Gene expression analysis revealed T-cell activation and upregulation of inflammatory pathways in ConB patients.

Ultraviolet light oxidation of fresh hemoglobin eliminates aggregate formation seen in commercially sourced hemoglobin

Summary: Elevated levels of circulating cell-free hemoglobin (CFH) play a significant role in several clinical conditions, and it is important to study the pathophysiology of these diseases using commercially sourced CFH. This study found that treating endothelial cells with commercial ferric hemoglobin led to the formation of dense cytoplasmic aggregates (CAgg), while fresh ferric hemoglobin did not. Chemical analysis revealed that these CAgg contain inorganic components and are not pure hemoglobin, suggesting that stabilizers and additives in commercially sourced CFH contribute to CAgg formation.

Summary: Reactivation of Epstein-Barr virus (EBV) is often seen in lung transplant recipients (LTRs), but the cellular immune responses to EBV in adult LTRs have not been fully described. In this study, we found significant changes in CD4/CD8 ratio, EBV-specific CD8(+)CD69(+) T cell responses, and NK cell phenotypic changes in adult LTRs with EBV-associated diseases. These findings provide valuable insights into the immune responses to EBV in adult LTRs and may contribute to improved management of EBV-related complications.

JAK-STAT activation contributes to cytotoxic T cell-mediated basal cell death in human chronic lung allograft dysfunction

Summary: Chronic lung allograft dysfunction (CLAD), characterized by decline in pulmonary function and bronchiolitis obliterans, lacks approved therapies. Single-cell RNA-Seq and spatial transcriptomic analysis identified the important role of JAK-STAT signaling in airway epithelial cells in CLAD. Activation of JAK-STAT signaling leads to upregulation of MHC-I in airway basal cells, resulting in cytotoxic T cell-mediated basal cell death. This study provides mechanistic insight and potential therapeutic strategy for CLAD.

Summary: A novel frailty scale specifically designed for lung transplant candidates has been developed in this study, showing better performance in evaluating frailty compared to existing measures. Through a four-step approach, multiple aspects of frailty were identified and measured, and models were built and improved by selectively adding variables. The newly developed lung transplant frailty scale demonstrated superior predictive validity in predicting mortality and functional recovery after lung transplantation.

Opportunities for improved clinical trial designs in acute respiratory distress syndrome

Summary: Acute respiratory distress syndrome (ARDS) is a common and high-mortality critical illness with no proven pharmacological treatments. The current definition does not capture the heterogeneity of clinical risk factors, imaging characteristics, physiology, timing of onset and trajectory, and biology of the syndrome. There is increasing interest in designing clinical trials that reduce heterogeneity in the trial population, while also working towards a global definition of ARDS. This article recommends strategies to improve future ARDS trials, including targeting treatable traits and increasing the representativeness of the trial population.
