Intrathecal Gadolinium-Enhanced MR Cisternography in the Evaluation of CSF Leakage

BACKROUND AND PURPOSE: Radiologic identification of the location of the CSF leakage is important for proper surgical planning and increases the chance of dural repair. This article describes our experience in analyzing clinically suspected cranial CSF fistulas by using MR imaging combined with the intrathecal administration of a gadolinium-based contrast agent. MATERIALS AND METHODS: A total of 85 consecutive patients with suspected CSF fistulas who presented with persistent or intermittent rhinorrhea or otorrhea lasting for more than 1 month between 2003 and 2007 were included in this study. RESULTS: We observed objective CSF leakage in 64 of 85 patients (75%). The CSF leak was located in the ethmoidal region in 37 patients (58%), in the superior wall of the sphenoid sinus in 8 patients (13%), in the posterior wall of the frontal sinus in 10 patients (15%), in the superior wall of the mastoid air cells in 6 patients (9%), and from the skull base into the infratemporal fossa in I patient (2%). Two patients (3%) showed leakage into >1 paranasal sinus. CONCLUSIONS: MR cisternography after the intrathecal administration of gadopentate dimeglumine represents an effective and minimally invasive method for evaluating suspected CSF fistulas along the skull base. It provides multiplanar capabilities without risk of radiation exposure and is an excellent approach to depict the anatomy of CSF spaces and CSF fistulas,

Summary: The new Scepter Mini dual-lumen microballoon catheter shows promise in the balloon-assisted embolization of cerebrospinal vascular malformations. It can be used for various applications, including embolization, balloon-occlusion, diagnostic angiography, and navigation support, with a high rate of navigability and success in complete occlusion. Technical failures were minimal, and no complications related to the Scepter Mini were observed in the study.

Summary: Our study shows that endovascular treatment for ACF DAVFs is safe and effective, with high rates of complete occlusion, particularly when using the transvenous approach. Whenever feasible, the transvenous approach should be preferred as the first line strategy.

Does measurement of the jugular foramen diameter on MRI help to differentiate transverse sinus thrombosis from unilateral transverse sinus hypoplasia?

Summary: The study aimed to differentiate transverse sinus (TS) thrombosis from TS hypoplasia by establishing a cutoff value based on the ratio of bilateral jugular foramen (JF) diameters on MRI. Results showed that the JF diameter ratio was lower in patients with hypoplasia compared to those with thrombosis. Calculating the JF diameter ratio can be an efficient method to differentiate between TS thrombosis and hypoplasia.

Summary: This study retrospectively analyzed data from multiple international academic institutions to investigate the change in complication rates and treatment outcomes for posterior circulation aneurysms treated with flow diversion. The results showed a trend towards treating smaller vertebral artery aneurysms, fewer large vertebrobasilar aneurysms, and an increased usage of a single flow diverter, leading to a decrease in the rate of thromboembolic and hemorrhagic complications.

Summary: This study aimed to investigate the safety and short-term efficacy of the new FRED X flow diverter. The FRED X was successfully implanted in all procedures, and the rate of major adverse events was 3.1%. Thrombotic events occurred in 4.3%, with only 1.2% leading to major adverse events. The rate of complete aneurysm occlusion after a mean follow-up of 7.0 months was 66%.
