Subjective Symptoms after Treatment of Early Lyme Disease

BACKGROUND: Controversy exists over the significance and even the existence of post-Lyme disease symptoms because of the high rate of similar background symptoms in the general population. METHODS: A European, prospective clinical trial in which doxycycline and cefuroxime axetil were compared in the treatment of adult patients with erythema migrans included a control group to address this question. Evaluations of patients were conducted at baseline, 14 days, and 2, 6, and 12 months after enrollment. Control subjects were evaluated at baseline and at 6 and 12 months. Subjective symptoms that newly developed or intensified since the onset of erythema migrans or the (late of enrollment for controls were referred to as new or increased symptoms. RESULTS: Doxycycline and cefuroxime axetil had comparable efficacy. At both 6 and 12 months, the frequency of new or increased symptoms in patients with erythema migrans did not exceed the frequency of such symptoms ill a Control group of individuals of similar gender and age without a clinical history of Lyme disease. At 12 months after enrollment, only 5 (2.2%) of 230 evaluable patients reported new or increased symptoms, and in none of die patients were these symptoms of sufficient severity to be functionally disabling. CONCLUSION: No significant differences were identified between doxycycline and cefuroxime axetil in the treatment of European patients with erythema migrans. The frequency of nonspecific symptoms in patients did not exceed that of a control group at >= 6 months after enrollment. We advocate inclusion of appropriate non-Lyme disease control groups ill future studies in which nonspecific Subjective symptoms are assessed after antibiotic therapy. (c) 2010 Elsevier Inc. Systemic immune responses in patients with early localized or early disseminated Borrelia afzelii lyme borreliosis

Summary: This study did not find significant differences in systemic immune responses represented by selected cytokines or chemokines in patients with early Lyme borreliosis, suggesting that systemic mediators are not pivotal in the pathogenesis of dissemination of the infection. Localized immune responses in the skin or other pathogenetic mechanisms may be more important in this regard.

Acrodermatitis chronica atrophicans: clinical and microbiological characteristics of a cohort of 693 Slovenian patients

Summary: Background information on patients with acrodermatitis chronica atrophicans (ACA) is limited. The study assessed clinical and microbiological characteristics of ACA patients at a single medical center, finding that the majority of patients had skin lesions on extremities, showing reddish-blue discoloration, swelling, and thinning of the skin. Most skin samples isolated Borreliae, with the majority being Borrelia afzelii.

Summary: This retrospective cohort study of 311 adults with Lyme neuroborreliosis (LNB) allocated according to diagnostic certainty found that less than half of the cases were definitively diagnosed, with the most frequent manifestation being cranial neuropathy. Patients with definite LNB more often presented with Bannwarth syndrome, had more severe disease, longer pre-treatment duration, and higher Borrelia seropositivity compared to those with possible LNB. A better diagnostic approach is needed to confirm borrelial etiology.

Legionella pneumophila-Epidemiology and Characterization of Clinical Isolates, Slovenia, 2006-2020

Summary: Legionella pneumophila is the causative agent of severe Legionnaires' disease (LD), and there is limited data on the molecular epidemiology in Slovenia. A retrospective study from 2006 to 2020 found 4.6% of pneumonia patients in Slovenia tested positive for L. pneumophila infection, with the most frequent sequence types being ST1 and ST23. Sequence-based typing identified 33 different sequence types, showing a high diversity among isolates.

Detection, identification and genotyping of Borrelia spp. in ticks of Coastal-Karst and Littoral-Inner Carniola regions in Slovenia

Summary: The density and spread of tick vector species in Europe have increased in the past 30 years, resulting in an increase in Lyme borreliosis cases in Slovenia. This study aimed to isolate and identify Borrelia strains in Ixodes ricinus ticks collected in two regions of the country and determine the prevalence of B. burgdorferi sensu lato and B. miyamotoi. The results showed a high prevalence of Borrelia spp., with B. garinii subtype Mlg2 being the most abundant.
