Summary: This study found that screening neuroimaging in patients with infective endocarditis (IE) can increase detection of septic emboli, but only territorial infarcts (in contrast to septic emboli) are associated with an unfavorable discharge outcome.

Summary: This study compares the characteristics and outcomes of infective endocarditis (IE) patients with and without brain abscesses (BA), as well as evaluates the MRI characteristics of BA. The results show that BA-IE patients have higher rates of predisposing factors, bacterial meningitis, rare pathogens, and extra-cerebral abscesses. MRI can help diagnose and manage BA patients.

Summary: The study focused on the alterations of structural and functional brain connectivity in neurologically asymptomatic patients with early-stage chronic kidney disease (CKD), finding significant differences in all measures of structural connectivity but not in global functional connectivity. The efficiency of the structural brain network was decreased in CKD patients, with significant differences in certain regions like the right insular cortex, right occipital pole, and right thalamus.

Summary: Cerebral imaging did not significantly change patient classification according to Duke criteria among those suspected of infective endocarditis. However, it did lead to the establishment of a new surgical indication for preventing embolic events in 20% of patients.

Summary: The study aimed to determine the clinical characteristics and echocardiographic features of patients with IE. The short- and long-term prognosis for IE was poor, with heart failure and septic shock as predictors for in-hospital and 1-year mortality. Staphylococcus aureus was the most common causative pathogen.

Summary: Infective endocarditis in heart transplant recipients presents with specific characteristics, with non-valvular involvement being common and valve involvement not having a predominant location. Three fifths of cases have a nosocomial or health care-related origin. The major pathogens include staphylococci, enterococci, and Aspergillus, with an in-hospital mortality rate of 17%.

Summary: This study describes the clinical features, diagnosis, and treatment outcome of patients with infective endocarditis in patients with a valve-tube ascending aortic graft. Male patients with staphylococcal infection were predominantly affected, and multimodality imaging played an important role in diagnosis and management. Conservative therapy may be a valid alternative in selected patients.

Summary: Infective endocarditis (IE) carries a high risk of vascular complications, which are correlated with increased mortality. The use of anticoagulation in IE patients remains controversial and challenging. An appropriate anticoagulation strategy is crucial for improving outcomes and requires a good understanding of the indication, timing, and regimen of anticoagulation in IE.

Summary: This study conducted a systematic review to summarize brain MRI findings in COVID-19 patients with neurological symptoms. The results showed that a considerable proportion of patients had neurological abnormalities detectable by MRI, with the most common findings being acute/subacute infarction, olfactory bulb abnormalities, white matter abnormalities, and cerebral microbleeds.

Summary: Congenital heart disease (CHD) patients with infective endocarditis (IE) have better survival rates compared to non-CHD patients. CHD patients are younger and have fewer comorbidities. Positive blood cultures for Streptococcus viridans and a higher prevalence of dental procedures were observed in CHD patients.

Summary: In patients with infective endocarditis, platelet reactivity is increased in those with Staphylococcus aureus (SA) infection. However, the use of aspirin can reduce this increased reactivity without increasing bleeding risk. More research is needed to confirm the potential benefits of aspirin in SA-positive infective endocarditis.

Summary: This study aims to delineate the specific coagulation patterns and their clinical consequence in patients undergoing cardiac surgery due to infective endocarditis. The findings show significantly altered coagulation profiles in patients with infective endocarditis, with concomitant hyper- and hypocoagulability. The incidence of bleeding complications and transfusion requirements were also increased in patients with endocarditis.

Summary: The introduction of transcatheter pulmonary valve implantation (TPVI) has greatly improved the management of right ventricular outflow tract dysfunction. Infective endocarditis (IE) is a serious complication of TPVI that affects valve durability and patient outcomes. This article provides a comprehensive review of the current evidence on TPVI-IE, including its incidence, risk factors, causative organisms, diagnosis, and treatment.

Summary: This study examined the impact of transoesophageal echocardiography (TEE) on therapeutic management and mortality in elderly patients with infective endocarditis (IE). The results showed that patients who underwent TEE had better therapeutic management and lower mortality rates, indicating the influence of geriatric status on treatment decisions.

Summary: Reported the outcomes of patients after transcatheter mitral valve implantation (TMVI) using balloon expandable transcatheter aortic valves in high-risk patients with mitral bioprosthesis or annuloplasty ring failure or severe mitral annular calcification, classified into elective, urgent or emergent/salvage TMVI groups. Emergent/salvage TMVI was associated with high early mortality, but 1-month survivors had similar outcomes to patients with elective/urgent TMVI. The degree of urgency of the procedure should not prevent TMVI in high-risk patients.

Summary: This study aimed to assess the outcomes of patients with nonsevere aortic stenosis (AS) and heart failure undergoing transcatheter aortic valve replacement (TAVR) or medical management. The results showed that TAVR was a major predictor of superior survival compared with medical management. These findings highlight the need for randomized-controlled trials comparing TAVR versus medical management in heart failure patients with nonsevere AS.

Summary: Valvular heart disease (VHD) is a growing epidemic that affects millions of people globally and has a significant impact on healthcare systems. However, it has not received sufficient attention from the public and policymakers. Despite advancements in understanding and treatment, VHD is often underdiagnosed, detected late, and undertreated, leading to inequality in access to care and limited prevention options. This review article discusses the gaps in VHD management and proposes potential actions for improving patient outcomes.

Summary: This study aims to investigate the prevalence, clinical characteristics, and outcomes of patients with or without isolated or combined mitral regurgitation (MR) and tricuspid regurgitation (TR) in heart failure (HF) patients. The study found a relatively high proportion of patients with isolated or combined MR and TR, and these patients had different baseline characteristics. It was also found that HF patients with TR had a higher risk and poorer prognosis.

Summary: This study describes the perioperative management and short-term outcomes of carcinoid heart disease (CHD) in patients with liver metastases of midgut neuroendocrine tumors (NETs). The findings suggest a high postoperative mortality rate and a worse outcome in patients with baseline right-sided heart failure.
