: This study reports a case of a 5-month-old female with multiple intra-abdominal leiomyomata, who was found to have a likely pathogenic splice site variant in the SUFU gene. This is the first reported case of leiomyomatosis secondary to a pathogenic SUFU variant in an infant and may represent an early, atypical presentation of Gorlin syndrome.

Summary: This study reports the initial experience of using the Connect-EA, an esophageal magnetic compression anastomosis device, for salvage of primary repair in two high-risk complex esophageal atresia (EA) patients. Two approaches, totally endoscopic and hybrid operative, were successfully employed. The Connect-EA offers a novel intervention for delayed primary esophageal repair in complex EA patients, who have high-risk tissue characteristics and multi-system comorbidities that limit operative repair.

Summary: This study aimed to determine the resolution time of I-123-MIBG avidity after radiation therapy (RT) in neuroblastoma (NB) and the rate of local failure (LF). The median time to resolution of I-123-MIBG activity after RT was found to be 78 days. Primary lesions without residual disease showed excellent local control, while the LF rate was higher after RT for residual disease and there was no benefit observed for boost RT.

Summary: The study demonstrates a gradual increase in the utilization of MBS among adolescents and a significant shift from laparoscopic gastric bypass to sleeve gastrectomy. There has been a shift towards more diverse patient demographics, with an increase in Hispanics and patients with public insurance. The data also shows a trend of MBS cases moving towards teaching hospitals over the years.
