iNOS Deletion in Alveolar Epithelium Cannot Reverse the Elastase-Induced Emphysema in Mice

Marija Gredic, Vinita Sharma, Stefan Hadzic, Cheng-Yu Wu, Oleg Pak, Baktybek Kojonazarov, Julia Duerr, Marcus A. A. Mall, Andreas Guenther, Ralph T. T. Schermuly, Friedrich Grimminger, Werner Seeger, Simone Kraut, Natascha Sommer, Norbert Weissmann

Summary: This study aimed to investigate the effect of iNOS gene deletion on elastase-induced emphysema in mice. The results showed that iNOS gene deletion did not produce significant therapeutic effects on elastase-induced lung emphysema and pulmonary hypertension. Therefore, this study suggests that iNOS expression in pulmonary vascular or stromal cells might play a critical role in lung regeneration in emphysema.

Decreased plasma levels of the brain-derived neurotrophic factor correlate with right heart congestion in pulmonary arterial hypertension

Katharina Schaefer, Khodr Tello, Oleg Pak, Manuel Richter, Mareike Gierhardt, Grazyna Kwapiszewska, Christine Veith, Ludger Fink, Henning Gall, Matthias Hecker, Baktybek Kojonazarov, Simone Kraut, Kevin Lo, Jochen Wilhelm, Friedrich Grimminger, Werner Seeger, Ralph T. Schermuly, Hossein A. Ghofrani, Daniel Zahner, Rudiger Gerstberger, Norbert Weissmann, Akylbek Sydykov, Natascha Sommer

Summary: BDNF levels are decreased in patients with pulmonary hypertension and negatively correlated with right ventricular dilatation. However, decreased BDNF levels do not worsen RV dilatation in animal models.

The HDAC2-SP1 Axis Orchestrates Protumor Macrophage Polarization

Xiang Zheng, Poonam Sarode, Andreas Weigert, Kati Turkowski, Prakash Chelladurai, Stefan Guenther, Carsten Kuenne, Hauke Winter, Albrecht Stenzinger, Simone Reu, Friedrich Grimminger, Thorsten Stiewe, Werner Seeger, Soni Savai Pullamsetti, Rajkumar Savai

Summary: Tumor-associated macrophages (TAMs), including antitumor M1-like TAMs and protumor M2-like TAMs, play diverse roles in lung cancer development. It has been found that the spatial proximity of HDAC2-overexpressing M2-like TAMs to tumor cells is significantly correlated with poor overall survival of lung cancer patients. Suppression of HDAC2 in TAMs alters macrophage phenotype, migration, and signaling pathways related to interleukins, chemokines, cytokines, and T-cell activation.

Schermuly, Soni Savai Pullamsetti

Summary: The ability of the right ventricle to adapt to an increased pressure afterload determines survival in patients with pulmonary arterial hypertension. The Wnt signaling pathway plays an important role in the development of the right ventricle and may also be implicated in adult cardiac remodeling.

Fibroblast growth factor 23 as a biomarker of right ventricular dysfunction in pulmonary hypertension

Laila Widmann, Stanislav Keranov, Leili Jafari, Christoph Liebetrau, Till Keller, Christian Troidl, Steffen Kriechbaum, Sandra Voss, Mani Arsalan, Manuel J. J. Richter, Khodr Tello, Henning Gall, Hossein A. A. Ghofrani, Stefan Guth, Werner Seeger, Christian W. W. Hamm, Oliver Doerr, Holger Nef

Summary: This study investigated the role of fibroblast growth factor 23 (FGF-23) as a biomarker for right ventricular (RV) function in patients with pulmonary hypertension (PH). The results showed that PH patients had higher levels of FGF-23 compared to healthy controls. Higher FGF-23 levels were associated with worse renal function and RV dysfunction. Therefore, FGF-23 may serve as a biomarker for maladaptive RV remodeling in patients with PH.

Elsa T. Roxlau, Oleg Pak, Stefan Hadzic, Claudia F. Brandes, Matthias Hecker, Friedrich Grimminger, Hossein A. Ghofrani, Ralph T. Schermuly, Werner Seeger, Natascha Sommer, Norbert Weissmann

Summary: The long-term effects of nicotine in e-cigarette vapour on the airways, lung parenchyma, and vasculature are unclear. In vitro and in vivo experiments showed that nicotine induced changes in gene expression and functional alterations in epithelial cells and smooth muscle cells. Nicotine also increased pulmonary endothelial permeability and caused inflammation and parenchymal alterations. Therefore, the inclusion of nicotine in e-cigarettes can lead to significant damage to the lungs.

Estimated plasma volume status: association with congestion, cardiorenal syndrome and prognosis in precapillary pulmonary hypertension

Summary: In patients with idiopathic pulmonary arterial hypertension or chronic thromboembolic pulmonary hypertension, estimated plasma volume status is associated with central venous congestion and prognosis. Patients with high estimated plasma volume status and without edema have a poorer transplant-free survival rate, which may represent an under-recognized subgroup.

Clinical and functional relevance of right ventricular contraction patterns in pulmonary hypertension

Summary: This study aimed to assess the relationship between right ventricular contraction pattern and right ventricular-pulmonary arterial (RV-PA) coupling. The results showed that early RV-PA uncoupling is associated with reduced longitudinal function, while advanced uncoupling leads to reduced anteroposterior movement and left ventricular preload. These findings are important for risk assessment in patients.

Clinical Utility of Pepsin and Bile Acid in Tracheal Secretions for Accurate Diagnosis of Aspiration in ICU Patients

Summary: This study evaluates the diagnostic value of gastric biomarkers, such as pepsin and bile acid, in detecting aspiration in the intensive care unit (ICU). The results indicate that measuring pepsin and bile acid does not reliably diagnose aspiration or significantly impact survival outcomes. Further validation studies are needed to assess the clinical usefulness and reliability of these biomarkers in diagnosing aspiration-related conditions.

Summary: Through experimental and clinical studies, it has been discovered that macrophage-derived CXCL10 plays a crucial role in the pathogenesis of BPD by causing macrophage chemotaxis through CXCR3, leading to lung fibrosis and alveolarization arrest. Targeting the CXCL10-CXCR3 axis could offer a new therapeutic approach for BPD.
