Summary: Therapeutic drug monitoring (TDM) in voriconazole therapy is crucial for determining treatment range and efficacy/toxicity relationships, while CYP2C19 polymorphisms significantly affect voriconazole metabolism with unclear implications for efficacy/safety. Further large-scale trials are needed to investigate the role of prospective genotyping and establish CYP2C19-guided voriconazole dosing recommendations.

Direct Oral Anticoagulant Use in Chronic Kidney Disease and Dialysis Patients With Venous Thromboembolism: A Systematic Review of Thrombosis and Bleeding Outcomes

Summary: DOACs and warfarin showed no significant difference in reducing recurrent VTE and bleeding events in moderate CKD patients, but the risk of overall major bleeding increased with higher degree of kidney impairment. There is controversy over whether DOACs may be more beneficial than warfarin in CKD/ESKD patients with VTE, and evaluation should be individualized.
