Peritoneal Dialysis-First Policy Made Successful: Perspectives and Actions

Peritoneal dialysis (PD) represents an important but underused strategy for patients who are beginning dialysis treatment worldwide. The development of a health care model that encourages increased use of PD is hampered by a lack of expertise and absence of pragmatic strategies. This article provides a brief review of a PD-first initiative that was implemented in Hong Kong more than 25 years ago and issues related to this policy. Clinical studies and research by the authors' and other teams around the world have shown evidence that, as a home-based dialysis therapy, PD can improve patient survival, retain residual kidney function, lower infection risk, and increase patient satisfaction while reducing financial stress to governments by addressing the burden of managing the growing number of patients with end-stage renal disease. Achieving a successful PD-first policy requires understanding inherent patient factors, selecting patients carefully, and improving technique-related factors by training physicians, nurses, patients, and caregivers better. Dialysis centers have the important role of fostering expertise and experience in PD patient management. Dialysis reimbursement policy also can be helpful in providing sufficient incentives for choosing PD. However, despite successes in improving patient survival, PD treatment has limitations, notably the shortcoming of technique failure. Potential strategies to and challenges of implementing a PD-first policy globally are discussed in this review. We highlight 3 important elements of a successful PD-first program: nephrologist experience and expertise, peritoneal dialysis catheter access, and psychosocial support for PD patients. The change in the prevalence of obesity and new-onset diabetes in Chinese peritoneal dialysis patients over 25 years

Summary: The prevalence of obesity has significantly increased in incident Chinese peritoneal dialysis patients in Hong Kong over the past 25 years, regardless of diabetes status. New PD patients with preexisting obesity or overweight have a higher incidence of new-onset diabetes compared to those without obesity. Further studies are needed to explore the prognostic implications and impacts on healthcare systems.

Summary: This study investigates the epidemiology and outcomes of COVID-19 patients with hyponatremia. The results show that hyponatremic patients have higher 30-day mortality, prolonged hospitalization, and increased ICU admissions. Hyponatremia may serve as a prognostic indicator for unfavorable outcomes in COVID-19.

Impact of Weight Gain on the Subsequent Survival of New Peritoneal Dialysis Patients

Summary: This study examined the change in body weight of 954 consecutive PD patients during the first 2 years of treatment and analyzed its impact on subsequent clinical outcomes. The results showed that weight loss >5% was significantly associated with a worse patient survival subsequently, while weight gain had no significant impact on patient survival.

Urinary and Kidney Podocalyxin and Podocin Levels in Diabetic Kidney Disease: A Kidney Biopsy Study

Summary: Diabetic kidney diseases (DKD) are the most common cause of dialysis-dependent kidney disease worldwide. Previous studies have shown that the urinary levels of podocyte-associated molecules may predict the prognosis of DKD. This study found that intrarenal podocalyxin level was an independent predictor of dialysis-free survival, while urinary podocin level was associated with the rate of kidney function decline.

Omentin-1 Levels and Outcomes in Incident Peritoneal Dialysis Patients

Summary: This study investigated the prognostic role of plasma omentin-1 levels in incident peritoneal dialysis patients. The results showed that higher plasma omentin-1 levels were weakly correlated with fewer hospital admissions and shorter duration of hospitalization. However, there was no significant association between plasma omentin-1 levels and patient survival or technique survival.

Relationship between serial serum neutrophil-lymphocyte ratio, cardiovascular mortality, and all-cause mortality in Chinese peritoneal dialysis patients

Summary: This study found that serial monitoring of serum neutrophil-lymphocyte ratio (NLR) is associated with cardiovascular (CV) burden and all-cause mortality in peritoneal dialysis (PD) patients. Patients with high baseline NLR and persistently elevated NLR values had higher all-cause mortality. However, NLR did not show a significant relationship with CV mortality. Regular monitoring of serial serum NLR may help identify patients at risk of adverse outcomes.

ISPD Catheter-related Infection Recommendations: 2023 Update

Summary: Peritoneal dialysis (PD) catheter-related infections are significant risk factors for catheter loss and peritonitis. The 2023 updated recommendations provide revised definitions and classifications of exit site infection and tunnel infection. The new target for the overall exit site infection rate is recommended to be no more than 0.40 episodes per year at risk. Changes have been made to the recommendation of topical antibiotic cream or ointment, and new recommendations include clarified suggestions for exit site dressing cover and updated antibiotic treatment duration with emphasis on early clinical monitoring. Additional catheter interventions such as external cuff removal or shaving and exit site relocation are also suggested.

Summary: The study aimed to evaluate the safety and virological outcomes of nirmatrelvir-ritonavir in patients with chronic kidney disease. The results showed that the modified dosage of the drug is well-tolerated and effective in suppressing the SARS-CoV-2 viral load in mild COVID-19 cases among patients with chronic kidney disease.

Summary: Peritoneal dialysis is an important treatment option for patients with kidney failure, providing various benefits such as enhanced participation in daily activities, flexibility in schedules, cost savings, and improved quality of life. However, the utilization of peritoneal dialysis has remained static or declining in certain regions due to concerns about PD-related infections. This review aims to provide an overview of the definition, epidemiology, risk factors, prevention, and treatment of PD-related infections based on current evidence.

Summary: The levels of GSK3 beta were higher in the kidneys and urine of DKD patients compared to non-diabetic CKD patients. However, only the ratio of pY216-GSK3 total GSK3 in the kidneys was associated with the progression rate of DKD. Further studies on the pathophysiological roles of GSK3 beta in kidney diseases are needed.
