Summary: This systematic review aims to identify the key factors that influence the selection of conservative management for end-stage renal disease. The study found that patient-specific factors, clinician-specific factors, organizational factors, and national and international factors all play a role in the decision-making process. Improving education and generating stronger evidence are crucial for better guidance and recommendations for clinicians.

Summary: The study retrospectively reports on RAI treatment in thyroid cancer patients with ESRD undergoing dialysis, highlighting the challenges posed by delayed biological clearance of RAI. Adjusted dose activity and balancing scheduling between RAI therapy and dialysis are recommended to ensure closer radiation exposure in ESRD patients. Pooling data from multiple centers is suggested to establish consensus and substantiated recommendations for RAI treatment in ESRD patients.

Summary: Experienced End-Stage Renal Disease Seamless Care Organizations (ESCOs) performed better under the Comprehensive End-Stage Renal Disease Care (CEC) Initiative, while ESCOs located in more affluent areas and smaller ESCOs also showed better performance. The number of dialysis facilities and years of experience in the CEC Initiative were also associated with ESCOs' performance.

Summary: End stage renal disease (ESRD) has detrimental effects on the immune system, leading to quantitative alterations of lymphocyte subpopulations. Using principal component analysis (PCA) and uniform manifold approximation and projection (UMAP), we found that ESRD patients have a more senescent immune phenotype, and dialysis prescription may play a role in determining the immunological groups within the patient cohort.

Summary: A study using Taiwan's NHIRD found that statin treatment did not effectively reduce cardiovascular adverse events in young adult ESRD patients aged 20-40 requiring dialysis. Patients receiving statin therapy showed higher rates of major adverse cardiac and cerebrovascular events compared to those who did not, indicating statin therapy may not provide protective effects in this specific group.

Summary: We conducted a meta-analysis to determine the association between HLA alleles and ESRD. The results showed no significant association between HLA-B*50, HLA-DQA1*3, B*40, DRB1*12, DRB1*13, and DQA1*6 alleles and ESRD. Further studies with larger sample sizes and adjustments for confounders are needed to confirm these conclusions.

Summary: This study highlights the significant impact of HCAIs on ESRD patients undergoing RRT, affecting both their survival and treatment outcomes. Factors such as advancing age, low hemoglobin concentration were associated with death, while recurrent HCAIs, use of central venous catheters were risk factors for treatment failure.

Summary: The available data suggest that in selected older patients, non-dialytic conservative care has the potential to achieve similar health-related quality of life and symptoms compared with a dialysis pathway. High-quality prospective studies are needed to confirm these provisional findings.

Summary: A significant proportion of patients with abdominal and thoracic aortic aneurysms do not receive intervention after reaching the treatment threshold, leading to high mortality. This retrospective multicenter cohort study investigated the end-of-life care received by conservatively managed patients and found low rates of palliative care consultation and advance care planning.

Summary: Older patients with advanced CKD are at high risk for complications and death, yet discussions on advance care planning (ACP) with kidney clinicians are rare. Study found discordant views between patients and clinicians on ACP, with cultural and religious barriers. Four themes influencing ACP discussions emerged, leading to insufficient ACP discussions with older adults with advanced CKD.

Summary: Despite the potential for hospice to improve care for those at the end of life, it is underutilized for patients with ESLD. Increasing the appropriate utilization of hospice for ESLD requires a better understanding of patient eligibility, which can be based on predictors of high short-term mortality and liver transplant ineligibility. Such hospice criteria should be data-driven and should accommodate the uncertainty faced by patients and physicians.

Summary: This study compared the incidence of adverse cardiovascular events in older adults with primary hyperparathyroidism treated with parathyroidectomy versus nonoperative management. The results showed that parathyroidectomy was associated with a lower incidence of cardiovascular events compared to nonoperative management, which is relevant for patients with a long life expectancy.

Summary: This study aimed to assess the range and variability of cancer outcomes in trials involving kidney transplant recipients. The results showed that cancer is an infrequently reported outcome and is inconsistently defined in trials of kidney transplant recipients. Consistent reporting of cancer outcomes would provide important information on the impact of cancer in patients after kidney transplantation.

Summary: This study aimed to explore patients' perspectives on incremental hemodialysis (HD) and their priorities and concerns regarding HD. The study found that patients with kidney failure preferred incremental initiation of HD to minimize disruption to daily living and improve quality of life. The top prioritized outcomes were quality of life, residual kidney function, and mortality.

Summary: This study explored the perspectives and experiences of self-monitoring of blood pressure in patients with hypertension. The findings revealed that self-monitoring of blood pressure can empower patients, provide reassurance and convenience, and complement medical diagnosis and treatment. However, there are challenges such as inadequate knowledge and understanding of blood pressure targets and interpretation, limited access to monitoring devices, and psychological burdens associated with self-monitoring.

Summary: The study revealed that COVID-19 exacerbated vulnerability and distress among individuals with chronic kidney disease and their caregivers, leading to uncertainty in accessing healthcare and reduced capacity for self-management. Optimizing telehealth services and providing more educational and psychosocial support may help improve self-management and the quality of care for this population during a pandemic.

Summary: This study analyzed data from the Australia and New Zealand Dialysis and Transplant Registry and found that the initiation of kidney replacement therapy (KRT) in children and young adults has been happening earlier over time. However, earlier initiation of KRT did not affect patient survival. Clinical practice variation played a significant role in determining the timing of KRT initiation.

Summary: This study aimed to assess the reliability, acceptability, validity, and responsiveness of the PedsQL and the CHU9D in measuring health-related quality of life (HRQoL) among children and adolescents with overweight and obesity. The results showed that both instruments demonstrated good internal consistency reliability and high acceptability. However, PedsQL showed better known groups validity and responsiveness compared to CHU9D. In conclusion, PedsQL and CHU9D have good psychometric properties in measuring HRQoL in pediatric overweight and obesity.

Summary: This study aims to quantify the financial impact and out-of-pocket expenditure of rural adult patients with chronic kidney disease (CKD) in Australia. The survey results showed that the mean out-of-pocket expenses were 5056 AUD annually, and 78% of households experienced financial hardship, with 54% classified as experiencing financial catastrophe. Additionally, some participants had to relocate for a period greater than 3 months to access care, raising concerns about equity in Australia.

Summary: There is a gender disparity in access to kidney transplantation, with women being 20% less likely to receive a transplant compared to men across different backgrounds and socioeconomics. Nephrologists perceive that this disparity is exacerbated by gender norms, stigma, prejudice, and educational and financial disadvantages faced by women.

Summary: The study demonstrates that intensive treatment for hypertension can slow the progression of cerebral white matter hyperintensities and is associated with changes in functional brain connectivity. Specifically, the auditory-salience-language network showed a significant decline in connectivity score in the standard treatment group compared to the intensive treatment group. Additionally, changes in the mean connectivity score of the default mode network correlated significantly with changes in white matter hyperintensities.
