Morty, Heiko Slanina, Christian G. Schuttler, John Ziebuhr, Shadi Kassoumeh, Claudio Ronco, Fiorenza Ferrari, Klaus Warnatz, Klaus Stahl, Benjamin Seeliger, Marius M. Hoeper, Tobias Welte, Sascha David

Summary: This study found that IgG deficiency is common in critically ill patients with COVID-19, leading to increased disease severity, higher risk of complications, and worse prognosis. Therefore, IgG deficiency should be considered as a potential therapeutic target and marker of disease severity in the treatment of COVID-19.

Summary: This study aimed to assess the physiological and prognostic impact of right ventricular (RV)-pulmonary arterial (PA) uncoupling during volume challenge in patients with precapillary pulmonary hypertension (PH). The results showed that RV-PA coupling significantly declined during fluid challenge in patients with PH and was associated with pulmonary resting hemodynamics and RV ejection fraction (RVEF). Furthermore, RV-PA uncoupling during fluid challenge and RVEF were predictors of clinical worsening.

Clathrin-Mediated Albumin Clearance in Alveolar Epithelial Cells of Murine Precision-Cut Lung Slices

Vitalii Kryvenko, Andres Alberro-Brage, Athanasios Fysikopoulos, Miriam Wessendorf, Khodr Tello, Rory E. Morty, Susanne Herold, Werner Seeger, Christos Samakovlis, Istvan Vadasz

Summary: A study investigated the mechanisms of cellular albumin uptake in a three-dimensional culture of precision-cut lung slices (PCLS). The study found that PCLS cells incorporate labeled albumin in a time- and concentration-dependent manner and identified a receptor-driven internalization process for albumin uptake. The findings highlight the potential for improving outcomes in patients with respiratory failure through pulmonary drug delivery.

Scott Budinger, Cara J. Gottardi, Jacob I. Sznajder

Summary: Persistent symptoms and radiographic abnormalities suggestive of failed lung repair are common in COVID-19 patients after hospital discharge. Hypercapnia, which often occurs in mechanically ventilated patients, limits the proliferative capacity of alveolar type II cells and impairs lung repair after injury. Understanding the role of hypercapnia in lung repair is important for preventing complications and improving patient outcomes.

Influenza virus decreases albumin uptake and megalin expression in alveolar epithelial cells

Andres Alberro-Brage, Vitalii Kryvenko, Christina Malainou, Stefan Guenther, Rory E. Morty, Werner Seeger, Susanne Herold, Christos Samakovlis, Istvan Vadasz

Summary: Acute respiratory distress syndrome (ARDS) is a common complication of influenza virus (IV) infection, which leads to the accumulation and impairment of alveolar protein. It has been found that IV infection inhibits the clearance of alveolar protein, possibly through upregulation of a proteinase called MMP. In addition, inhibition of MMP proteinase may improve the clearance of alveolar protein and thus provide a potential therapeutic approach for ARDS.

Clinical Utility of Pepsin and Bile Acid in Tracheal Secretions for Accurate Diagnosis of Aspiration in ICU Patients

Dirk Bandorski, Khodr Tello, Harun Erdal, Janine Sommerlad, Jochen Wilhelm, Istvan Vadasz, Matthias Hecker, Dieter Walmrath, Werner Seeger, Ekaterina Krauss, Stefan Kuhnert

Summary: This study evaluates the diagnostic value of gastric biomarkers, such as pepsin and bile acid, in detecting aspiration in the intensive care unit (ICU). The results indicate that measuring pepsin and bile acid does not reliably diagnose aspiration or significantly impact survival outcomes. Further validation studies are needed to assess the clinical usefulness and reliability of these biomarkers in diagnosing aspiration-related conditions.

Summary: Persistent symptoms after COVID-19 infection are common, but not related to the severity of the initial infection. Reduced exercise capacity is mainly seen in patients who required ICU care, possibly due to deconditioning or fatigue rather than cardiopulmonary impairment. Persistent cardiopulmonary impairment is uncommon.
